Job Description
Job Title: Dispensing LPN
Location: Resources for Human Development: Montgomery County Recovery: 316 DeKalb Street, Norristown, PA 19401
Salary: $29.83 - $31 hourly
Employment Type: PRN
Montgomery County Recovery Center (MCRC) is a licensed medication-assisted treatment program seeking two LicensedPracticalNurses (LPN) in Norristown, Pennsylvania. This program is part of the Resources for Human Development (RHD)/APIS non-profit organization.
About the Role
The primary role of a LicensedPracticalNurse (LPN)/Dispensing Nurse is the oversight, coordination, and direct intervention of any and all health-related issues directly or indirectly related to any and all participants of the MCRC program.
Hours: Part-time/PRN (approximately 5-10 hours per Week).
Schedule: Every other Saturday and PRN (as it suits your schedule) weekdays between 6:00 am and 10:30 am, making up to $31 per Hour. PRN shifts are dependent/determined by your availability!
Responsibilities
Have knowledge of the properties of methadone/buprenorphine and how it works in the body.
Identify and take appropriate action when client exhibits the sign of an inappropriate dose level.
Dispense methadone and buprenorphine. Observe the client before and during medication.
Ensure the client has consumed medication before leaving the dispensing area.
Be aware of the client's physical conditions, maintenance level, psychological and physiological effects as well as any stressors the client may be experiencing.
Inform the designated nurse of any medical supplies that are needed.
Maintain security of the methadone and dispensing area.
Assist the physician in conducting physical examinations as directed by the physician.
Enter new clients or medication changes into the dispensing system.
Verify client doses and eligibility for medicating on courtesy dosing clients.
Maintain open communication with medical staff.
Record medication other than methadone when ordered by the physician.
Provide emergency medical treatment.
As assigned by the Supervisor to ensure continuity of care and foster a supportive nurturing environment for growth.
Other duties as needed and assigned.
Requirements
LicensedPracticalNurse with current license in PA.
Prefer 1 year nursing experience in mental/behavioral health or addiction recovery field.
Physical Demands

Job Description:
Evaluate, observe, and assess individuals' physical health conditions.

SUMMARY OF JOB DESCRIPTION: PHMC is proud to be a leader in public health. Our homeless medical respite programs offer post-acute medical care to individuals released from a hospital stay following an illness, surgery or an injury, and are too ill or frail to recover in a shelter. Participants are provided with several services, including medication management, discharge planning, appointment transportation and scheduling, case coordination and connecting clients to primary care. At any given time, both medical respites can serve 20 individuals.
The primary responsibility of the LPN Coordinator is to serve as a liaison and multi-task both medical respite programs including Medical Respite on Cedar and Serenity Court Homeless Medical Respite, which is our oldest facility. Support administration and clinical operational processes in the Medical Respites. This individual will work closely with Medical Respite Staff to establish, develop, and maintain working relationships with current and potential referral sources, support on-site patient evaluations to identify long term care needs, and assist with housing arrangements. Medical Coordinator will work with the medical respite programs' leadership team and will serve as the ambassador for the program. This individual will adhere, and comply with all appropriate Standards for Medical Respite as outlined by the National Institute for Medical Respite Care
Reports to: Program Administrator
ESSENTIAL DUITES AND RESPONSIBILITIES:
* Provide on call nursing support including alternating weekends.
* Assists internal program audits including chart check.
* Review Medication Administration Record (MAR).
* Review Medication Disposal Record (WASTE).
* Support daily quality of patient care.
* Developing strong internal and external partnerships.
* Developing strong community partnerships.
* Support staff on all admissions and discharge's goal plan.
* Review medical respite criteria for admissions with referring entities.
* Review referrals working in collaboration with Nurse Supervisor and Case manager using established criteria to determine appropriateness for the program.

* Conduct pre-admission phone screening assessment and fax referral source to determine participant appropriateness for program.
* Complete weekly inventory for medical supplies

* Seek alerts and alternate insurance payment options.
* Maintain document and submit insurance payment.
* Assures the safety and comfort of all staff and patients.
* Assists with the scheduling of medical appointments.
* Assist with training and support to the Certified Nursing Assistants on their shift.
* Participates in professional trainings, webinars, etc.

