Thorough knowledge of the procedures and patient care techniques involved in the delivery of respiratory care

Job Summary
Staff Respiratory Therapist, 36hr Day shift
Respiratory therapists treat, evaluate, monitor, and manage patients with breathing disorders or cardiovascular problems.
Care provided by respiratory therapists may include administration of oxygen, cardiopulmonary resuscitation, chest physical therapy, management of mechanical ventilators, administering drugs to the lungs, monitoring cardiopulmonary systems, and measuring lung function.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
1. Determines respiratory treatment goals in consultation with physicians, nurses, and by prescription
2. Delivers and assesses responses to ordered therapy to be sure it is restoring the patient's pulmonary function.
3. Administers prescribed doses of aerosol drugs
4. Utilizes various types of oxygen and other therapeutic gas equipment such as masks, BIPAP/CPAP, ventilators per hospital policy
5. Administers Respiratory Therapy by performing various techniques of bronchopulmonary drainage, such as percussion/vibration, Acapella, incentive spirometry
6. Monitors physiological responses to therapy, such as vital signs, arterial blood gases, and blood chemistry changes
7. Educate patients and family members on a variety of topics such as correct MDI or aerosol use, smoking cessation, and other respiratory equipment
8. Assists in educating students
9. Ability to perform arterial blood gas puncture
10. Respond to Code Blue, stat calls, and be a member of RRT (Rapid Response Team)
11. Assist with endotracheal intubation
12. Extubate patients per order
13. Maintain the integrity of the patient-ventilator system by proper airway management, measure ventilator volumes and pressures, and obtain ventilator mechanics (RSBI, Raw, spontaneous RR and minute volume, NIF)
14. Be an active critical care team member in meeting patients' goals by attending rounds, giving pertinent patient information, and offering suggestions.
15. Be a team player both on the floor and in the ICU to meet the best patient goals and needs
16. Assist and direct treatments given by CRTs and those with limited licenses.
17. Assist with the orientation of new employees
18. Monitors, records, and communicates patient condition and general treatment acceptance.

Current BLS certification

WORKING CONDITIONS:
Respiratory therapists are on their feet walking and working the majority of the time. They push ventilators which are on wheels to different locations. They perform chest physical therapy on patients and assist during CPR, sometimes performing cardiac compressions. Therapists typically work three 12-hour shifts and take turns covering weekends and holidays.

Summary:

This position is responsible for respiratory care for patients of all age groups, who are diagnosed with cardio-pulmonary dysfunction. Services are provided throughout the Tufts Medical Center are complex. This includes the ICUs, in-patient rooms, the Emergency Room, and outpatient clinics. A request for service originates from the medical staff of Tufts Medical Center. It may be written or verbal and can be communicated to the therapist by direct telephone conversation and/or beeper page. The patient's nurse or unit secretary conveys the order.
All patients are assessed for their needs initially and on an on-going basis until no longer required. Documentation of services provided is recorded in the Respiratory Therapy Progress Notes section located on the department flow sheet. A copy of this document is filed in the patient Medical Record.
A list of the specific practices available by this department is noted below. A Respiratory Therapist is under the direction of the Director and Assistant Director of Respiratory Care. Patient services are available 24 hours/day, 365 days/year. Services are provided in accordance to policies and procedures agreed upon by the Medical Directors of the department. Clinical Practice Guidelines have and continue to evolve as the framework of clinical respiratory care practice. These practices are well referenced and sponsored by the American Association for Respiratory Care.
PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:
* Neonatal and pediatric training
* Advanced certifications: ACLS, NRP, PALS

* Participation in neonatal and pediatric transport program

* Provides resuscitation and support services at all high-risk deliveries.
* Monitors effectiveness of therapy using non-invasive techniques such as pulse oximetry, end tidal CO2 and transcutaneous PO2/PCO2, in addition to invasive techniques such as arterial blood gas sampling.
* Analyzes inspired oxygen concentration using various oxygen analyzers.
* Calibrates monitors and maintains Transcutaneous Oxygen Monitors, End Tidal CO2 and pulse Oximetry monitors.
* Assists in Cardio-Pulmonary Resuscitation as part of Code Team primarily providing airway management and manual resuscitation. Commonly administers drugs via the endotracheal tube during cardiopulmonary rescue e.g. Lidocaine, Atropine Sulfate and Epinephrine.
* Documents therapy, patient assessment, and interventions performed and test results in the progress notes of the medical record in a concise, accurate, and thorough manner.
* Transcribe a detailed shift note for all patient's cared for during the shift.
* Responsible for acknowledging the proper assembly of all respiratory therapy equipment prior to and during patient use.
* Performs daily rounds and fairly bills patients for services rendered.
* Attend rounds daily with the intensive care unit critical care team and is an active participant in the patient's care.
* Assists with development of in-service programs for departmental personnel.
* Assists the Associate Director with the orientation of new employees; acts as a preceptor for new employees and is utilized as a clinical instructor for students enrolled in local universities.
* Administers therapeutic agents such as Heliox, Surfactant, Tobramycin, Ribavirin and Pentamidine in accordance with established Departmental procedure.
* Provides services for new clinical programs as research protocols are developed.

Responsibilities as the Registered Respiratory Therapist:
Patient Assessment: Evaluate cardiopulmonary status, perform diagnostic tests, and monitor vital signs to guide respiratory care.
Ventilator & Airway Management: Set up, manage, and adjust mechanical ventilation and artificial airways per clinical protocols.
Therapeutic Procedures: Administer oxygen therapy, bronchodilators, aerosol treatments, chest physiotherapy, and suctioning.
Emergency Care: Respond to code blue and rapid response events, providing airway support and life-saving interventions.
Clinical Documentation: Record treatments and patient responses in EMRs; collaborate with medical staff for coordinated care.
Protocol & Equipment Oversight: Adhere to clinical standards and maintain, calibrate, and troubleshoot respiratory equipment.
Qualifications for the Registered Respiratory Therapist:
Education: High school diploma or GED required.
Licensure: Registered Respiratory Therapist (RRT) credential required (must hold a current Massachusetts license).
Certification: CPR (AHA) and Neonatal Resuscitation Program (NRP) certification required (ACLS and PALS preferred).
Experience: 2+ years of experience in an acute care setting, with demonstrated ability to manage ventilators, perform airway interventions, and respond to emergency situations.
Technical Skills: Proficient with mechanical ventilation systems, oxygen delivery devices, aerosol therapy, and electronic charting systems (e.g., Epic).
