Responsible for the delivery of safe, effective, and quality patient-family centered care in the OR and other areas of perioperative services for all patient populations. Facilitates operative or other invasive procedures by preparing and providing the required sterile instruments, supplies and equipment. Maintains the sterile field and anticipates and responds to the needs of the surgical team.
This position is for entry into a 9-month formal education program, which incorporates didactic, simulation, virtual reality, and a comprehensive exam that must be passed. Formal training will occur at an off-site simulation lab in addition to practicing the skills and implementing the knowledge learned in the RRMC surgical suite under direct guidance. As the training progresses to working in specialty areas the trainee will work under the guidance of team leads. Taking call with the primary preceptor will begin during the second month and functioning independently in the scrub and circulator roles for all services for routine cases by 9 months.

Successful completion of RRMC OR Training program within 9 months.

A DAY IN THE LIFE OF AN INTAKE REGISTERED NURSE (RN)
As an Intake RN, you are responsible for the timely assessment and processing of referred clients. Under the direction of the Director of Quality and Intake Services, you ensure timely effective response and initiation of home care and hospice referrals in compliance with regulatory, quality, industry and VNA & Hospice of the Southwest Region standards. Responsibilities include but are not limited to the following:
Screen, triage, and process requests for agency services inclusive of admission criteria, discipline and programs, utilizing and demonstrating solid understanding of home care /hospice criteria, and payment sources.
Assess the appropriateness of each referral according to VNAHSR agency policies and procedures and in collaboration with the primary physician and referral sources.
Utilize all computerized referral source programs and reporting programs timely and effectively.
Facilitate and ensures quality, coordinated and timely clinical services as the patient moves across the continuum of care.
Oversee and assists with coordination of work flow processes as assigned.
Identify and communicates priority visit needs and potential high-risk situations to the designated supervisor to facilitate timely and appropriate scheduling of professional and support staff and services.
Ensure all pertinent data is accurate and complete utilizes all available information and resources to promote quality care and the achievement of patient goals and outcomes.
Work collaboratively with scheduling, billing and quality departments to ensure and facilitate the home care/hospice services are in compliance will all regulatory and agency guidelines.
Ensures F2F documents are complete in accordance with CMS guidelines and assists in obtaining any missing documentation or other required paperwork as needed at the time of referral.
QUALIFICATIONS FOR A REGISTERED NURSE (RN)
Graduation from an accredited school of nursing required. Bachelor's of Science in Nursing preferred.
Home care/Hospice experience preferred.
Must be a team-player, with excellent communication, interpersonal, and customer service skills.
Ability to obtain valid Vermont State Registered Nurse licensure by date of employment required.
Valid state driver's license required. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required.
Weekend work may be required periodically.
