How you will contribute:
· You will greet donors as they enter and exit the donor floor.

JOB DESCRIPTION
The Detroit Medical Center (DMC) Registered Nurse (RN) is a member of the Patient Care Services Team, and is responsible for the satisfactory completion of nursing care by the nursing team. Summary Description
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including:
Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care
Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
Education provided to physicians, patients, families and caregivers

· Completes comprehensive assessment within 24 hours of patient admission to identify and document the anticipated transition plan for patients

· Completes Complex/Psycho-social assessment and plan for patients identified as high risk for readmission.
· Provides psycho-social assessment and intervention for patients identified with identified needs including behavioral health, lack of support systems, financial barriers, end of life, and/or medication adherence.
· May delegate the implementation of the transition plan to LVN/LPN or Assistant staff. And follows up to ensure the transition plan is completed timely and accurately
· Ensures all elements of the transition plan are implemented and communicated to the healthcare team, patient/family and post-acute providers
· Provides information to patients to make informed choices when community services per Tenet policy
· Completes Final Discharge Disposition Form Assessment for Medicare patients per Tenet policy

· Screens patients for factors that may affect the progression of care and intervenes as needed to promote timely and appropriate throughput

How you will contribute:
· You will greet donors as they enter and exit the donor floor.
· You will perform venipuncture of donors and programming of plasmapheresis machine.
· You will monitor donors during the donation process and manage donor reactions.
· You will perform all tasks required for the setup, verification, operation, and troubleshooting of plasmapheresis equipment within scope of training.
· You will install, prime, and disconnect disposable sets on the plasmapheresis machines
· You will stock supplies, break down empty cartons and assist with proper disposal.
· You will take and record donor pulse, blood pressure, and temperature measures and monitor electronic donor questionnaire system.
· You will perform finger sticks, test sample, and record other donor measures to include hematocrit, total protein, and weight.

Summary Description
Under general supervision and following established policies and procedures, performs diagnostic CT exams utilizing appropriate imaging equipment.
Verifies patient identity and reviews patient's medical record for appropriateness of exam or procedure. Interviews patient to obtain, verify or update medical history and assesses risk factors such as medications, pregnancy and/or psychological indicators.
Explains procedure to patient, including side effects of pharmaceuticals administered, risks, benefits, alternatives and follow-up. Sets up equipment and acquires appropriate images as per script.
Reviews images, makes initial observations, documents exceptions from established protocols and communicates data to radiologist. Recognizes and responds to medical emergencies as appropriate.
Provides physician-prescribed post care instructions to patients. Participates in process improvement activities within radiology practice including but not limited to quality of care, patient flow, reject-repeat analysis and patient satisfaction.
Provides imaging education to residents, student technologists and new employees. Able to take call/standby as required by the department. May perform the duties of a Radiologic Technologist, as needed.
Qualifications:
Minimum Qualifications
1. Registered by the American Registry of Radiologic Technologist (ARRT) in Radiology (R) or Computed Tomography (CT) preferred.
2. Required to have or successfully complete American Heart Association (AHA) Basic Life Support - Healthcare Provider (BLS) training by end of orientation period.
