The Quality Assurance Coordinator has accountabilities for the monitoring, implementation and evaluation of process and programs to meet regulatory, accreditation and standards of practice.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
A. Responsible for the implementation and monitoring of the organization's quality improvement program.
a. Ensures that there are processes in place to monitor and measure all activities related to performance improvement.
b. Provides direction and coordination of quality improvement activities utilizing continuous quality improvement principles and methodologies.
c. Monitors the organization's Outcome Based Quality Initiative (OBQI) outcomes. Provides feedback and best practice standards to the Clinical Director to educate personnel to improve outcomes.
d. Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Data is analyzed and compared internally and externally with other sources when available.
e. Works collaboratively with Clinical Director to ensure documentation is complete and consistent with care and reflects legal requirements.
f. Member of the Patient Grievance and Corporate Compliance Committees and collaborates with management personnel in follow-up of concerns expressed by patients, families, physicians and the Division of Program Integrity following the regulatory requirements.
g. Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, as well as team development.
h. Interprets and implements quality assurance standards in Home Health Agency to ensure quality care to patients: reviews quality assurance standards, studies existing agency policies and procedures, and interviews office and some field personnel and patients to evaluate effectiveness of quality assurance program.
i. Auditing of client's charts and support the home healthcare agency's philosophy and standards to assure all are in compliance with State and Federal regulations.
j. Generates weekly incident reports and complaints, hospitalizations, infection and missed visit logs, communication notes and then review, investigate and send abnormal findings to the Executive Director and Clinical Manager.
k. Generate a monthly Plan of Care, Discharges and Employee Compliance reports and forward discrepancies to the various department heads and the Executive Director.
l. Reviews and evaluates 5-10% of high risk patients' medical records per month (100% Skilled charts, 80% EPD Waiver and 20% State Plan)
m. Selects a specific quality assurance project annually and work on it and write a report
n. Completion of the Plan of Correction post federal and state audits.
o. Compiles statistical data and writes narrative reports summarizing quality assurance findings every quarter.
p. Shall provide educational guidance, infection control, emergency preparedness plan and quality improvement plan to the clinical manager for personnel education.
q. Review and updates periodically Infection Control, Emergency Preparedness and Quality Assessment and Performance Improvement Programs.
r. Determine and document any areas that may need improvement and review and analyze the effectiveness of the modifications made.
s. Reviews and updates the current Policies and Procedures Manuals as needed and improvise plans to improve upon the existing quality standards.
t. Ensures accuracy & completeness of patient records.
u. Responsible for ensuring processes to monitor and evaluate safety, risk management and infection control programs.
v. Acts as resource to personnel in identifying safety and risk management issues for patients and personnel.
w. Responds to all Plan of Corrections for all Surveys in a timely manner.
x. Assumes the duties of the Clinical Manager if he/she is absent from work for a period of more than one week in reviewing all initial assessments and Plan of Cares and assignment of the patient to the nurse coordinators.
y. Conducts all federal and State Surveys with the Clinical Manager.
z. Attends DHCF weekly meetings and share the meeting minutes with all departments concerned.
B. Collaborates with the Clinical Director to provide a process to promote and ensure employee orientation, competency and in-service education programs.
C. Maintains aggregate records of employee orientation and annual competency requirements as part of the PI plan.
D. Reports via email to the DON (*********************** )and administrator (************************) any Personal Care Aide whom you suspect his/her work attendance.
E. Monitor organization compliance with regulatory and accreditation standards.
a. Maintains current knowledge related to Medicare Conditions of Participation to ensure that the organization is compliant with state, federal and accreditation guidelines.
b. Responsible for ensuring policies and procedures reflect all related local, state, federal and makes recommendation for policy changes to the governing board.
c. Acts as a resource to the Clinical Manager regarding standards, policies, regulations and provides in-service training to leadership and personnel as needed in these areas.
2. Provides assistance to community education team in meeting community education needs.
A. Makes public speaking engagements as scheduled by the community education team.
B. Maintain positive relationships with referral sources.
C. Performs other duties as assigned by the Executive Director.
D. Maintain positive image in the community.
E. Acts as a representative of the organization at community meetings, civic and professional organizations as needed.
3. Promotes self-growth and collegial relationships with others in the home care industry.
A. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participates in professional organizations or societies.

Administer respiratory related medication to the tracheobronchial tree according to physician orders, B.R.C., AARC standards, and department policy and procedures. Accurately document in the medication administration records (MARS) the administration of medication according to physician's orders.
Consistently and accurately document in the patient medical records patient assessment with clinical parameters such as, breath sounds, tracheostomy tube data, sputum production, ventilator settings based on physician orders, weaning and tolerance, in compliance with department daily documentation records.
Provide the in-coming shift with room-to-room walking detailed, verbal report on the patient condition, any change in orders and tolerance to care provided. Consistently and accurately provide information regarding Spo2, Fio2, SVT, RR, sputum production, breath sounds, mode of therapy, medication administration and tolerance, among others. For patients on ventilatory support, latest ventilator settings, tolerance to weaning and Fio2 titration.
Perform diagnostic procedures in accordance with physician orders such as, sputum collection and arterial blood gases following B.R.C. safety standards and department policy and procedure.
Perform physician rounds when they are in the facility and update orders accordingly in the respiratory care daily assignment record.
Always maintain equipment and supply room clean and organized. When bulk supplies are in the equipment and supply room, remove supplies from packaging and properly place them on the shelves for easy access. No beverages and/or food consumption is allowed in the equipment and supply room.
When patients are transferred or discharged from the unit, immediately remove all equipment from the patient room and proceed to clean, disinfect and placed in the appropriate clean area for easy access. When patients are transferred or discharged from the unit or you are changing supplies according to the department change-out policy and procedure, discard all soiled disposable supplies in a RED biohazard bag. Only soiled, disposable supplies are to be disposed of in the red biohazard bags. Biohazard bags are ONLY to be disposed of in the dirty soiled utility room. All other trash is to be placed in a clear, bag-lined trash receptacle.
At all times, follow universal precautions according to CDC guidelines and the facility infection control policy and procedures to comply with regulatory and safety standards for the protection of self and other employees and patients.

Responsibilities:- Provide comprehensive primary care services in an outpatient setting.
- Conduct routine office visits and address acute medical concerns.
- Perform annual wellness visits and health risk assessments.
- Educate patients on their conditions, treatment plans, and preventive care strategies.
- Coordinate care with specialists, diagnostic facilities, and other healthcare providers.
- Utilize telemedicine as appropriate for patient follow-up and consultation.
- Participate in care team discussions, ensuring best practices in patient management.
- Maintain accurate, thorough documentation of patient encounters.
- Engage in population health initiatives, including routine screenings and preventive care measures.
- Other duties as assigned.
Qualifications:- Certified Nurse Practitioner (NP) or Physician Assistant (PA) with at least 1 year of experience in primary care (internal medicine, family medicine, or geriatrics preferred).
- Experience with Medicare patients and senior-focused care.
- Strong understanding of medical documentation, coding (ICD-10, HCC), and risk adjustment.
- Active, unrestricted state license and DEA registration.
- Medicare enrollment in good standing or eligibility to enroll.
- Passion for primary care and patient-centered medicine.
- Tech savvy and a quick learner!
- Experience with Athena preferred.
- Strong teamwork and collaboration skills.
