Job Summary

**Responsibilities:**

The Clinical Evaluator documents their evaluation and assessments in ASAP and presents the patients current status, recent history and collateral information to the CPEP psychiatrist who uses that information in conducting their own psychiatric evaluation and medical decision-making regarding disposition, Clinical Evaluator functions include identification and intervention around individual and systemic barriers to discharge; collaboration with other agencies, services, community providers as needed towards achieving discharge; notification and involvement of administrative personnel as needed to achieve a safe discharge.
**ESSENTIAL FUNCTIONS**
+ Conducts comprehensive clinical evaluation and risk assessments, including obtaining collateral information and review of recent clinical history for patients across the age spectrum as assigned who present to the CPEP in crisis. Provides assessment and intervention services to children and adults suspected of being victims of abuse, neglect or domestic violence in line with program and hospital policy and procedure

+ With latitude for clinical judgement and knowledge / understanding of CPEP regulations regarding discharges for CPEP; directs, develops, and facilitates discharge planning from CPEP for those patients who are evaluated as not in in need of acute psychiatric inpatient admission to inpatient or admission to the CPEP Extended Observation Beds. Problem-solving barriers to patient care, assigning tasks to CPEP Clinical Staff as needed. Engages CASAC, Peer Specialist or other members of the CPEP interdisciplinary clinical team in the assessment and intervention to achieve a safe discharge.
+ Includes safety planning with patient/family, making referral for outpatient follow-up or securing outpatient follow up as indicated and available.
**MINIMUM EDUCATION & EXPERIENCE**
+ Master's degree from a Counsel of Accreditation of Counseling and Related Education Programs (CACREP) accredited program with major course work in mental health counseling or master's degree in Social Work.

The successful candidate will oversee the growth of the division's established programs and the expansion and management of the Division of Palliative Care. The successful candidate should have leadership experience and be committed to faculty, staff, resident, and fellow education and mentorship. We seek an academic leader with a track record of leadership in programmatic growth, management, and oversight.
URMC is committed to palliative care. URMC was the very first academic center to achieve Advanced Certification in Palliative Care from the Joint Commission (2011). The responsibilities of the incoming Chief will include leadership and oversight of all programs within the Division of Palliative Care at URMC and all affiliated hospital sites. T Specifically, the incoming leader will participate in direct patient care and will play a key role in the recruitment and mentorship of all faculty and advanced practice practitioners, as well as the advancement of the clinical, research, and education missions of the URMC Department of Medicine. The Division Chief will interface with other Divisions in the Department of Medicine, Department of Medicine leadership, Medical School, and health care systems. The Division Chief will oversee and expand active palliative care services at Strong Memorial Hospital (adult patients with serious illnesses), FF Thompson, and Golisano's Children's Hospital and will collaborate with the palliative care program that supports Highland Hospital. **Responsibilities:**
Responsible to assemble, serve, and collect all patient meals, between meal snacks and floor stock. Maintains sanitation of work areas and follows sanitary food practices at all times.
SUPERVISION AND DIRECTION EXERCISED:Patient Service Assistant Manager, Assistant Director of Food and Nutrition, Food and Nutrition ServicesMACHINES AND EQUIPMENT USED:Microwave, oven, Refrigerator, Freezer, Coffee maker, Food and Meal carts, Telephone, Pager.TYPICAL DUTIES:1. Assembles and delivers patient trays for Breakfast, Lunch and Dinner meals in a timely manner to ensure temperature retention. Use judgment in approaching patients and families and in satisfying their requests for adjustments within their prescribed diet. In this capacity, serves as an effective representative of the hospital to patients, families and visitors.2. Passes all nourishments as well as fills water pitchers. Pass all tube feedings and infant formulas.3. Passes all patient specific menus. Assists with patient menu selection on the telephone or at bedside.4. Collects and submits all completed patient menu selections. Communicates patient preferences and requests for nourishment's to the Diet Technician as necessary.5. Collects all soiled meal trays from Breakfast, Lunch, and Dinner meals. Also, moves soiled carts to elevator area.6. Records all patients in s and Out's. This includes amount of food eaten and liquid consumed by all patients being monitored.7. Maintains appropriate levels of floor stock by completing a daily inventory and filling the "Blue Orders" as necessary. Stocks the blue orders supplies on assigned units.8. Responsible for supplying all necessary late trays and substitute selections for patients. Consults with the Dietetic Technician or Nutrition Services Supervisor and use judgment in adjusting trays for changes in patients' diet orders and/or new admission.9. Performs all assigned cleaning duties such as unit microwave, freezer, refrigerator, galley work surfaces.10. When microwaving trays, checks to see food is brought to proper temperature by using a thermometer to test.11. Assembles trays on the trayline for meal service to patients. Assists with limited food preparation, portioning and serving both in the tray assembly area and on the units, when appropriate.QUALIFICATIONS: High School diploma with ability to understand, read, write and speak English and with experience in a service environment; demonstrated ability to work independently; or an equivalent combination of education and experience.
