Intensive In Home QP Needed to provide services to youth at risk of removal from home and their families. Services are provided in community based setting to include home, school and other community settings. Under direct supervision of team lead, the QP will meet regularly with families, implement interventions as directed in the person centered plan, interact with community partners and document interventions. These interventions are strength-based and focused on promoting recovery, symptom reduction, increased coping skills, and achievement of the highest level of functioning in the community.
Pay: $20-$24 per hour depending on experience
Schedule: 40 hours per week, Flexible with some evening and weekends required based on needs of families served, on call rotation for crisis calls
Location: Serving families in Caldwell and Burke Counties; Office located in Lenoir, NC
Work directly with at risk youth and families to implement interventions to address mentalhealth and substance use needs, reduce challenging behaviors, and provide referrals to other resources as needed.
Other duties include:
Conduct sessions in home, school and community settings.
Participate in development of Person Centered Plan and implement interventions from plan during sessions.
Assist with authorization process.
Participate in weekly staff meetings and maintain regular communication with team lead and clinical supervisor.
Provide crisis intervention strategies to ensure safety
Provide referral information for families to access needed services in community
On call crisis rotation
Persons who meet the requirements specified for Qualified Professional (QP) status have the knowledge, skills, and abilities required by the population and age to be served may deliver supports within the requirements of the staff definition specified in the service definitions. Relies on supervision and direction to provide quality services.
Education, Licensure, and Experience required for the position include:
Must meet the requirements of a Qualified Professional - Bachelors degree in Human Services Field and 2 years full time experience with youth diagnosed with MH condition; Bachelors degree in non Human Services field and 4 years full time experience with population served and/or Master Degree in Human Services field and 1 year Full Time experience with population served.
Physical requirements to perform essential functions of the job included:
Regularly required to lift 10 lbs. Must be able to lift a minimum of 15 lbs. Must be able to pull minimum of 10 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear

The Therapeutic Foster Care Therapist will provide services following the Therapeutic Foster Care (TFC) model. This includes comprehensive, trauma-informed, treatment-focused services for youth in a community placement that combines some of the strongest elements of residential care in the community and a home-like setting. The youth served in this program can be transitioned from congregate care into the community with community-based services. This alternative to congregate care should prevent future step-ups to congregate/residential care and will provide a placement option when youth leave psychiatric hospitalizations, residential facilities, shelters, Welcome Centers and other high care placement options. The goal is to provide therapeutic services while the youth is placed in an enhanced community based, home-like setting.
Essential Duties and Responsibilities ·Assess and evaluate mentalhealth needs as outlined by Medicaid ·Complete mentalhealth assessments and coordinate treatment plans using the Integrated Assessment and Treatment Planning process (IM+CANS) ·Provide individual and family therapy to assigned caseload of youth and including their biological and foster families to promote positive cognitive, behavioral or psychological changes as identified in the treatment plan, i.e, two times weekly and in accordance with the permanency plan ·Provide crisis intervention to youth and families and psychoeducation regarding specific clinical needs of youth ·Assist in placement stabilization by assessing barriers and developing stabilization plans, as needed to support youth and foster family ·Document all services rendered and any significant interactions using Medicaid and TFC guidelines and submit to supervisor in required time frames ·Participate in decision-making related to youth's care and treatment by attending internal and external clinical staffings and advocating for clients' emotional and physical well-being ·Communicate pertinent events/aspects of a youth's case with all team members on an ongoing basis ·Supervise visitation when therapeutic intervention is needed ·Adhere to Code of Ethics for Child Welfare Professionals, as well as ethical standards designated by any other ethical code one is professionally obligated to, e.g, ACA Code of Ethics, NASW Code of Ethics. ·Participate in individual and group supervision on a monthly basis ·Commit to providing evidence-based, trauma-informed services by attending and utilizing trainings on relevant and approved treatment modalities. ·Collaborate with other clinical staff on the development and presentation of trainings for staff and foster parents based on relevant psychological topics

Develop and implement treatment plans using a variety of generally accepted intervention techniques to address areas of assessed need
Plan and administer therapeutic treatment, such as behavior modification and stress management therapy, to assist patient in controlling disorders and other problems
Regularly discusses progress toward goals with client and encourages client feedback and client ownership of outcomes
Ensure there is a medical rule out of symptoms and coordinates the approach with medical and other providers serving the client
Adapt clinical methodology to the needs of the client using an evaluative tool and client feedback approach to ensure client service is meeting needs, and as needs change, adapts method and degree of therapy
Integrate all services provided by serving as a central point of communication for all mentalhealth related services, supportive services treatment and counseling
Maintain comprehensive client file with progress/process notes, completing assessments and quarterly reports, closing summaries, preparing reports for court as needed, testifying in court as needed, and participating in multidisciplinary staffing as needed
Provide community education services, consultation as assigned, and crisis intervention as needed.
Develop collaborative relationships with staff and other programs both within and outside of the agency
Represent agency at court hearings and Child and Family Team meetings and attends required staffing meetings
Monitor client progress and periodically reassess client's level of functioning as necessary, utilizing standard clinical instruments as well as diagnostic and statistical manual as required (minimally every 6 months)
Take a protective role in overall service by ensuring integration of clients other providers by including them in the service planning
Conduct relaxation exercises, peer counseling groups, and family counseling during clinical therapy sessions
Perform other related duties as needed