The Public Health Nurse Home Visitor (Mabel Morris Program) (MM NHV) is responsible for providing comprehensive community nursing services to pregnant people and/or their families eligible for the Mabel Morris Family Home Visit Program (MM) in Philadelphia. The MM NHV maintains the highest standards in clinical practice and adheres to the Parents as Teachers (PAT) model. The PAT mission is to promote the optimal early development, learning, and health of children by supporting and engaging their families and caregivers. Bachelor's prepared registered nurse home visitors deliver the curriculum to enrolled families raising children under five years old. Nurse and family partner to implement evidence-based strategies connecting resources needed to meet goals for their children and family's holistic health and educational well-being. MM NHVs partner with Philadelphia families to meet their family's goals by implementing and maintaining fidelity to the PAT model. The MM NHV reports directly to the Public Health Nurse Supervisor (Mabel Morris) position and works in collaboration with other NHVs, administrators, and senior staff to build and improve the organization's strengths-based, client-centered culture. In-house topic experts and support staff support the NHVs.
All MM services are voluntary, non-clinical in nature, individualized, and focused on the expressed desires of the family. Services are family-centered, strengths-based, trauma informed, and meet the needs of families in a respectful and culturally appropriate manner. Our services are conducted in the home of the client, or if necessary or requested by the guardian, in another location such as a park or agreed-upon location. Visits may also be conducted virtually. Above all, playtime and fun are vital component to every visit with a family.
The Mabel Morris Family Home Visit Program is a program of the National Nurse-Led Care Consortium (NNCC), a nonprofit organization working to strengthen community health through quality, compassionate, and collaborative nurse-led care. MM NHVs split their time between NNCC's West Philadelphia office (4601 Market Street) and field work in the homes and communities of families served.
RESPONSIBILITIES:
* Complete all required PAT and NNCC education, including but not limited to Life Skills Progression, Bracken School Readiness, and Ages and Stages Developmental and Social Emotional Assessments.
* Provide regularly scheduled in-home visits and telehealth visits for clients.
* Build and maintain a caseload of 18-20 active families raising children under five years old within nine months of employment.
* Develop therapeutic relationships with clients utilizing concepts and processes from motivational interviewing, reflective listing, and program strategies.
* Plan home visits in accordance with client goals and PAT outcomes and documents all work appropriately.
* Support the achievement of client-identified goals through one-on-one education, active listening, screening, and referral processes to community resources.
* Support the policies, procedures, guidelines, and standards of PAT and NNCC.
* Adhere to ANA and PA nursing process and standards, code of ethics, HIPAA confidentiality standards, and the PAT model of home visitation.

The role of the Registered Nurse is to provide person-centered nursing care, through the promotion of health and welling being. The Registered nurse participates as team member within the interdisciplinary team (IDT), in the plan of care for consumers served by the agency. The RN is required to practice in accordance with the code of professional conduct as set out by the Connecticut Board of Examiners of Nursing (BOEN), and within the polices, guidelines, established by the Department of Developmental Service.
The Registered Nurse provides health and clinical oversight to individuals with intellectual Disability and comorbid conditions. The RN performs assessments of individuals in their care to actual or potential health problems, participates in the development and implementation of the plan of care, and reviews and evaluates the effectiveness of the plan on a continuing basis. The RN provides clinical oversight / direction and delegates responsibility for performance of certain nursing task/responsibilities to trained non-licensed staff according to the standards identified by the Connecticut Nursing Board and the Department of Developmental Service regulation. The RN implements the plan of care as ordered / executed by the direction of the licensed physician, dentist, Physician Assistance, or APRN.

DIRECT DUTIES:
Perform RN job duties in accordance with RHD policy.
The RN is responsible for the health care oversight of individuals residing in his/her current program assignment.
The RN shall help develop a plan of care in collaboration with assigned individuals and key interdisciplinary team members who support the individual in the assigned programs.
The RN participates as a team member in the multidisciplinary team to achieve a plan of care that is optimal for the consumer, which may include individual specific health recommendations.
The RN participates in scheduling medical appointments, participates in transition, and/or discharge plan of care.
The RN monitors the overall healthcare, promotes health and wellness, as well as prevention of illness. This may include but is not limited to:
Nursing assessment, planning and implementation of individual plan of care.
Individual specific education in the plan of care including medical orders and /or medical recommendations, areas of personal hygiene, key health areas, diet, family life and sex education, as needed.
Acts as a liaison to community medical providers to provide programmatic staff with education and support relating to the medical needs of the individual.
The RN may be required to attend medical appointments.

The RN provides health care oversight, which may include the monitoring of medical appointments, orders, and recommendations.
Ensure that relevant documentation of medical visits / follow-ups has been completed.
RN provides timely nursing documentation as per CT DDS regulation.
RN is responsible for direct oversight for the medication administration program for assigned programs.
Direct oversight includes Medication Administration as defined by CT Medication administration policy.
The RN should ensure the implementation of all nursing and medically related policies and guidelines are in place, which may include the implementation of task specific nurse delegation and procedures, to ensure that tasks are delegated safely.
The RN may participate in the development and revision of agency policies.
The RN participates in staff training annually and as determined by program needs. Staff training may include but is not limited to:
Individual Specific Nurse Delegation, Vital signs, Signs and Symptoms of Illness, Protocol and procedure, Infection Control and Individual Specific Medical Information.
RN may be required to function as a medical resource for assigned programs.
The RN acts as a direct liaison between medical providers, interdisciplinary team members and the programmatic staff.
The RN may be required to provide direct medical crisis support and /or support via phone, during regular business hours (8am to 5pm on Monday to Friday) and as assigned.
Ensure proper and effective utilization of emergency services when necessary.
The RN may provide dietary oversight as per Connecticut DDS safe eating and drinking guidelines to ensure proper food and fluid intake.
OTHER RESPONSIBILITIES:

Regulation set forth by the Department of Developmental Service, which bears direct oversight for homes that are licensed and funded by DDS.
