Essential Functions:
Independent in evaluation, treatment, documentation, and follow through with patients, their families, and providers. Takes direction from supervisor and mentors; functions well with appropriate supervision.
Ability to handle simple to moderate case complexity, involving one to two outside providers, mild behavioral or mental health issues and limited co-morbidities.
Supervises a minimum of one grad student per year (8-12 weeks) plus two observation students, as appropriate
Provides education and coaching to others outside the physical therapy discipline. Examples include describing the role of physical therapy, advocating for physical therapy, instructing other disciplines on physical therapy treatment, educating physicians and other providers, instructing and coaching parents in home programs
Carries out special projects as assigned
Education and Experience:
Bachelors, Master's Degree or DPT degree from an accredited Physical Therapy program
Licenses & Certifications:
Current physical therapy license in the state of Wisconsin. If working in the Birth to Three program, must have a valid and current State of Wisconsin driver's license and a vehicle in good operating condition.

$20,000 Sign on Bonus for full time new hire!40 hours per week Clinic open 7 AM -6 PM Monday through Thursday, 7 AM-5:30 PM Fridayable to work 4 or 5 days per week and must work until close on at least two days.This position requires participation in weekend/holiday rotation in hospital (average 3-4 weekend and up to 1 holiday per year)
Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
May supervise physical therapy assistants following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. May supervise physical therapy extenders (rehab aides/techs) in administering treatments to patients and will teach them appropriate techniques to perform with specific patients. May supervise physical therapy students after completing one year of clinical experience.
Minimum Job Requirements
Education
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards .
Certification / Registration / License
Physical Therapy License in the State of Illinois or State of Wisconsin, must be licensed in the State upon which you work. Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)

This position helps provide a homelike environment, which enables residents to learn and develop daily living skills as well as socialization and leisure skills.
**Essential Job Functions:**

+ Assists consumers in learning independent living skills, which are defined in the resident's Individual Service Plan. (i.e. laundry, housekeeping, cooking, time and money management.). As well as encouraging management of symptoms through support services and education.
+ Participates in ensuring that nutritious community meals are planned, prepared, and served within the home, involving training residents in food preparation serving and storage.
+ Monitors consumer medication adherence as directed by their physician.
+ Reports to Master's Level Clinician, WHCG RN, or physician any changes in client behavior and functioning.
+ Ensures that residents maintain acceptable standards of personal hygiene.
+ Ensures that residents leave for their day program, job or school appropriately groomed. Facilitate transportation as necessary.
+ Ensures that the house is clean at all times and complies with DHS 83 CBRF regulations.
+ Ensures that all physical repairs are reported to the Property Management Department.
+ Performs room, water, temperature or any other daily weekly monitoring that is assigned by the Program Coordinator including labeling and putting away the weekly grocery order.
+ Practices emergency evacuation procedures and performs fire alarm system checks with residents and documents these drills.
+ Documents residents' daily behavior and routine in the resident daily log consistent with their Individualized Service Plan goals.

This 19 bed unit serves a great purpose in the community treating various health needs. Our compassionate team is comprised of Psychiatric hospitalists, RN's, CNA's, Occupational Therapists, mental health therapists, utilization specialists, and administrative professionals. This environment allows for collaboration and learning on a daily basis. Our team is dedicated to the highest quality of care and fosters great learning opportunities.

KEY ACCOUNTABILITIES:
* Assist RN in maintaining and managing a safe and therapeutic environment for the patients.
