As a PhysicalTherapist, you welcome patients and put them at ease. You provide PT to patients with care and compassion to ensure rehabilitation success.
Will provide skilled physical therapy to Unity clients per N.Y.S. Department of Health and Federal regulations.

RESPONSIBILITIES:
Patient Evaluation - Assess patient impairment and current condition to evaluate rehabilitation potential; report on patient progress, communicate effectively with physician and other team members; counsel patient and family according to degree of disability or impairment.
Treatment & Planning - Develop physical therapy treatment projects for each patient, including training in functional mobility, use of assistive devices, therapeutic exercise, balance and coordination retraining, massage, heat, and cryotherapy; apply diagnostic and progressive functionality tests.
Leadership - Train, mentor, and coach team members to ensure standards of the department are met; address scheduling needs to ensure proper coverage is provided.
REQUIRED QUALIFICATIONS:
Master's Degree in Physical Therapy or Bachelor's degree with prior experience as a licensed PhysicalTherapist required.
Current New York State license or a limited permit as a Registered PhysicalTherapist.
If a New York State license or limited permit is not held, must be hired within 90 days of graduation from a New York State or APTA-accredited physicaltherapist education program and meet Practice Exemption under New York State Physical Therapy License Requirements.
If working in home care or traveling between locations, a NYS license is required.
PREFERRED QUALIFICATIONS:
Prior PhysicalTherapist experience preferred.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. * Educate clients and caregivers on therapy programs and home exercises.
* Monitor progress and report changes promptly to the physician.
* Ensure compliance with laws, regulations, and agency policies.
* Complete documentation at point of care in a timely manner.
*Minimum Qualifications:*
* Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.
* Bachelor's degree in physical therapy from an APTA-accredited program which includes an internship.
* Current PhysicalTherapist license in Connecticut.
* Minimum 1 year as a PhysicalTherapist or completion of 6-8 week preceptorship.
* Familiarity with home healthcare and regulatory requirements preferred.
* Maintain clinical skills and adhere to organizational policies and ethical standards.
* Ability to work with minimal supervision and maintain professional relationships with clients and families.
* Valid driver's license and reliable transportation required

Job Summary
Supervises a therapy services department; develops and implements overall therapy policies, procedures, services and programs. Accountable for expense management and team financial/quality/service/ efficiency targets. Aware of budget targets and team's impact to achievement of financial goals. Has discretion over limited expenditures for the work unit.
Plans, organizes and delivers specialized therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, and increase strength. Conducts examinations, evaluations, and interventions patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents.
Demonstrates and incorporate knowledge of current research into daily treatment of clients/patients/residents. May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Qualifications
Appropriate education level required in accordance with state licensure.
Two years of experience in therapy and rehabilitation.
Requires current licensure as an Occupational, Physical, or Speech Therapist in state(s) of practice and/or possess multi-state licensure privileges as required by position. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.
When applicable, may require valid driver's license and expected to travel up to 50% of the time based on needs.

A Brief Overview
The Sr. PhysicalTherapist is responsible for providing guidance and support to rehab staff in the assigned area. Plans and provides physical therapy treatment for patients in accordance with professional and MLH standards of treatment. What you will do
Provides day-to-day direction to associates in assigned area. Assists in scheduling to ensure adequate departmental staffing, participates in interviewing and training potential new Associates, provides input into Associate evaluations and corrective actions and collaborates with technical staff in organizing daily workflow.
Serves as a resource for associates, answering questions, researching issues, and resolving problems.
Performs initial patient assessments.
Assesses history, medical diagnosis, and current condition of assigned patients.
Ensures that policies, procedures, and standards of Physical Therapy are followed at all times.
Performs and implements patient treatments based on assessment and written plan. Performs patient treatment activities to reduce physical, emotional and social limitations including teaching self care skills and the use of adaptive equipment, including mobility, manual dexterity and daily activities; assisting patients to maintain maximum function by teaching proper body mechanics and adaptations to daily activities and assessing patient's potential to return to work and determining appropriate referrals.
Provides ongoing patient assessment through observation of patient and records. Documents and changes in patient's condition and any resulting changes in patient treatment (both physical and human response).
Evaluates treatment plan and patient's response to treatment. Re-assesses short-term goals as needed.
Identifies patient specific Physical Therapy rehabilitation learning needs and provides appropriate education for patients and their families. Assesses effectiveness of patient and family education. Documents learner response and outcomes of teaching.
Demonstrates initiative in identifying opportunities for self-development and enhancement of professional competency.
Performs other job functions as assigned or requested.

Qualifications
Appropriate education level required in accordance with state licensure.

When working at a Sanford Health Facility (not required but preferred for GSS Facilities): Must also be certified through the National Board of Certification in Occupational Therapy (NBCOT). When applicable, may require valid driver's license. Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

As an Occupational Therapist, your strong attention to detail and excellent evaluation skills are crucial to successfully restoring or improving each patient's optimal level of function, independence, and quality of life.
Assesses and restores patients' function, alleviates pain, and prevents disabilities by designing, planning, and implementing medically prescribed occupational therapy interventions. Provide superior customer service by modeling the Brand Promise and Core Values.
STATUS: PerDiem
LOCATION: Golisano Rehab - 1555 Long Pond Rd, Rochester, NY 14626
DEPARTMENT: Rehabilitation
SCHEDULE: Day & Evening 8-hour shifts; 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:00 p.m. Shift duration varies from 2 to 8 hours. Weekends vary
ATTRIBUTES
Degree in Occupational Therapy, bachelor's, masters or doctorate
Three (3) to six (6) months of previous Occupational Therapy experience preferred
Required Licensure / Certification Skills:
Current NYS Occupational Therapist License or limited permit as a Registered Occupational Therapist.
Compassionate, kind, and patient-focused
Excellent communication and customer service skills
For ElderONE Employment: 1 year of experience with the frail elderly population preferred
RESPONSIBILITIES
Evaluation & Development. Perform a complete initial evaluation of referred patients using evaluation protocols, standardized tests, and clinical observations; develop and implement treatment plans, including short- and long-term goals and methods to achieve them; adjust treatment plan and goals as needed
Documentation & Communication. Maintain complete documentation of patient evaluation, treatment, progress, and discharge status; counsel and teach patients and families; coordinate care plans with other involved health professionals, and discuss changes in treatment programs with MDs
Employee Development. Assist with the supervision and guidance of new personnel, assistants, students, and volunteers as assigned; develop and conduct in-service training programs

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
