The Qualified Professional (QP) serves as a key member of the interdisciplinary team (IDT) within the Intermediate Care Facility (ICF), responsible for coordinating, monitoring, and supporting the delivery of person-centered services to individuals with intellectual and developmental disabilities (IDD). This role ensures the development and implementation of individualized Person-Centered Plans (PCPs), supports successful transitions and admissions into the facility, and plays a lead role in maintaining full bed occupancy by actively managing the referral and intake process.
As the primary coordinator of care, the QP ensures that services align with each individual's goals, preferences, and support needs-emphasizing independence, well-being, and inclusion. The QP fosters collaboration among team members, including nursing, direct support staff, clinical specialists, administrators, and guardians to drive positive outcomes and regulatory compliance.
DUTIES AND RESPONSIBILITIES:
Admissions & Census Management:
Lead the intake and admission process, including pre-admission screenings, conducting meet-and-greet visits with prospective individuals and their support teams, and determining program compatibility.
Coordinate all aspects of the admission process, ensuring timely collection of required documentation and smooth transitions into the facility.
Develop and implement the initial Person-Centered Plan (PCP) upon admission, in collaboration with the IDT.
Maintain awareness of vacant beds and actively support referral development and follow-up efforts to ensure timely occupancy.
Service Coordination & Planning:
Serve as the primary coordinator of the IDT, ensuring that interdisciplinary team meetings are scheduled, facilitated, and documented according to regulatory and organizational timelines.
Interview individuals served to understand their needs, goals, and preferences; translate this information into actionable, person-centered goals.
Supervise the development and implementation of PCPs that are based on comprehensive assessments, observations, and input from individuals and their support networks.
Monitor and review the delivery of services to ensure fidelity to the PCP and assess progress toward identified outcomes (personal, clinical, and functional).
Monitoring & Oversight:
Conduct regular site visits to homes, day programs, and other service locations to evaluate the quality and consistency of PCP implementation.
Monitor home environments for cleanliness, appearance, safety, engagement opportunities, and community integration.
Ensure continuous quality improvement by reviewing internal audits, state surveys, and quality assurance data; participate in the development of corrective action plans as needed.
Advocacy & Communication:
Maintain strong communication with families, legal guardians, and external stakeholders to keep them informed and engaged in service planning and delivery.
Act as an advocate for the individuals served, ensuring their rights are upheld and their voices are central to all decisions affecting their care.
Other Duties:
Ensure timely and accurate documentation in accordance with Medicaid, ICF, and company requirements.
Participate in ongoing training, compliance reviews, and team meetings.
Perform other duties as assigned to support the health, safety, and well-being of the individuals served.
SUPERVISORY RESPONSIBILITIES:
This position may have supervisory responsibilities, which may vary based on the size and scope of the program.
MINIMUM QUALIFICATIONS:
Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master's degree preferred).
At least 2 years of experience (in North Carolina), 1 year of experience (in Tennessee) working with individuals with intellectual and developmental disabilities; 1 year required if Master's degree is held.
Demonstrated experience in developing, writing, and implementing Person-Centered Plans.
Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules.
Excellent communication, organizational, and documentation skills.
Valid driver's license, auto insurance, and reliable transportation.
Must be at least 18 years of age.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.
Must be able to use hands and fingers to handle or operate objects, tools, or controls.
Required to stand and walk for extended periods.
Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.
Vision requirements include close, distance, and peripheral vision.
Must be able to talk and hear.

The Painter is an in-house position within the Facilities & Fleet (F&F) Department responsible for addressing painting and surface-finishing needs across RHA Health Services facilities. This role supports the upkeep, safety, and appearance of residential homes, facilities, and administrative spaces by performing high-quality interior and exterior painting, surface preparation, and minor related repairs.
The Painter works collaboratively with Facilities leadership to assess project needs, recommend materials, and execute work efficiently while minimizing disruption to operations and individuals served. This position requires travels to RHA locations.
KEY RESPONSIBILITIES:
Perform interior and exterior painting projects across RHA residential homes, facilities, and administrative buildings.
Prepare surfaces for painting, including cleaning, sanding, scraping, patching, and priming walls, ceilings, doors, trim, and other surfaces.
Conduct minor drywall repairs, including patching holes, cracks, and surface imperfections prior to painting.
Perform light carpentry or minor maintenance tasks directly related to painting projects, as appropriate.
Assess job sites and develop material estimates and scope of work, including recommended paint types, quantities, supplies, and preparation needs for each project.
Assist with assembling supply lists and coordinating paint and material purchases in alignment with departmental processes.
Safely operate and maintain painting tools and equipment, including ladders, sprayers, rollers, and brushes.
Ensure all work meets quality standards, safety requirements, and infection control expectations appropriate for healthcare and residential environments.
Maintain clean and organized work areas during and after project completion.
Project scope may vary from full facility paint refreshes to targeted patchwork and touch-up projects.
Coordinate work schedules with Facilities leadership to minimize disruption to operations and individuals supported.
Identify and report facility conditions or maintenance concerns observed during painting projects.
Comply with all company policies, safety guidelines, and regulatory requirements.

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.

Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.
Maintain medical records for each beneficiary, ensuring documentation of current status, service changes, and referrals.
Ensure compliance with 10A NCAC 27G.0202 and other regulatory guidelines.
Complete all required records per agency policy and the State CAP manual, including discharge summaries when CAP services are completed.
Review and ensure proper billing codes and compliance for case management, in-home aide documentation, paid live-in caregiver, re-certifications, and supply billing.
Participate in NC Medicaid-certified training programs and ensure program compliance within 90 days of employment.
Collaboration and Advocacy:
Work closely with RNs and interdisciplinary team members to ensure a comprehensive approach to beneficiary care.
Participate in case discussions and provide input to ensure quality care and service delivery.
Advocate for program participants and their families to secure necessary resources and services.
Serve as a liaison between beneficiaries, families, and external providers to address care needs effectively.
Provide training and support to families to empower them in managing their child's care.
Continuing Education and Professional Development:
Complete all state-mandated training and agency-required continuing education annually.
Stay current on CAP guidelines, best practices, and new developments to enhance service delivery.
Ensure timely updates to the CAP Business system and other documentation systems as required.

MINIMUM QUALIFICATIONS:

As an AFL caregiver you will be asked to provide support for individuals with disabilities in a home setting. You will be asked to provide the supports identified below based on the needs of the individual in your care:
Seek to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working. Supports are provided according to the person's abilities and interests while ensuring privacy and dignity.
Prepare, teach and assist the person in food preparation, and cleaning of the home.
Honor the choices, preferences and culture of the person you support.
Assist in providing opportunities for social, emotional and spiritual needs to be met.
Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
Ensure that the home environment is safe and in good condition.
Implement services and supports to address goals identified through the Person Centered Plan. This includes medical, psychiatric, recreational, spiritual, and emotional needs.
Assist the person supported by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
Participate in supervision as agreed upon in the RHA supervision contract.
RHA will assist you in the following capacities:
Provide support through a multidisciplinary team for crisis assistance and training as needed as you begin your journey as an AFL caregiver. Training includes RHA's proprietary ProAct training, which focuses on supporting resiliency, emotional stability and problem solving.
Assist in coordinating respite care to support your needs as a caregiver.
Supervision will be provided to assist you in adjusting to your new role. The frequency and amount of supervision will be determined in a collaborative relationship with your RHA supervisor.

Performs a variety of administrative, training and personnel functions. Supervises and coordinates activities of Direct Support employees and manages operations, staffing and budget of assigned facilities. Conducts unit investigations for compliance and allegations of abuse, neglect, and exploitation. Coordinates and monitors unit's quality assurance system. Assists Administrator with the development of services and oversight of the unit. Takes rotating on-call responsibility. Relies on experience, knowledge, and judgment with limited direction and supervision.
DUTIES AND RESPONSIBILITIES:
Develops and maintains a system for assuring integration of basic developmental care and programming services.
Supervises and coordinates activities of Direct Support employees and manages operations, staffing and budget of assigned facilities.
Interviews and hires Direct Support Professionals
Assists with creating employee schedules ensuring that there is always adequate staffing coverage.
Works closely with clinical and professional healthcare staff to maintain communication and provide feedback, standardizing procedures, and expediting the implementation of person-centered plans.
Provides shift coverage in times of acute shortages of direct support staff. Provides on call coverage for people supported they support and services as needed.
Provides after-hours and on-call coverage as part of the direct care rotation
Works closely with Qualified Professional and other clinical/professional staff to maintain communication and provide feedback, standardize procedures, expedite Person Centered Plan implementation and workflow and to improve efficiency of team
Partners with the Worker's Compensation team to ensure prompt reporting and investigation of employee injuries, ensures employee receive needed safety training and coaches employees not working safely and properly
Periodically monitors all shifts and basic developmental care to ensure that legislative standards are met. Constantly monitors activities to ensure that health and sanitary standards are met; corrects any deficiencies noted by internal/external authorities.
Assists in the Interdisciplinary Team Process, as needed. Participates in the Quality Assurance Assessment system which may include; Mealtime Assessment, Environmental Assessment; Interaction Assessment and Third Shift Assessment.
Interviews, hires, coaches and terminates employees as needed.
All other duties as assigned.
SUPERVISORY RESPONSIBILITIES:
This position supervises Group Home Supervisors and Direct Support Professionals
MINIMUM QUALIFICATIONS:

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.
The Case Manager works closely with families, RNs, and other interdisciplinary team members to assess needs, coordinate care, and provide ongoing support to help beneficiaries achieve the best possible quality of life.
DUTIES AND RESPONSIBILITIES:
Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.
Maintain medical records for each beneficiary, ensuring documentation of current status, service changes, and referrals.
Ensure compliance with 10A NCAC 27G.0202 and other regulatory guidelines.
Complete all required records per agency policy and the State CAP manual, including discharge summaries when CAP services are completed.
Review and ensure proper billing codes and compliance for case management, in-home aide documentation, paid live-in caregiver, re-certifications, and supply billing.
Participate in NC Medicaid-certified training programs and ensure program compliance within 90 days of employment.
Collaboration and Advocacy:
Work closely with RNs and interdisciplinary team members to ensure a comprehensive approach to beneficiary care.
Participate in case discussions and provide input to ensure quality care and service delivery.
Advocate for program participants and their families to secure necessary resources and services.
Serve as a liaison between beneficiaries, families, and external providers to address care needs effectively.
Provide training and support to families to empower them in managing their child's care.
Continuing Education and Professional Development:
Complete all state-mandated training and agency-required continuing education annually.
Stay current on CAP guidelines, best practices, and new developments to enhance service delivery.
Ensure timely updates to the CAP Business system and other documentation systems as required.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
MINIMUM QUALIFICATIONS:
Strong understanding of Medicaid programs and compliance requirements.
Excellent communication and interpersonal skills to work effectively with families and multidisciplinary teams.
Ability to manage multiple priorities and maintain detailed records.
Experience working with medically fragile children or in pediatric healthcare settings and physically disabled adults with complex care needs.
