Position Summary: The Bus Driver will be responsible for safely transporting passengers to their destinations on assigned bus routes, ensuring a comfortable and timely journey.
Essential Responsibilities:
* Operate a bus and follow designated routes and schedules.
* Ensure the safety of passengers during transit, including adherence to traffic rules and regulations
* Assist passengers in boarding and disembarking the bus, ensuring a courteous and helpful attitude.
* Communicate effectively with passengers, providing information about routes, schedules, and fare payment options.
* Maintain a clean and well-maintained bus interior, including regular cleaning and reporting any maintenance or repair needs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations.

Position Summary: Provide and supervise the administration of high-quality, person-centered healthcare services to residents in accordance with the Michigan Nurse Practice Act. Ensure the promotion of resident rights, dignity, freedom of choice, and individualized care. Collaborate with interdisciplinary teams to maintain a safe, compliant, and compassionate environment.
Essential Responsibilities:
* Deliver and supervise direct nursing care per physician orders and nursing standards.
* Develop, implement, and update individualized care plans, particularly for transitional care residents.
* Administer medications and treatments accurately and timely; educate residents as appropriate.
* Monitor and document resident status, including physical, emotional, and behavioral changes.
* Maintain compliance with infection control protocols and safety standards.
* Communicate effectively with physicians, families, home health agencies, and team members.
* Promote and protect residents' rights, dignity, and choice in a person-centered care environment.
* Conduct routine rounds on all units to ensure quality of care and staff support.
* Assist with resident admissions and ensure completion of all necessary documentation (UDAs, order checks, care plans).
* Perform narcotic/pharmacy audits and crash cart checks; ensure appropriate witnessing and documentation.
* Waste and return narcotics according to policy and pharmacy guidelines.
* Review recent falls to confirm incident reports are complete and interventions have been implemented.
* Ensure resident care plans are updated in response to condition changes or communication updates.
* Monitor and respond to secure messages, pending orders, and other care-related tasks in a timely manner.
* Manage daily staffing, respond to call-ins, and adjust schedules to maintain adequate coverage.
* Respond to call lights and assist with direct resident care as needed.
* Lock facility doors after 8:00 PM in accordance with safety procedures.
* Check fax communications regularly and ensure proper routing to the correct unit or nurse.
* Check medication room stock and restock supplies as needed.
* Support a professional team environment by fostering open communication, resolving issues, and encouraging professional growth.
* Remain calm and resourceful in emergencies, ensuring appropriate response and follow-up.
* Maintain confidentiality and adhere to all Resthaven policies and procedures.
* Perform other job-related duties as assigned.
Education, Training, & Experience:
* Graduate of an accredited RN nursing program.
* Minimum of 3 years of nursing experience, preferably in long-term care or geriatrics.
* At least 1 year of supervisory or charge nurse experience.
* Ongoing participation in continuing education to maintain licensure and enhance clinical skills.
Licenses & Certifications:
* Current LPN or RN license issued by the Michigan Board of Nursing.
* Current CPR certification (BLS preferred).
Knowledge, Skills & Abilities:
* Strong clinical judgment and knowledge of nursing principles and geriatric care.
* Proficient in electronic medical records and basic computer applications (Word, Excel, Email).
* Effective verbal and written communication skills.
* Ability to manage time, prioritize duties, and handle multiple responsibilities.
* Demonstrates compassion, professionalism, and respect for residents and colleagues.
* Ability to use appropriate interventions for dementia and other cognitive impairments.
* Familiarity with standard office and medical equipment (fax, copier, phone, etc.).
* Strong organizational skills and attention to detail.
Physical Demands:
* Requires standing, walking, sitting, bending, kneeling, lifting (up to 50 lbs), and other physical activities typical of nursing care.
* Must be able to see, hear, and communicate effectively.
* Manual dexterity for operating equipment and performing clinical procedures.

Position Summary: Lead quality programming for residents in an environment that promotes their rights, dignity, freedom of choice, and their individuality.
Essential Responsibilities:
* Facilitate a variety of group programming to engage with residents and meet the different domains of health such as physical, cognitive, emotional, social and spiritual. Programs may include: cardio drumming, flower arranging, seated yoga, painting and more!
* Chart daily activity participation and attendance for all residents.
* Initiate one-on-one activities for individual residents based upon their functional level, including but not limited to, pampering, playing music, reading and conversation.
* Clean and maintain the activity equipment and rooms/areas.
* Assist and direct volunteer support.
* Complete project assignments on schedule.
* Participate in resident outings and bus trips.
* Develop and maintain a good rapport with all units involved with the care plan to ensure that a team effort is being achieved in developing a comprehensive plan of care specific to the individual's personality and daily living.
Knowledge, Skills & Abilities:
* In progress or completion of HS Diploma or GED.
* Ability to speak clearly and loudly when leading activities and announcements.

Education, Training & Experience:
* College course work in social work, gerontology, or human services and/or previous experience working with the elderly preferred.
