The assessment consists of talking to the client in person at their home - and asking a few questions about their non-medical care needs (i.e. Bathing, dressing, grooming, light house cleanning, etc).
The assessments take 30 minutes. Travel time is paid from the person's home and back at $40/hour. Flexible schedule. Can work anytime from 8am to 8pm Monday through Sunday. JOB SUMMARY
The CNA supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. SUPPORT DUTIES
Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff.
Become knowledgeable of each individual resident's background, life history, disease, medical and behavioral care needs, and preferences in detail.
Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication.
Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training.
Encourage family involvement through volunteering and visitation.
Provide a clean, safe, non-hazardous unit for the residents and staff.
Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented.
Complete all required documentation.
Maintain all program policies and procedures and state and federal regulations.
Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach.

Principal Duties and Responsibilities

Support the administration and processing of payroll for assigned pay groups across U.S. and Canadian locations, ensuring accuracy and timeliness
Verify employee record changes prior to payroll processing

Complete all required paperwork according to program policies and procedures: weekly attendance records, USDA reports (daily meal counts), Child Abuse and Neglect Reporting form, monthly volunteer records, and all other records as required. Conduct and document observations/screenings as assigned.
CURRUCULUM AND ASSESSMENT
Work collaboratively with agency staff assigned to the classroom to prepare and conduct daily, individualized programming, for children and families according to team-developed lesson plans based on developmentally appropriate and best practices.
Use the Positive Guidance policy and personal safety guidance with classes including any program approved curriculum. Provide behavior management for individual children, including consulting with the Mental Health Consultant who will assist with further screening and ISP development as beneficial.
Follow all conduct and safety policies and procedures to ensure the children's safety at all times including ensuring children are within sight and sound of RWCFS staff at all times.
Participate in the IEP, IFSP process for serving children with special needs and implementation of appropriate support plan.
FAMILY PARTNERSHIP BUILDING
Advise mothers, fathers and guardians of classroom and Head Start activities, events, meetings and program initiatives, such as Family Literacy, Parent Center Meetings, Family Nights, Fatherhood Friday and other appropriate workshops, trainings or other opportunities for family engagement.
Assist and participate in planning, set up, conducting and clean-up of Family Nights.
Assist the lead teacher in making a minimum of three required home visits to each child's home and two parent/teacher conferences held in the family's home or at the center. These home visits will be used to provide orientation to the program, solicit parental input for their child's curriculum, and support effective transitions to the next appropriate educational setting, to share child's progress and to maintain a close partnership with parents.
FAMILY ENGAGEMENT
Communicate with parents through a variety of mediums to provide program orientation, involve them in their child's education, discuss their child's progress, suggest activities that can be continued in the home, and develop individualized programing.
Assist families with transition planning for children into and out of Head Start, and to the next appropriate setting.
TRANSPORTATION
Follow all procedures and policies in regards to Transportation
Provide bus monitor services as determined by classroom need, including but not limited to: providing supervision of children during the bus ride, child boarding and exiting, securing children in appropriate safety restraint systems, oversight of child pick up and release, responding to emergencies, assist with conducting/participating in monthly bus evacuation drills.
Implement activities on the bus as outlined in lesson plans.

MINIMUM REQUIREMENTS
Child Development Associate (CDA) credential or state-awarded certificate that meets or exceeds the requirements for a CDA credential, are enrolled in a program that will lead to an associate or baccalaureate degree or, are enrolled in a CDA credential program to be completed within two years of the time of hire. Transcripts are required to validate educational requirements.

Perform a total of 21 site checks per week at assigned TLP location(s). These checks may be split between multiple staff members per location.
Site checks to include the following:
Complete inspection of the physical space to ensure residents are in compliance with House Rules and Extended Supervision terms setby the Department of Corrections (DOC); including verifying they are within property limits, performing household maintenance, participating in lawn care and snow removal responsibilities, visitation policies, non-possession of contraband and maintenance of a drug, alcohol, and crime-free living environment.
Maintain, monitor usage, and light upkeep of household provided supplies, including but not limited to: dinnerware, utensils, lawnequipment, appliances, furniture, outdoor areas, linens, etc. Ensure appropriate food is supplied when necessary.
Create electronic reports based on observations for each site check (see details below).
Meet with Transitional Living Director on a regular basis via phone, video conferencing, or in-person to report on the on-going TLP operations at given location(s).
Complete required documentation, which includes:
Incident reports on all program rule or law violations.
Record daily summaries of resident interactions, resident consequences issued, suicide watch and related issues in communication log.
Complete site check log each day to include exact time of all site checks and whether there was face-to-face contact with the offender(s) during each checks
Maintain thorough knowledge of offender(s) criminal, family, mental health, employment, and substance abuse histories in order to assist in comprehensive supervision within the community.
Assist Director in ensuring residents are maintaining sufficient food items for three meals per day.
Collect, inventory, and secure property of residents who are unsuccessfully discharged within 24 hours of such discharge.
Maintain regular and reliable attendance.
Hold accountability to TLP Director. Supervision is ongoing and occurs through individual consultations, in services, departmental meetings, and as-needed when requested by the DOC.
