Part-time Description
The Respiratory Care Practitioner (aka Respiratory Therapist or RT) with Montefino does more than provide essential direct medical care to their patients. Our RTs have smaller patient caseloads, in some cases working with a single patient at the assigned facility on a given shift. This allows RTs to apply their skills in more direct and intentional ways to contribute toward positive patient outcomes. RTs build meaningful relationships with the patient's loved ones, their care team, and most importantly, the patient to foster positive impacts on their daily lives.
Montefino RTs follow established standards and procedures, provide supervision, care, support, and training to individuals with Developmental Disabilities. The position provides critical assistance to individuals with ADL's (activities of daily living). Effective RTs are organized, analyze situations accurately, take effective action, diligently follow directions, and communicate clearly and effectively. They possess knowledge of the industry/field, applicable state/federal laws (e.g. Title 17 & 22), and associated organizations and agencies (e.g. Regional Centers, Dept. of Social Services, Dept. of Development Services, State Council on Developmental Services, etc.). Successful candidates are enthusiastic in their efforts to expand upon their technical knowledge (e.g. dual Diagnosis, Individual Program Plans, emergency protocols, etc.), advocate for their patients, and are consummate professionals.
RTs maintain a safe, clean, and therapeutic environment that ensures respect, dignity, protects privacy, rights, and physical/emotional well-being to all residents. The RT possesses skills in organization, time management and analytical problem solving. Clear and effective written and verbal communication, interpersonal skills, multi-tasking, critical thinking and active listening skills and essential to success.
Essential Duties
Include but are not limited to:
Set up and operate devices such as mechanical ventilators, therapeutic gas administration apparatus, environmental control systems, and aerosol generators, following specified parameters of treatment.
Provide emergency care, including artificial respiration, external cardiac massage and assistance with cardiopulmonary resuscitation.
Determine requirements for treatment, such as type, method and duration of therapy, precautions to be taken, and medication and dosages, compatible with physicians' orders.
Monitor patient's physiological responses to therapy, such as vital signs, arterial blood gases, and blood chemistry changes, and consult with physician if adverse reactions occur.
Read prescription, measure arterial blood gases, and review patient information to assess patient condition.
Work as part of a team of physicians, nurses, and other health care professionals to manage patient care.
Enforce safety rules and ensure careful adherence to physicians' orders.
Maintain charts that contain patients' pertinent identification and therapy information.
Inspect, clean, test and maintain respiratory therapy equipment to ensure equipment is functioning safely and efficiently, ordering repairs when necessary.
Educate residents and their families about their conditions and teach appropriate disease management techniques, such as breathing exercises and the use of medications and respiratory equipment.
Explain treatment procedures to patients to gain cooperation and allay fears.
Perform bronchopulmonary drainage and assist or instruct patients in performance of breathing exercises.
Demonstrate respiratory care procedures to trainees and other health care personnel.
Make emergency visits to resolve equipment problems.
Provide supervision and guidance for residents during skills building activities.
Develop and implement individual treatment plans for residents.
Assist with daily living activities, including preparing meals and assisting with personal hygiene.
Develop a rapport with residents and cultivate a safe and supportive relationship.
Update resident case notes on a daily basis.
Ensure an appropriate staff-to-client ratio at all times.
Additional duties and projects as assigned.
Requirements
High School Diploma, GED, or equivalent.
Possess (and maintain) a current Respiratory License/Credentials from an accredited body. Preferred types include Registered Respiratory Therapist (RRT); Certified Respiratory Therapist (CRT); CA Respiratory Care Practitioner License (RCP)
1+ years of experience as a Respiratory Therapist or similar role is desired, but not required.
Possess a current driver license with a clean driving record and the ability to operate a motor vehicle safely.
Possess a service-oriented mindset with genuine passion to provide care for others.
Possess knowledge of the field and best practices. This includes, but is not limited to, concepts such as: Methods of oxygen delivery and possible side effects; Laws regulating the field/patient populations and regulatory entities; Proper sanitation and other care procedures, etc.

Key Responsibilities:
Collaborate with specialists in Pulmonary, Emergency Room (ER), and Intensive Care Units (ICU) in various aspects of clinical care of patients.
Specialize in airway management and mechanical ventilation.
Initiate and manage life support for people in ICUs and emergency departments.
Stabilize and monitor high risk patients being moved from hospital to hospital by ambulance.
Administer inhaled drugs and medical gases such as mucolytics, bronchodilators, oxygen, and nitric oxide.
Shall be competent in the following tasks:
Therapeutic and diagnostic procedures with the use of medical gases and administration devices including oxygen and nitric oxide.

