This role assists with process improvement which supports the reduction of medical/health errors and other factors that lead to unintended adverse patient outcomes. Core responsibilities include assistance with safety assessments, activities of the Patient Safety Committee, the incident reporting system and follow-up, and provides education on the system-based causes of medical errors. Job duties: *Investigation, analysis and reporting of incident/medical error reports. *Gathers and manages necessary information for root cause analysis, patient harm events, etc. *Abstracts, submits and reports data for publicly reported patient safety indicators as well as external benchmarking programs. *Provides education and support to all areas of the health system as needed to promote patient safety *Reviews / maintains policies and procedures aimed at reducing medical errors and patient harm *Prepares and maintains data for the patient safety dashboard and assists the Patient Safety Officer with corrective action plans. *Stays abreast with current Patient Safety guidelines, regulations and recommendations. *All other patient safety duties as assigned by the Patient Safety Officer.
