Job Summary:
Work is active both in the community and in a group home setting. Contributing to the development, implementation, monitoring, and updating of the individualized rehabilitation plan; performing therapeutic rehabilitative skill development; offering services appropriate to meet the consumers' needs; performing crisis intervention, and coordinating and managing services. Promote independence and overall health and safety of consumers in CODI group homes.


Required:
Must be a minimum of 21 years of age
Must obtain and retain CPR and First Aid Certification within ninety (90) days of hire or transfer
Valid New Jersey driver's license with less than 5 points
Bachelor's degree from an accredited college or university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice. Staff with a bachelor's level degree from an accredited college or university in a field other than a helping profession shall have a minimum of two years of professional experience in a community behavioral healthcare setting.
Certification as a psychiatric rehabilitation practitioner (CPRP) may be substituted for one year of experience.
Must be able to pass a drug test, including non-use of marijuana
Must provide proof of vaccination against COVID-19 or documented religious or medical exemption to apply


Essential Functions:
Work efficiently with people from different cultures and backgrounds and show genuine respect for others who are different
Educate consumers on Illness Management, including benefits of medication compliance, problem-solving techniques, and coping strategies
Provide input towards the Individualized Rehabilitation Plan (IRP) and assist the consumer in setting and attaining goals
Transport consumers as required
Maintain 65% of the time in face-to-face contact with assigned CSS consumers in the community based on Individualized Rehabilitation Plan goals and objectives
Provide training in the development of independent living skills as designated in IRP
Provide support in planning, shopping for, and preparing nutritious and well-balanced meals
Promote community integration and restoration of maximum possible functional level by improving and/or developing social, interpersonal, problem-solving, and communication skills
Assist consumers with accessing, navigating, renewing, and providing linkages to public entitlements
Prepare and maintain required Medicaid documentation
Provide on-call support after regular business hours
Engage consumers in social and/or recreational activities
Assist consumers in maintaining cleanliness and safety of home
Be capable of climbing /descending stairways to respond to consumer needs
Be capable of assisting consumers with evacuating Group Home in 180 seconds
Fully cooperate with the Department of Human Services during any investigation

As a Residential Counselor, you will:
· assists residents with personal care (bathing, dressing, and eating);
· drive them to appointments, errands, and to community events;

· help prepare meals, clean, do laundry, and help keep the home pleasant and comfortable.

Provides support, assistance, psycho-social education and supervision to Adult Residential Services residents. Will assist 16 adults. Responsible for cooking and assisting with daily activities. This position is PRN.

Job Responsibilities:
Supervise and monitor residents' compliance with Program rules and behavioral expectations.Re-direct residents on non-compliance with rules and expectations and refer major incidents of non-compliance to the Residential Coordinator.Supervise, educate on and monitor residents' self-medication procedures and document in residents' charts. Is responsible for keeping file and closet locked while on duty.Review referral information as it is presented to help determine eligibility for admission and continued stay for Supported Housing and Crisis Respite where relevant Complete authorizations for stay as needed with each Managed Care Company Supervise meal preparation and compliance of residents with dietary guidelines, providing education as needed.Receive and dispense residents' funds from locked filing cabinet in office as needed and document all money received and given to residents in residents' charts.Inform the Residential Coordinator of any needed household supplies or household repairs.Plan and supervise psycho-educational activities recreational activities during residents' free time and provide van transportation on outings.Inform Residential Coordinator of all medical emergencies, crisis situations with residents or any disruptions in program operation.Receive briefing at beginning of shift from Residential Coordinator or other staff and provide briefing to other staff at end of shift.Document a progress note on each resident once per shift and at any time there is a significant incident.Attend all Program staff meetings and all scheduled staff development activities.Conduct room inspections as needed to monitor and educate on cleanliness of rooms, residents' compliance with hygiene/grooming and household task completion.Report all resident grievances to Residential Coordinator.Perform other duties as assigned.