Incentive Spirometry Limited bedside pulmonary function testing to include Vital

4. The RTP shall perform secretion clearance using various high frequency
oscillation and cough assist devices.
5. The RTP shall perform safe endotracheal, nasotracheal, and orotracheal
suctioning using both open and closed systems.
6. The RTP shall perform chest Physiotherapy and postural drainage by both
manual and mechanical means.
7. The RTP shall set up and monitor non-invasive ventilation devices using modes
such as Continuous Positive Airway Pressure (CPAP), Automatic Positive Airway
Pressure (APAP), and Bilevel Positive Airway Pressure (BIPAP). Intubation and
extubation assisting in accordance with approved policies and procedures, found in
Section D, attachments 03-07.
8. The RTP shall set-up, use, monitor, and troubleshoot various acute care and
transport ventilators; Implement, monitor, and troubleshoot various ventilation
modes including volume, pressure and spontaneous.
9. The RTP shall respond to Code Blues and perform cardiopulmonary
resuscitation as indicated; Assist with intubation, extubation and securement of
artificial airways; Respond to Rapid Response Calls, perform patient assessments,
and make recommendations based on assessment.
10. The RTP shall complete Bronchoscopy set-up, assistance, and pre-cleaning.
11. The RTP shall obtain sputum specimens for lab analysis.
12. The RTP shall perform Arterial puncture and blood gas analysis.

Home Oxygen Clinic as requested.

Schedule
Sleep Medicine Respiratory Therapists (RTS) will be expected to be available 24/7, including holidays. The normal tour of duty is a 10-hour day and 40 hours per week.
Responsibilities
The RTS shall independently evaluate patients with sleep related breathing disorders and their treatment modalities in clinic setting. This includes the evaluation of treatment efficacy through patient interview, equipment data analysis, and review of previous notes and study data.
The RTS shall perform airway humidification using various devices to include heated humidifiers, cool pass over humidity and Heat and Moisture Exchanger (HME).
The contract RTS shall evaluate PAP equipment and treatment based on doctor's orders verifying the type of CPAP equipment, pressure settings, comfort levels, mask size and type (nasal, full face, nasal pillows, etc.).
The RTS shall assess patient understanding and ability to manage their PAP therapy at home.
The RTS shall establish and maintain understanding of various PAP equipment and functional modes, as PAP equipment is constantly changing and evolving. This includes set up, adjustment and theory of operation.
The RTS shall communicate with patients via telephone and assist in troubleshooting CPAP equipment problems over the phone. Discuss, understand, and review wireless remote data with the patient.
The contract RTS shall order various sleep accessories utilizing the Computerized Patient Record System (CPRS) Consult system to Prosthetics, coordinate with that department when necessary.
The contract RTS shall establish and maintain competency in set up, evaluation, and adjusted of various PAP equipment currently in use for treatment of sleep disordered breathing.

DUTIES AND RESPONSIBILITIES Respiratory Therapists (RTP or Contract RTP)
assigned to Pulmonary.
A Pulmonary Respiratory Therapist is a specialized
healthcare practitioner who has graduated from a college or a university and passed a
national board certifying examination. Respiratory therapists educate and treat people
who are suffering from heart and lung problems. They often collaborate with specialists
in Pulmonary, Emergency Department (ED), and Intensive Care Unit (ICU) in various
aspects of clinical care of patients. Respiratory Therapists are specialists in airway
management and mechanical ventilation. They may be responsible for initiating and
managing life support devices for people in ICU and ED, as well as administering and
monitoring patient response to inhaled drugs and medical gases.
The contract RTP shall be proficient in the following tasks:
1. The RTP shall administer and monitor therapeutic medical gases to include
oxygen, nitric oxide, and Heliox using appropriate devices.
2. The RTP shall perform airway humidification using various devices to include
aerosols and high flow systems. Inhaled administration of aerosolized medications
using appropriate devices, to include bronchodilators, mucolytics and antibiotics.
3. The RTP shall perform lung inflation strategies to include the MetaNeb, and
Incentive Spirometry Limited bedside pulmonary function testing to include Vital
Capacity (VC), Negative Inspiratory Force (NIF), and Minute Ventilation (Ve)
measurements.
4. The RTP shall perform secretion clearance using various high frequency
oscillation and cough assist devices.
5. The RTP shall perform safe endotracheal, nasotracheal, and orotracheal
suctioning using both open and closed systems.
6. The RTP shall perform chest Physiotherapy and postural drainage by both
manual and mechanical means.
7. The RTP shall set up and monitor non-invasive ventilation devices using modes
such as Continuous Positive Airway Pressure (CPAP), Automatic Positive Airway
Pressure (APAP), and Bilevel Positive Airway Pressure (BIPAP). Intubation and
extubation assisting in accordance with approved policies and procedures, found in
Section D, attachments 03-07.
8. The RTP shall set-up, use, monitor, and troubleshoot various acute care and
transport ventilators; Implement, monitor, and troubleshoot various ventilation
modes including volume, pressure and spontaneous.
9. The RTP shall respond to Code Blues and perform cardiopulmonary
resuscitation as indicated; Assist with intubation, extubation and securement of
artificial airways; Respond to Rapid Response Calls, perform patient assessments,
and make recommendations based on assessment.
10. The RTP shall complete Bronchoscopy set-up, assistance, and pre-cleaning.
11. The RTP shall obtain sputum specimens for lab analysis.
12. The RTP shall perform Arterial puncture and blood gas analysis.
13. The RTP shall complete Home Oxygen patient assessments and assist the
Home Oxygen Clinic as requested.
14. The RTP shall provide patient and/or family education as indicated.
15. The RTP shall recognition of the need for medical intervention if patient safety is
at risk
