Working in the community, providing supportive mobile crisis intervention to children, adolescents and adults. Providing resources and support, diverting hospitalizations and police involvement. Administrative Complete all documentation as required by RHD, Funder, licensing/accreditation entities. Community Interaction Community Involvement and teamwork including but not limited to providing referral and other case management services for consumers and families at times of contact, either on the phone, in the community, or with the resident prior to discharge. Other duties as assigned Qualifications Bachelor's degree preferred with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, education or a related field. Must have a high school diploma or equivalency and 12 semester credit hours in sociology, social welfare, psychology; gerontology; or other social sciences. 2 years of experience in public or private human services with one year of mental health direct care experience. Working conditions This position is set in a residential or community environment. While performing the duties of the job, employees are frequently required to stand, walk, lift, and reach.     SERVICE AREA: Housing & Justice Related Services (HJRS) PROGRAM: Forensic Intensive Recovery (FIR) The FIR Case Manager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements. This individual will provide support to the participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community. Reports to: FIR Case Management Supervisor RESPONSIBILITIES: Responsibilities include, but are not limited to, the following: * Provide case management services to a chemically dependent forensic population, with a caseload of up to 45 active participants. * Establish and maintain service productivity goals at minimum designated service units per week. * Document service provision within 24 hours. * Participate in monthly team meetings and scheduled case conferencing with Probation, meeting reporting requirements. * Participate in biweekly supervision meetings. * Collaborate and attend meetings with Probation/Parole, treatment providers, and other collaborating agencies. * Complete discharge summaries within timeframes specified by the program. * Assist individuals to link to recovery support services, including PRO-ACT recovery center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports. * Create weekly schedules detailing activities to be completed for each week. * Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements. * Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services. * Identify, coordinate, and link community resources to each participant to promote accessibility to recovery-oriented community services. * Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach. * Spend an average of 80% of each workweek in the field for off-site participant appointments and meetings. JOB OVERVIEW: The Mental Health Professional (MHP) serves as a primary therapist to the children and families participating in the program. JOB OVERVIEW: We are seeking a full-time substance abuse and mental health therapist to provide clinical treatment to PTR participants. RESPONSIBILITIES: * Complete biopsychosocial assessments. * Work with each participant and the treatment team to devise a comprehensive treatment plan based on the psychological evaluation and other assessments. * Conduct individual, group, and family therapy sessions. Work to ensure maximum client show rate. * Record individual and group progress notes in the DAP format after each therapeutic activity. * Maintain a continuous record of all contacts, events, and progress toward the accomplishment of goals as stated in the treatment plan.
