Perform specific tasks according to daily work assignments. Perform assigned tasks according to established laundry procedures. Collect, sort, count and/or weigh soiled laundry, linen, garments, etc. and place in appropriate containers or assigned areas. Separate items that require special stain removal/treatment. Press/iron garments as instructed. Fold, count, stack, hang, label and distribute clean laundry to residents daily or as instructed. During emergency conditions, assure that clean laundry, linen, garments, etc., are distributed to designated areas as instructed. Clean floors, including sweeping, dusting, and mopping as requested. Keep laundry area clean and sanitary. Remove dust, dirt, grease, etc. from equipment and/or floor surfaces as necessary. Discard waste/trash into proper containers and reline receptacle with plastic liner. · Perform daily housekeeping tasks as assigned · Perform specific tasks according to daily work assignments · Empty and sanitize ash trays daily · Clean/polish furnishings, fixtures, ledges, room heating/cooling units, etc. in resident rooms, recreational areas, etc. daily · Clean, wash, sanitize, and/or polish bathroom fixtures. Ensure that water marks are removed from fixtures · Clean windows/mirrors in resident rooms, recreational areas, bathrooms and entrance/exit ways · Clean floors, to include sweeping, dusting, damp/wet mopping, stripping, waxing, buffing, disinfecting, etc. · Ensure that appropriate caution/safety signs are properly set up prior to performing duties · Clean carpets, to include vacuuming, shampooing, deodorizing, and disinfecting · Clean walls, ceilings by washing, wiping, dusting, spot cleaning, disinfecting, deodorizing · Remove dirt, dust, grease, film, etc. from general surfaces throughout the facility · Clean hallways, stairways, and elevators · Discard waste/trash into proper containers and reline trash receptacles with plastic liners · Clean vacant rooms as assigned · Ensure that work/assignments areas are clean and that equipment, tools, supplies, etc., are properly stored at all times, as well as before leaving such areas for breaks, meal times, and end of the workday · Perform isolation cleaning procedures in accordance with established infection control procedures Please visit our careers page to see more job opportunities.

    ←Back to all jobs at Rest Haven Health and Rehab Licensed Practical Nurse The primary purpose of the Licensed Practical Nurse position is to provide direct nursing care to residents as a member of an interdisciplinary team of care providers and to plan, supervise and evaluate the day-to-day nursing activities performed by nursing assistants. · Plan, assign, supervise and evaluate all responsibilities to be performed by members of the nursing staff under your supervision, so that care and services are provided on the basis of staff qualifications, resident acuity, required duties and physical layout of the facility. · Provide leadership, mentoring and guidance to associates for whom you are responsible. · As requested by the Director of Nursing or designee, assist in reviewing department policies, procedures and job descriptions and making recommendations for revisions. · Implement and monitor compliance with all established policies and procedures. · Meet regularly with your assigned nursing staff and support personnel to plan and communicate appropriate care-related services and activities for your residents, communicate residents' concerns and needs and discuss other vital information related to resident care and facility practices. Develop staff assignments as indicated. · Cooperate with other resident services to ensure that residents' individual care plans are being followed. · Participate in the facility's quality improvement to discuss resident, personnel and facility related business. · Meet with other nursing service and facility personnel on a regularly scheduled basis, to assist in identifying and correcting problem areas and/or improve services. · Review complaints made by your assigned personnel and communicate such information to the Director of Nursing or designee, as appropriate. · Participate in facility surveys made by authorized regulatory agencies. · Participate in the admission, transfer and discharge of residents as required. · Implement and maintain established facility objectives, consistent with federal and state licensure regulations and nursing standards of practice. · Follow the facility's policies and procedures governing the provision of direct care to residents, including the administration of medications, treatments and other care in accordance with physicians' orders and the residents' care plans. · Perform continual observation and evaluation of each resident's physical, mental, cognitive and emotional status and Director of Nursing or designee, to the attending physician and to the resident's responsible party, according to facility policy and protocol. · Communicate timely with physicians and/or nurse practitioners related to resident care needs and concerns. Be responsible to properly implement and communicate to other caregivers all physician &/or nurse practitioner orders. · Respond immediately to resident events, accidents, incidents and/or allegations of resident abuse, neglect and/or misappropriation of property. Perform an assessment of the involved resident(s) and provide emergency intervention as required. Notify the Director of Nursing or designee, the Administrator (as appropriate), the resident's attending physician and the resident's responsible party when the resident is involved in an accident/incident. Initiate an immediate investigation of the accident/incident/unusual circumstance, when indicated, and in accordance with facility policy & procedures. · Attend in-service education programs in order to stay current in your profession and practice and meet facility educational requirements. · Conduct weekly skin checks and document/report findings to unit manager. Identify residents before administering treatments, serving meals, etc. as necessary. Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner. Use only authorized abbreviations established by this facility when recording information. Report all changes in the resident's condition to the RN Team Leader as soon as possible. Report and provide statement for all events, accidents and incidents you observe on the shift as they occur. Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisor. Follow work assignments, and/or work schedules in completing and performing your assigned tasks. Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and the shift Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services and participate in Team Conference. Participate in and receive the nursing report upon reporting for duty. Follow established policies concerning exposure to blood/body fluids. Assist residents with hygiene needs to include: · Daily dental and mouth care (i.e. brushing teeth/dentures, oral hygiene, special mouth care, etc.) · Bathing (i.e. bed bath, tub or shower bath, etc.) as directed · Dressing/undressing as necessary · Hair care needs (i.e. combing, brushing, shampooing, etc.) · Nail care (i.e. clipping, trimming and cleaning the finger/toe nails) · Shaving male residents · Controlling hair growth on female residents (i.e. facial hair, under arms, on legs, etc.) as instructed · Keeping residents dry (i.e. change gown, clothing, linen, etc. when they become wet or soiled) Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident. Make beds (occupied and unoccupied.) Put extra linens on bed as requested. Position bedfast residents in correct and comfortable position, utilizing positioning devices as indicated. Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.) Maintain intake and output records as instructed. Keep incontinent residents clean and dry. Check and report bowel movements and character of stools as instructed. Assist residents in preparing for medical tests and appointments (i.e., lab work, x-ray, therapy, dental, physician dialysis, etc.) Assist residents in preparing for activity and social programs (i.e. church, parties, visitors, physician, dialysis). Assist in transporting residents to/from appointments, activity and social programs, etc. as necessary. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Assist residents to walk with or without self-help devices as instructed. Provide eye and ear care (i.e., warm/cold compresses, removing wax and hair, cleaning eyeglasses/hearing aids, dentures, etc.) as instructed. Assist in preparing the resident for a physical examination. Measure and record temperatures, pulse, and respirations, as instructed. Answer resident calls promptly. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. Assist with the care of the dying resident as directed. Administrative Functions · Organize and assign all jobs to be done on tour of duty so that workload is evenly distributed among nursing personnel on the basis of staff qualifications, size and physical layout of the facility. · Periodically review department policies, procedures and job descriptions and make recommendations for revisions. · Implement established policies and procedures. · Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility. · Meet with your assigned nursing staff and support personnel to plan appropriate services, programs and activities for your shift. · Cooperate with other resident services when coordinating nursing services to ensure that the resident's individual care plans are being followed. · Participate in the development, maintenance, and implementation of the facility's quality assurance program. · Request conferences with the Director of Nursing as needed to discuss resident, personnel and facility related business. · Participate in facility surveys made by authorized regulatory agencies as requested. · Periodically review the resident's written discharge plan and participate in updating that plan as required. · Admit, transfer and discharge residents as required. · Place and receive, personally, all call to and from physicians. Charting and Documentation · Assist in updating the resident care plans as instructed by the MDS/Care Plan Coordinator and/or Director of Nursing. · Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care. · Document status of each resident on the chart at least weekly, more often if significant change occurs or as directed. Follow up with monthly and quarterly summaries as instructed. · Make rounds with physician, as directed, and record visits in resident's chart. · Transcribe, accurately and promptly, all physician's orders. · Direct documentation and make weekly detailed evaluation of all resident documentation so that chart reflects progress and condition of resident at all times. · Approve and sign charts when observations are made. · Ensure that all medications and treatments are charted and signed following administration of the medication/treatment, by the administering person. · Report all discrepancies noted concerning physician's orders, diet change, charting error, etc. to the ADON or DON.
