Case Manager
GENERAL DESCRIPTION: Provides coordination of services or targeted case management to adults and children with cognitive, emotional, physical, and developmental disabilities residing in residential or community settings in Ohio, Marshall and Wetzel Counties in West Virginia.
QUALIFICATIONS:
Bachelor's degree in a human services field (social work, special education, psychology or counseling), or RN with valid WV license and one or more years' experience in the I/DD field or
*Bachelor's degree in a human services field and less than one (1) year experience in the I/DD field, or
*Bachelor's degree in a non-human service field and one year of experience in the I/DD field.
*(Restriction) - Upon hire, this person will be under the supervision of the Service Coordinator Supervisor. Clinical supervision involves review of clinical activities and review of case notes for six (6) months. This must be verified by supervisory documentation once per month.
RESPONSIBILITIES:
Provide service coordination services to assigned Waiver consumers as set forth in the current Medicaid Title XIX I/DD Waiver Manual and agency policy.
Creates and submits calendars, notifications along with the coordination of Individual Program Plans (IPP) for assigned consumers.
Distributes completed Individual Program Plans to all IDT members within 14 days of the IDT meeting.
Schedules and chairs interdisciplinary treatment team (IDT) meetings for assigned consumers in accordance with funding requirements and consumer needs.
Coordinates the APS Waiver budget and Medicaid budget process and the development of assigned consumer's budget, assuring that needed information is submitted to the Care Connection system and appropriate prior authorizations are granted prior to services being delivered.
Maintains a working knowledge of federal and state regulations and agency policies regarding, but not limited to: Licensure regulations, Medicaid policies, and Medicaid Waiver Manual.
Assists Utilization and Quality Assurance Specialist in identifying and resolving billing issues related to assigned consumers.
Schedules and attends a minimum of monthly home visits and bi-monthly site visits for each assigned consumer and completes appropriate DD-3 forms accurately and in a timely manner.
Assures that re-certification of I/DD Waiver Medical eligibility for each assigned consumer is maintained.
Assures completion of necessary documentation in a timely manner to maintain the eligibility of Medicaid TCM consumers.
Ability to accurately calculate and predict number of units of services needed by the consumer to fulfill the service needs of the consumer as set forth in the treatment plan.
Competence in basic word processing and spreadsheet skills, ability to accurately input information into Medicaid Title XIX I/DD Waiver ("Waiver") website.
Ability to maintain the highest levels of confidentiality and safeguard participant, agency and administrative documentation and/or information.

POSITION SUMMARY
The Health Services Administrator is responsible for effectively and efficiently managing the institution's overall health care delivery system and monitoring all health service contract activities; consulting with the Regional Manager or designee regarding routine institutional issues of an administrative nature, and discussing issues of a complex or unusual clinical nature involving patient management with the Site Medical Director.
Coordinates and monitors preparation of the institutional health services budget with the Regional Manager and Statewide Director of Operations.
Supervises requisition of supplies and equipment to ensure appropriate usage and availability; and maintain property and equipment control records as required.
Assists in developing and maintaining staff schedule in accordance with contract terms of staff coverage. Responsible for making changes in time schedules, and granting special requests for time off, if applicable.
Approves monthly statistical reports and ensure timely submission; complete all necessary reports on a timely basis.
Ensures the confidential maintenance and safekeeping of all medical records and information to comply with HIPAA regulations.
Monitors overall performance utilizing national standards and contract deliverables.
Performs administrative employee relations functions to include: supervision of performance, recruitment and selection, developing job descriptions, performance management, implementing corrective action and individual development plans, scheduling and approving PTO, etc.
Supervises scheduling of patients for outside appointments. Reviews initial hospital specialty referrals with the Medical Director to ensure appropriateness and oversee interactions with medical specialists.
Resolution of audit findings by the development, implementation and monitoring of corrective action plans.
Evaluates and respond to Corporate Risk Management Department inquiries, complaints or grievances by patients, family members, lawyers or other advocates.
Works jointly with Clinical and Education staff to ensure that appropriate training in CPR and First Aid is completed per regulatory and contractual requirements.
Collaborates with co-workers and coordinate internal projects, as directed
Participates in a company and state approved CQI Program
Upon receiving the necessary training and/or instruction, performs other related duties as required or assigned
JOB REQUIREMENTS

GENERAL DESCRIPTION: The BSP performs assessments, develops habilitation program plans to increase independent living skills, provides training in the person-specific aspects and methods of a plan of intervention or instruction to primary care providers for consumers with significant maladaptive behaviors. The plan of intervention will require a behavioral guideline, protocol, or behavior plan depending on the consumer's need. The BSP also provides monitoring and evaluation of the plan of intervention or instruction for adults with cognitive, emotional, physical, and developmental disabilities in Ohio, Marshall, and Whetzel Counties in West Virginia.
Qualifications:
Must have a valid driver's license.
Successfully pass criminal background requirements set forth in licensure regulations.
Competency in Microsoft Word, Excel and the ability to learn new software programs, utilized for scheduling, billing, consumer data and other related purposes.
Possess 1 of the following:
Ø Four-year degree in a human service field and one or more years' experience in the IDD field.
Ø Four-year degree in a human service field and less than one year of experience in the IDD field. (Restrictions - must be under the supervision of the case manager
supervisor. Clinical supervision involves review of clinical activities, review of case notes, and review of treatment plans for six months. This must be verified by supervisory documentation once per month).

BSP RESPONSIBILITIES:
Provide BSP services to assigned clients as set forth in the Medicaid Title XIX I/DD Waiver Manual, alternative funding source requirements and agency policy regardless of participant's funding source or lack thereof.
Maintain compliance with Medicaid Title XIX I/DD Waiver regulations
Performs assessments to establish baseline data for behavioral interventions.
- Drafts positive behavioral support plans, behavior management plans, behavioral protocols and accompanying documentation.
- Analyzes data and makes modifications as needed.
Creates program strategies, techniques, materials and methods to assist consumer in obtaining objectives in accordance with documented priority needs through the annual assessment as approved by the treatment team.
ADDITIONAL RESPONSIBILITIES
Attends ongoing training as requested or in accordance with need and reads scholarly books and articles on topics related to the field of I/DD and community resources.
Presents community education as requested.
Attends staff meetings, social and recreational events as directed.
Participates in governmental, community, and civic organizations or groups as directed.
Provides training to new and existing staff as directed.
