This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care; Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards. Qualifications: Education: Required: Master's of Social Work Experience: Preferred: 1 years of acute hospital experience and/or 1 year experience in outpatient behavioral health setting Certifications: Required: LiCSW or LMHC.
