Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Responsibilities include but are not limited to:
Conduct and Ensure Compliance of Technical Security Operations: Lead and execute Technical Surveillance Countermeasures (TSCM), Foreign Visit, Gift Inspection, Digital Counterexploitation, and Technical Evaluation activities. Analyze and Mitigate Security Deficiencies: Conduct in-depth analysis of complex physical and technical security issues, ensuring NGA's compliance with ICD 705 (SCIF physical security standards) and DoD standards for collateral areas. Perform Signal Intelligence and Threat Assessment: Conduct Radio Frequency (RF) signal of interest searches, assess and locate identified signals during surveys and IPMS operations, and evaluate electronic processing equipment for security vulnerabilities.
Generate Comprehensive Reports and Communications: Prepare all required correspondence, documents, briefing materials, spreadsheets, and threat reports in accordance with customer guidelines. Minimum Qualifications:
6+ years experience writing and editing of technical documents
Must have an Interagency Training Center (ITC) TSCM Certification
Shall complete at least 40 hours of discipline-specific development or refresher training every fiscal year. The NGA customer will sponsor Government-offered, discipline-specific development or refresher training IAW National and DoD standards
Shall meet minimum training requirements, within one calendar year of assignment, for access to DoD networks in accordance with DoDD 8140.01, by attaining and maintaining at least baseline certification for DoD 8570.01-M Information Assurance Technician Level II (IAT2) compliance.

The Center Clinical Manager (CCM) is an active leadership role responsible for the day-to-day management of back office center operations, which may include but is not limited to: clinical quality, compliance, human resources, patient experience and direct team member supervision. Collaborates with Center and Divisional leaders to provide VIP service to patients, family members and staff.

Manages medical equipment and supply inventory. Ensures availability and proper preventative maintenance of emergency equipment. Troubleshoots malfunctions, initiates service and repair requests and inspects new equipment.
Ensures key performance targets are met, including but not limited to: Key Performance Metrics (KPI) metrics, wait times, patient experience and staff engagement and turnover.
Leads high quality selection, hiring and on-going training and education for clinical staff and Pharmacy/Medication Technicians. Facilitates orientation and onboarding of new clinical team members ensuring they are fully trained in their duties including philosophy of care, patient flow and clinical competencies.
Ensures regulatory compliance with OSHA, HIPAA and other governing agencies. Enforces clinical and quality standards and internal audit requirements of ChenMed.
Completes all required training to dispense medications from medication room.
Performs other duties as assigned and modified at manager's discretion.

Bachelor's Degree in Music with a concentration in Music Therapy and/or Music Therapy from an American Music Therapy Association (AMTA) approved university (or undergraduate equivalency program approved by AMTA).
Board Certified as Music Therapist (MT-BC) by the Certification Board for Music Therapy (CBMT)
Previous experience with terminally ill patients / families is highly desireable
Must have reliable automobile, valid driver's license and the minimum state required liability auto insurance.
Responsibilities:
As a Hospice Music Therapist you will:
Complete the music therapy assessment with patient and family when referred in accordance with the interdisciplinary plan of care
Plan music therapy interventions
Guide the patient in the use of creative therapeutic activities for the purpose of improving or maintaining mental and physical function and quality of life
Evaluate the effectiveness of music therapy services with the patient and / or family-of-choice
Provide in-services to staff, hospitals, nursing homes, and other facilities as requested and determined appropriate by therapist.
As one of the couneslors in the interdisciplinary grou (IDG), the MT servces as the counselor at IDG meetings, participates in on call rotation, and assists with sign ups and consents for newly referred patients
Support the bereavement program at the site level
Required Certifications and Licensures:
Board Certified as Music Therapist (MT-BC) by the Certification Board for Music Therapy (CBMT)
Must be a licensed driver who can travel to all business locations

Business Analysis job description (Must-have):
Practiced in Agile and Scrum processes
Liaison between technical groups and the business
Ability to interact and handle different groups with competing needs
User Story or Requirements writing, definition, and elicitation
Ensure User Story meet INVEST or other current best practices
A willingness to provide and accept constructive feedback on projects
Motivated to independently research topics using available tools
Ability to perform impact analysis on systems, users, projects, and dependencies
Organizational skills: ability to coordinate all communications strands of a project
Proficiency in Word, Excel, and other MS Office products
Salesforce Experience with an understanding of the SF data structure.
MuleSoft Experience
Wireframing experience
Integration experience
Business Analysis Tools (Nice to have):
Business Process Modeling Notation (BPMN)
Jira project management experience
MS Visio or Lucid charts experience (BPMN)
Used to construct process flows to clearly show impact of project (as-is and to-be flows)
Integration to ICOMS
Integration to CSG

Certified Nursing Assistant (CNA) Responsibilities:
Shift Needed: 3:00 PM to 11:00 PM
As a certified nursing assistant (CNA), you will assist with lifting, turning, moving, positioning, and transporting residents in your nursing home.
You will assist your nursing home's residents with grooming, quality of life and self-care.
You will assist the nursing staff with resident care.
You will chart accurately and maintain records, in a timely manner.
Compensation details: 15-21.5 Hourly Wage
