You will be asked to provide the supports identified below based on the needs of the individual in your care: Seek to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working. Supports are provided according to the person's abilities and interests while ensuring privacy and dignity. Prepare, teach and assist the person in food preparation, and cleaning of the home. Honor the choices, preferences and culture of the person you support. Assist in providing opportunities for social, emotional and spiritual needs to be met. Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities. Ensure that the home environment is safe and in good condition. Implement services and supports to address goals identified through the Person Centered Plan. This includes medical, psychiatric, recreational, spiritual, and emotional needs. Assist the person supported by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals. Participate in supervision as agreed upon in the RHA supervision contract. RHA will assist you in the following capacities: Provide support through a multidisciplinary team for crisis assistance and training as needed as you begin your journey as an AFL caregiver. Training includes RHA's proprietary ProAct training, which focuses on supporting resiliency, emotional stability and problem solving. Assist in coordinating respite care to support your needs as a caregiver. Supervision will be provided to assist you in adjusting to your new role. The frequency and amount of supervision will be determined in a collaborative relationship with your RHA supervisor. The position of Licensed Clinical Social Worker-Associate includes active engagement and coordination of care with internal resources including psychiatry and community support as well as external bodies including the legal system, families, and social support agencies. The position may call for back-up to the program director and or other therapists as needed. The position requires the individual to be an active and positive member of the clinical team. Based on setting, special skills and/or work with specific populations will be required. Relies on knowledge, experience, and judgment to provide quality services with very little supervision and direction. Must be an individual that holds a license issued by the governing board regulating a human service profession. Some services may require additional experience, licensure, or education.
