Using evidence based practice, the incumbent of this position will focus on the improvement of pharmaceutical processes, treatments, plans of care and other related interventions and makes recommendations to improve the delivery of high quality pharmaceutical care to the patient. Essential Functions & Responsibilities: Provides clinical pharmacy services to patients in clinical areas as assigned. Participates in pharmacy related committees such as P&T Committee. Evaluates current drug usage and makes recommendations utilizing current research, monographs, and medication audits. Participates in Joint Commission mandates and National Patient Safety Goals initiatives. Participates in continuing education as required by the Department. Participates in medication safety related activities and initiatives as required. Collaborates with nursing in medication and patient related initiatives. Precepts pharmacy students and residents as required. Develops and implements annual cost-containment measures in their respective areas of specialization. Performs health-economic studies annually to assess the impact and value of a pharmaceutical care service line in their area of specialization. Develops, implements, and coordinates CQI measures annually in their area of specialization. Integrates, on a continual basis, the clinical specialist functions with the medication distribution functions of the Department. Performs all aspect of pharmaceutical care, including drug distribution, parenterals admixtures, compounding and basic clinical pharmacokinetics functions when called upon. Collaborates with peers, nurses, and physicians to analyze and evaluate current systems of pharmaceutical care delivery, identifies and supports implementation of new practice patterns as appropriate: serves as a clinical resource. Applies principles of communication and change theory in providing health care consultation. Works independently, with high regard to coordination with other specialty areas. Provides and assists in program development. Actively participation in a professional organization. Ability to utilize critical thinking and skills necessary to intervene in critical situations. Performs other duties as assigned. Qualifications: Doctor of Pharmacy degree and post-graduate residency or fellowship required. Licensed, or eligible for licensure, as a pharmacist in the Commonwealth of Virginia required. CPR certification required within first 90 days of employment. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) required within first 90 days of employment or placement. Three years hospital experience with enhanced clinical responsibilities or one year of same and completion of an ASHP-accredited pharmacy residency or research fellowship required. Ability to work independently required. Ability to work with computers required. Experience providing drug information and performing pharmacokinetics required. Collaborates with ancillary services (e.g., PT, OT, Social Work) as needed to ensure a well-rounded and effective plan. Leads the clinical care team, including nurses, physicians, social workers and ancillary staff in daily SNAP huddles to align discharge goals. Identifies and addresses any barriers to patient progression and discharge, ensuring timely and coordinated care. Manages documentation of designated patient progression elements in the care coordination software to include the classification, documentation, and resolution of progression and discharge barriers; escalates unresolved barriers to senior leadership via escalation huddle. Ensures timely and ongoing communication of unit capacity status with the Hub to support efficient system throughput and optimal patient flow. Partners with virtual Utilization Review (UR) Nurse to align payor and status requirements with patient's clinical progression; verifies appropriate regulatory letters are delivered to patient and family when indicated. Communicates progression and discharge plan to the patient and/or their designated proxy in coordination with the patient's primary care team (RN, physician). This position serves as a spoke of the Hub providing ED progression leadership and serving as the primary liaison between the Emergency Department and the Hub, Nursing Units (C3s) and support departments, and in partnership with physicians, to ensure patient care needs are met and treatment plans are executed timely. The ED C3 leads and collaborates work with members of the healthcare team to improve patient throughput and LOS performance of the organization Essential Functions and Responsibilities: Assesses patients' clinical presentation against Milliman Care Guidelines (MCG) to identify admission status, working DRG and target LOS; communicates recommended status to physician; validates admission order matches determined status and level of care. Collaborates with ED and Hospitalist providers/clinicians to proactively evaluate appropriateness of potential admissions (i.e., status, level of care); evaluates alternatives for patients not meeting admission criteria. Assesses patient discharge needs (for patients not meeting admission criteria); collaborates with physicians and ancillary services (i.e., PT, OT, Social Work) when indicated to ensure a comprehensive discharge plan that addresses patient needs, barriers, and readmission risk. Communicates plan of care and discharge plan to patient and/or their family in coordination with the patient's primary care team (RN, physician). Evaluates and identifies cases where status discrepancy requires physician level review; escalates case through appropriate venue for review and status determination. Consults and educates medical and nursing staff regarding status, placement, payor requirements, patient progression and care coordination. Communicates and coordinates patient admission activities with the Hub and Unit Clinical Care Coordinators (C3s) to facilitate system-wide planning; ensures appropriate patient placement, level of care, patient progression, and patient flow. Leads clinical care team (RN, physician, ancillary staff, and social worker) in SNAP huddles for ED Holds to ensure care team alignment on progression of care; identifies and escalates barriers to senior leadership as appropriate. Manages all aspects of admission/patient progression in Care Advance software (i.e., bed requests, status, patient attributes/alerts, DRG/TLOS, etc.); communicates timely, updates of patient status and level of care with the Hub to facilitate unit and bed assignment for admitted patients. Oversees and ensures timely compliance with patient's preprocedural requirements for service department diagnostics and procedures; ensures communication of service event issues with care team. Proactively identifies and removes barriers that may impede department or system-wide patient flow. Serves as a resource on patient status, progression, and care coordination for the Emergency Department Associates. Performs other duties as assigned. Qualifications: Required: Minimum of three (3) years recent acute care clinical experience Bachelor of Science in Nursing is preferred (applicable to all external and internal placements.)
