Primary Function:

Knowledge and understanding of cultural diversity and the impact of various socioeconomic characteristics on our youth
Must demonstrate sensitivity to Elk Hills service populations cultural and socioeconomic characteristics

POSITION SUMMARY:
A Mental Health Counselor has specialized therapeutic training in the field of trauma and child abuse to support severe cases of mental health issues in our schools. Students who would be candidates for their services include: children with significant trauma and/or abuse histories who are unable to be productive members of the classroom because of the severity of their symptomatology, students who are expressing suicidal or homicidal ideation, students with disruptive behaviors and no known etiology, students who have witnessed violence, students who are sexually reactive, or students having signs of a serious mental illness, including but not limited to, psychotic features. The Mental Health Counselor works with identified students and their families to provide best practice wrap-around mental health services. Collaborates with outside services and sets up appropriate resources for family access. The goal is to provide short term therapeutic assessment with interventions to stabilize and cessate maladaptive behaviors. QUALIFICATIONS:
Holds a master's degree, from an accredited college or university, in counseling or in a human services related field and is licensed or eligible for a license.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications:
A minimum of a Bachelor's Degree in human services, psychology, or a related field is required for this position.
Two years' experience working with youth or families.
Current certification in Family Centered Treatment is preferred for candidates aspiring to join our team.
Additional state specific requirements include:
NC, VA- QMHP or ability to register as a QMHP
MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.

The Peer Counselor must be available to work 10 hours per week between 8:00 a.m. and 5:00 p.m. Monday through Friday. There is also the potential for full-time summer employment.
Duties and Responsibilities:
Provide students, parents, and the general public with professionally and courteously accurate information and instructions regarding the financial aid process. The WWRC Rehabilitation Counseling Department is looking for a qualified individual to serve as a vocational rehabilitation counselor. The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.

Job Description Join Our Dynamic and Caring Family! For this position you Must be a Registered Medication Aide “RMA” or Licensed Nurse. Be able to provide oversite, direct care services, assistance and support to residents and administration while providing a conduit between administration, direct care, medication aides and physicians to facilitate best care practices, service standards, community philosophy, and its mission.
Some of this Position's Responsibilities Include:
Schedule nursing/direct care staff and facilitate coverage needs.
Direct, evaluate and supervise all RMAs and direct care staff and report to administrator satisfactory and unsatisfactory performance.
Maintain confidentiality of all resident information.
Assure nursing forms needed for documentation are copied and available.
Assist with education and in-service training to nursing staff.
Interpret existing policy and procedure to nursing staff, residents, families, and physicians.
Assist new nursing employees in following facility policies and procedures.
Coordinate Physician/NP site visits with prepared listing of resident issues/concerns or needs and follow up to facilitate efficient provider care and resident well-being, etc.
Review new orders to ensure orders, eMAR, and medications match and are complete.
Review documentation on resident admitted and compare physician orders with each resident's plan of care to assure appropriate implementation of the physician's orders and plan of care is occurring.
Maintain resident's charts in appropriate location/file.
Audit medication carts monthly and as needed.
Create, implement and review Individual Service Plans (care plans or ISP) per regulations, routinely.
Complete “Uniform Assessment Instrument” (UAI) form.
Assist with the admission and discharge process of residents.
Report changes in condition of residents to administrator and physician.
Answer business phones and nurse call bells and respond to alarms as required.
Tour inquiring individuals or families when administrator or marketing/admission staff are not in facility.
Team Effort:
Develop and update each resident care plan/” Individualized Service Plan” (ISP) and UAI.
Assist in ensuring compliance with State and Local Regulations.
Follow instructions provided by the Administrator and provide appropriate feedback often.
Work with Administrator on strategies to improve quality of care and service to residents.
Work with the Administrator to develop training for staff and staff recognition, where appropriate.
Work with the facility Team leaders to provide consistent treatment and messaging to all staff, residents, and resident families for stability of the workplace.
Action:
Identify resident issues and communicate them to the Administrator and the resident's physician.
Identify staff issues and communicate them to the Administrator.
Keep the environment safe for residents, staff and guests.
Respond to resident and facility emergencies; provide first-aid assistance and arrange for appropriate medical attention and follow-up.
Practice safety in working around and with equipment, chemicals, tools, and utensils.
No horse play or violation of the facility code of conduct.
Success:
Contribute to resident satisfaction by ensuring that customers' expectations are met or exceeded.
Maintain efficient and appropriate communication with residents, family members and guests.
Contribute to marketing efforts through appropriate interaction with prospective residents and guests.
