The Pharmacist must have in depth knowledge of medications and their uses. Duties will include: •Reviewing prescriptions/orders issued by physicians or other authorized prescribers to assure accuracy and appropriate medication management of patients/residents. Promotes use of formulary items by conferring with prescriber as necessary when non-formulary medications are ordered. Processes orders within PIS and assists in maintaining correct patient profiles and charging. Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis •Monitoring patient profiles to ensure rational drug therapy. Review of drug allergies, drug interactions, and duplicate therapies will be performed during order entry. Reviews and monitors drug therapies that requires dosing adjustments as authorized by the physician.
