    FT Case Manager/Mitchell, SD *Remote RHD South Dakota Case Management is seeking a full time case manager in the Mitchell, SD area. This position is full time, 40 hours per week. This is a professional level position that will have the ability to work from home; however, in-state travel is required in caseload coverage area. The overall responsibility of this position will be administering the CHOICES waiver and CTS program, and coordinating services for adults and children who have an intellectual or developmental disability. This includes, but is not limited to, coordinating services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Case Managers will provide support to individuals to actively engage in and direct the person centered plan development process. Essential Duties and Functions: Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective. Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant. Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources. Facilitate team process for the initial and annual development of the Individual Service Plan (ISP) Ongoing monitoring of ISP and documenting results and intervene as necessary Report and review Critical Incident Reports (CIR) per protocol Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate Focus on Personal Outcome measures Knowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning Evaluate the needs of those on caseload in order to remain successful in the community Link those on caseload to programs and supports that are needed and requested to meet their needs Complete required documentation of services, including input of data, reports, and plans into Therapy Qualifications: Bachelor's degree and one year of experience OR equivalent combination of education and experience. Inform all parties of their participants' treatment progress, including arrests or any other problematic behavior that might jeopardize their status with Treatment Court. * Prepare summaries about specific participants' progress on selected Wednesdays and present summaries at the bar of the court on those selected Wednesdays. * Ensure the delivery of comprehensive services to assigned participants in the areas of housing, education, vocation, HIV, and recovery-oriented community services to 40 to 50 individuals. * Maintain written documentation of services in each participant's record/chart in accordance with agency standards, managed care insurance providers, and licensing requirements. Relevant documentation can include telephone calls, phase progressions, and updates from status listings. * Prepare graduation packets for participants completing Phase IV on the Thursday before the graduation ceremony. * Provide monthly caseload statistics, as well as submissions for potential graduates for the following month, on the Friday following each monthly graduation ceremony. * Assist participants with enrollment in Department of Public Assistance services. * Develop intra- and inter-organizational linkages, fostering collaborative relationships with various programmatic entities. * Conduct urine drug screens for Treatment Court participants. * Meet regularly with participants in treatment facilities and recovery houses. Follow up on court-ordered sanctions or requests and phase calculations. * Check email and telephone voicemail throughout the workday. The Case Management unit will be available to participants, families, treatment facilities, judge, and legal team Monday through Friday, 8:30 A.M. to 5:00 P.M. * Consistently assess each assigned participant's progress during individual sessions using ASAM standards to ensure their progress is accurately reflected at the ASAM criteria's level of care. * Perform all other duties as assigned.     SERVICE AREA: Housing & Justice Related Services (HJRS) PROGRAM: Philadelphia Treatment Court (PTC) Case Management ROLE TYPE: Full-Time The Bilingual Treatment Court Case Manager will coordinate supportive services for a caseload of individuals in Philadelphia Drug Treatment Court. Language Requirement: Must be fluent in Spanish and English Reports To: Adult Treatment Court Case Management Supervisor RESPONSIBILITIES: * Provide case management services to individuals or participants with current forensic affiliations who abuse illicit psychoactive substances. * Supervise each participant once per month, or more if clinically necessary, at the Treatment Court Case Management Office (1401 Arch Street) to assist participants in meeting their recovery needs. * Serve as a liaison between the treatment facility and the legal team, including the Treatment Court Judge, Assistant District Attorney, and Assistant Public Defender. Inform all parties of their participants' treatment progress, including arrests or any other problematic behavior that might jeopardize their status with Treatment Court. * Prepare summaries about specific participants' progress on selected Wednesdays and present summaries at the bar of the court on those selected Wednesdays. * Ensure the delivery of comprehensive services to assigned participants in the areas of housing, education, vocation, HIV, and recovery-oriented community services to 40 to 50 individuals. * Maintain written documentation of services in each participant's record/chart in accordance with agency standards, managed care insurance providers, and licensing requirements. Utilize the DAP format (Data, Assessment, Plan). * Document interactions in the CRS and PAJCIS databases. Relevant documentation can include telephone calls, phase progressions, and updates from status listings. * Prepare graduation packets for participants completing Phase IV on the Thursday before the graduation ceremony. * Provide monthly caseload statistics, as well as submissions for potential graduates for the following month, on the Friday following each monthly graduation ceremony. * Assist participants with enrollment in Department of Public Assistance services. * Develop intra- and inter-organizational linkages, fostering collaborative relationships with various programmatic entities. * Conduct urine drug screens for Treatment Court participants. * Meet regularly with participants in treatment facilities and recovery houses. Follow up on court-ordered sanctions or requests and phase calculations. * Check email and telephone voicemail throughout the workday. The Case Management unit will be available to participants, families, treatment facilities, judge, and legal team Monday through Friday, 8:30 A.M. to 5:00 P.M. * Consistently assess each assigned participant's progress during individual sessions using ASAM standards to ensure their progress is accurately reflected at the ASAM criteria's level of care. * Perform all other duties as assigned. This will be specific to each family, but should be an ongoing process during each case. * Complete reports including a detailed background, including names and DOBs for all kids being served, the reason for placement or system involvement, situation of bio parents, brief case history, current placement details, court history, and identification of program court-ordered status (if applicable). * Complete timely supervision notes that include: case background, updated family list, notes about communication with the team, progress on the case AND identified next steps. Steps from the previous note should be completed and identified as completed. * Adhere to all standards for confidentiality. * This is a hybrid position.     SERVICE AREA: Housing & Justice Related Services (HJRS) PROGRAM: Forensic Intensive Recovery (FIR) The FIR Case Manager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. AMP is a diversion program which assesses social determinants of health for program participants and provides them with an alternative to other legal sanctions.