POSITION HIGHLIGHTS:
Provides independent diagnostic, psycho-social and professional counseling for clients (adults) with complicated substance use, mentalhealth and co-occurring disorders- with an appropriate level of supervision.
Provides independent diagnostic and psycho-social evaluations and counseling which focus on complicated substance use as well as the issues generated from its impact on daily living and individual and family relationships.
Evaluate the client's situation and their ability to deal with their problem; conduct an in-depth clinical interview to determine psycho-social and drug history, formulate a psycho-social assessment and initial treatment plan; provide in-depth counseling which can range from crisis intervention to dealing with personality disorders which are associated with their addictive and/or abusive behavior, and follow-up.
The caseload is characterized as complex with a preponderance of adults and families with substance use, mentalhealth and co-occurring disorder patients.
The counseling methods are best characterized as individual and group utilizing cognitive-behavioral, family, and supportive modalities. It is individualized depending on the needs of the clients.
In providing counseling services, the employees may deal with a variety of collateral issues such as finances, medical, vocational, health, and behavior.
Provide information to and confer with schools, employers, and families on treatment plans, behavior management, and other problems and needs.
Work with emergency referrals, conduct community educational workshops, provide professional services to referrals from the judicial system and statewide institutions.
May be subject to inside environmental conditions and exposed to fumes, odors, dusts, mists, gasses or poor ventilation and subject to work in close quarters. Employees may be exposed to human body fluids and are covered by the OSHA rules and regulations on blood borne pathogens.
Provides independent diagnostic, psycho-social and substance use evaluations and counseling which focus on complicated substance abuse and addiction as well as the issues generated from its impact on daily living and individual and family relationships.
ADDITIONAL INFORMATION:
Considerable knowledge of the practical application of different therapeutic approaches, group dynamics, crisis intervention, and family therapy.
Considerable knowledge of community resources.
Knowledge of the psycho-social, socioeconomic and behavioral problems and their treatment.
Knowledge of laws, regulations, and policies which govern the program.
Knowledge of medical terminology, disease processes and psycho-pharmacy.
Skill in establishing rapport with a client and/or family and applying techniques of assessing psycho-social, behavioral, and psychological aspects of the client.
Ability to establish and maintain effective working relationships with co-workers, supervisors, and members of civic, medical, social, and religious organizations.
Ability to formulate treatment goals and to record concisely required documentation.
Knowledge of various methods of psychological evaluation, including, but not limited to, conducting semi-structured diagnostic clinical interviews.
Ability to communicate effective in oral and written forms.
Must be able to physically perform the basic life operational functions of stooping, reaching, and walking, fingering, grasping, talking, hearing, and performing repetitive motions.
Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force constantly or frequently to move objects.
Must possess the visual acuity to perform extensive reading.

POSITION HIGHLIGHTS:
Provides independent diagnostic, psycho-social and professional counseling for clients (adults) with complicated substance use, mentalhealth and co-occurring disorders- with an appropriate level of supervision.
Provides independent diagnostic and psycho-social evaluations and counseling which focus on complicated substance use as well as the issues generated from its impact on daily living and individual and family relationships.
Evaluate the client's situation and their ability to deal with their problem; conduct an in-depth clinical interview to determine psycho-social and drug history, formulate a psycho-social assessment and initial treatment plan; provide in-depth counseling which can range from crisis intervention to dealing with personality disorders which are associated with their addictive and/or abusive behavior, and follow-up.
The caseload is characterized as complex with a preponderance of adults and families with substance use, mentalhealth and co-occurring disorder patients.
The counseling methods are best characterized as individual and group utilizing cognitive-behavioral, family, and supportive modalities. It is individualized depending on the needs of the clients.
In providing counseling services, the employees may deal with a variety of collateral issues such as finances, medical, vocational, health, and behavior.
Provide information to and confer with schools, employers, and families on treatment plans, behavior management, and other problems and needs.
Work with emergency referrals, conduct community educational workshops, provide professional services to referrals from the judicial system and statewide institutions.
May be subject to inside environmental conditions and exposed to fumes, odors, dusts, mists, gasses or poor ventilation and subject to work in close quarters. Employees may be exposed to human body fluids and are covered by the OSHA rules and regulations on blood borne pathogens.
Provides independent diagnostic, psycho-social and substance use evaluations and counseling which focus on complicated substance abuse and addiction as well as the issues generated from its impact on daily living and individual and family relationships.
ADDITIONAL INFORMATION:
Considerable knowledge of the practical application of different therapeutic approaches, group dynamics, crisis intervention, and family therapy.

RESPONSIBILITIES:
Education and Engagement
Engage individuals who may be at-risk and provide stage-appropriate recovery education and supports.
Assist in the orientation process for persons who are new to receiving mentalhealth and/or co-occurring disorders services
Assess individuals in their abilities, strengths, and assets towards the development of a recovery plan that addresses all domains of an individual's life.
Assist individuals in identifying their personal interests and goals using motivational interviewing.
Provide individual and group coaching, counseling, mentoring instruction, training, feedback and positive reinforcement for individuals in areas such as money management, grooming/hygiene, meal preparation, public transportation, and other independent living skills.
Educate individuals on evidenced based practices, e.g., illness management and recovery, Co-occurring disorders, supported employment, family psychoeducation, etc.
Demonstrate cultural sensitivity and competence and be trauma informed as relevant to the needs of individuals in recovery
Educate individuals in understanding their Wellness Recovery Action Plans as a means to recognize early triggers and signs of relapse, and use of individual coping strategies as an alternative to more restrictive services
