Job Summary: The In Home Program provides independent living services funded by Waivers/SILS for clients with developmental disabilities and mental illness. This position is responsible for assisting clients in maintaining or increasing independence in their home, a foster home, or their family home. Individuals in this position are responsible for working with clients with challenging behaviors, dual diagnosis, and program plans that require a high degree of skill/experience on the part of the staff person. Direct Support Professionals act independently and without direct supervision. Major Job Responsibilities: Personal Cares: conduct meal preparation, provide personal cares such as bathing/hygiene activities, assist with routine schedules (sleep time, getting ready for work/appointments); increase clients' quality of life through recreational activities, community outings, leisure activities, reading to clients, playing games, etc. Household cares: assist with laundry, cleanliness of living areas, and safe walkways/sidewalks. Provide client behavior assistance, support & behavior modification according to the directives in the Client Treatment Plans, developing and maintaining a therapeutic and skill building relationship with clients; respond to challenging client behaviors with the knowledge obtained from Nursing staff, Supervisory staff, Case Managers, and by increased self-learning by identifying the incremental steps to behavior change. Provide medical/health care and medication administration to clients as directed by nursing staff and the treatment plan, as well as observe and keep accurate documentation. Communication: conduct daily review of Communication logs, complete client Progress Notes, communicate with family members, caseworkers, & supervisors as requested/when necessary. Maintain direct support skills by completing all required program trainings and attending all home/house team meetings. Job Requirements: Education: High School diploma or equivalent. Job Related Experience: Experience working with individuals with disabilities/impairments preferred. Additional Requirements: Valid MN driver's license required. Must pass our insurance standards to be an agency driver. Must also pass a background check. Position Summary: The Community-Based Therapist provides mental health therapy to children, adolescents, adults, and families in homes, community settings, via telehealth, and at Nexus Family Healing offices. Using a trauma-informed, strength-based approach, the therapist delivers individualized care through evidence-based models like EMDR, DBT, TFCBT, and Systemic Family Therapy. Primary responsibilities: * Intake & Assessment * Complete intake forms and diagnostic assessments. * Ensure timely and accurate clinical documentation. 2. Trauma Informed Care * Complete CANS assessments using TCOM principles. * Develop, update, and monitor individual treatment plans (ITPs) based on assessments and CAT/CANS. * Provide crisis intervention and safety planning as needed. * Participate in mandatory clinical supervision and team case consultation meetings. * Participate in family collaborative intake meetings. * Provide client-centered, strength-based therapy in collaboration with families. 3. Therapy Services * Deliver individual, group, and family therapy aligned with treatment goals. * Meet direct service and documentation requirements. * Support discharge planning and post-care coordination. 4. Case Management * Coordinate referrals and collaborate with families, schools, and providers. * Attend supervision, team meetings, and required trainings. * Maintain compliance with all clinical and regulatory standards 5. Community Collaboration * Communicate client needs with care teams, families, and community partners. * Ensure confidentiality and HIPAA compliance. Requirements Required Education and Licensure: * Master's degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields required * Fully licensed in Minnesota as an LICSW, LMFT or LPCC * OR clinical trainee level with 5 years' shelter/residential experience * Valid driver's license required.
