ESSENTIAL JOB FUNCTIONS:
Must remain patient and maintain good customer service when working with participants and internal or external staff
Escorts participants to all outside appointments that have been arranged by the scheduling department
Assists with the needs of the participant during any outside appointment (i.e. participant paperwork, toileting, mobility and possible transfers)
Documents services provided during the escorted appointment consistently and accurately via Electronic Health Records (EHR). Follows and fills out forms completely and neatly. Documentation is completed in a timely manner
Communicates with Rocky Mountain Health Care Services (RMHCS) for outside appointments, as necessary, including ensuring that the participant is signed in/out of the center and appointment
Views email throughout the day to receive updated scheduling information
Checks in with the scheduling department when a participant cancels or no-shows an outside appointment to assist with department needs
Maintains proper boundaries when dealing with participants. Notifies manager of any possible ethical or boundary issues immediately upon discovery
Must be available to work various hours to include early morning and late evening appointments. Possible overtime
Must remain patient and maintain good customer service when working with participants and internal or external staff

Qualifications
High school diploma or equivalent is required
One year of experience with a frail, or elderly population is required preferred
Must be knowledgeable in how to use a Computer and Microsoft Office applications
Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public
Capable of and experienced in assisting individuals with special mobility needs
Knowledge of how to operate medical equipment (wheelchair, walkers) desired
Ability to work independently with minimal supervision

ESSENTIAL JOB FUNCTIONS:
Autonomously initiates Medicare, SNAP, and/or Long-Term Care Medicaid Waiver application process for potential participants. This includes but is not limited to application completion, assistance with gathering and processing verification items, application submission, and coordination with SEP case managers to ensure functional eligibility.
Consistently tracks and follows-up on application status to ensure timely enrollment, smooth transition of care, maintenance of tracking database, and increased company census.
Regularly reviews Colorado Benefits Management System (CBMS) and contacts the Department of Human Services' (DHS) office to obtain eligibility status on new and currently enrolled participants. Updates department specific trackers to maintain accurate records of all current and potential participants.
Serves as the initial and regular point of contact with potential participants, current participants, and appropriate representatives for all items/areas related to eligibility as well as community resources.
Provides consistent professional written and verbal communication with community partners, participants, and appropriate representatives. Documents these encounters in the Electronic Health Record to retain accurate auditable files.
Requests and tracks requests for records and other required documentation from doctors' offices, financial institutions, retirement and investment companies, insurance companies/burial policy issuers, property assessors, social security offices, veteran's affairs, prior participant employers, and others.
Assists participants and/or their appropriate representatives with establishing income trusts at a financial institution of their choice when appropriate and confirming validity directly with state partner, Health Care Policy and Finance (HCPF). Coordinates the joint appointment with financial institutions and calculates required monthly deposits.
Independently and as appropriate guides clients and their representatives through Medicaid compliant spenddowns when requested to maintain eligibility.
Coordinates appropriate disability applications through the Medicaid appointed Action Review Group (ARG). Tracks disability determination and regularly communicates results with participant, ARG, and DHS and leadership of Intake and Enrollment.
Independently tracks on average 140-200 current participant's recertification (RRR) annually to ensure there is no lapses in coverage year to year. This includes but is not limited to coordinating with the SEP to complete Continued Stay Review (CSR) packet, revalidating all required documentation and applications through DHS, and working all cases out of pending status for outstanding items and/or information.
Supports RMHCS finance department in validation of Medicaid payments through monthly case management of outstanding accounts receivable cases. This includes working closely with DHS and HCPF to correct cases, validate finances, and/or guide participants through needed documents.
Independently maintain and updates participant shared payments for Assisted Living Facilities (ALF) or Skilled Nursing Facilities (SNF). Uses participant's recording income to calculate payment due to facility and individual's personal needs allowance. Conducts semiannual liability project to validate data, reviews with current participants, obtains signatures, files in participant records, provides to claims, and sends to applicable facility.
Prepares a variety of periodic and special reports, which may require gathering data from several sources, compiling such data, and arranging it into proper format.
Maintains professional affiliations and any required certifications.

Qualifications
Graduate of an accredited Medical Assistant program is required
Preferred Certified or Registered as a Medical Assistant with one of the National Certifying organizations; The American Association of Medical Assistants (CMA), The American Medical Technologists (RMA), The National Center for Competency Testing (NCMA), The National HealthCareer Association (CCMA), and National Association for Health Professionals (NRCMA). The employee will be expected to take and pass a Medical Assistant examination from one of the above listed Certifying Agencies within or immediately upon completing 640 hours (4 months) of employment. Certification will be required to remain eligible for employment
One year of experience with a frail or elderly population preferred, typically in a hospital, long term care facility, or home health care program
Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records

Supports the Recreation Therapy team in completing initial assessments, formulating the initial care plan, supporting appropriate 6 - month documentation, and annual assessments. Daily documentation as assigned by the Recreation Therapy and Wellness Manager
Develops engaging programs by consistently monitoring the diversity of the participants served and their interests/needs
Drives the Recreation Therapy Wellness calendar based upon those interests/needs as identified in the initial care plans, assessments, and participant feedback
Ability to provide recreation therapy co-treatments and strategies such as Leisure Education and Leisure Counseling to other program departments
Provides training and programmatic feedback to Recreation/Wellness Assistants

Assists in the organizing of special events

Qualifications
BS Degree in Recreation/Therapeutic Recreation required

Minimum 2 years solid program development and implementation preferred
Knowledge of the principles, practices, standards, and techniques of Recreation Therapy
Solid background in the care plan process doing assessments, identifying potential problem areas, developing subsequent goals and objectives
Possess critical thinking and problem-solving skills
Ability to work closely with other departments to provide recreation therapy co-treatments/ interventions as needed
Demonstrates effective conflict management skills during situations with clients, co-workers and family members while maintaining professionalism. Must be able to effectively enforce rules and regulations at all times
Must be a flexible team player able to identify challenges and recommend resolutions

Qualifications
Registered Nurse (RN) licensure in the State of Colorado preferred; Bachelor of Science in Nursing (BSN) preferred.
A combination of relevant education and demonstrated experience in infection prevention, participant safety, quality improvement, or clinical operations may be considered in lieu of RN licensure, where permitted by applicable regulation and organizational policy.
Minimum of one (1) year of experience in infection prevention, quality improvement, risk management, or participant safety within a healthcare setting (preferred).
Demonstrated knowledge of infection prevention and control standards, including CDC, OSHA, and applicable State of Colorado regulations, or the ability to rapidly acquire such knowledge.
Required to obtain the CDC Nursing Home Infection Preventionist Training Certificate within 120 days of hire.
Certification in Infection Control (CIC) preferred.
Experience conducting or supporting infection and safety surveillance, data analysis, trend identification, and root cause analysis.
Ability to develop, implement, monitor, and evaluate infection prevention and participant safety programs, including audits and corrective action plans.
