Essential Job Functions: Maintains high levels of facility cleanliness and appearance by performing routine cleaning tasks daily which consist of, but not limited to vacuuming, dusting, mopping, and complete bathroom cleaning. Refreshes bath area by cleaning tub, toilet, and sinks; removing used towels and bathmats. Presents a positive image within Salemtowne by having a friendly and cooperative relationship with residents and staff. Supports & promotes the Salemtowne Shines culture and philosophy. Contribute to teamwork by assisting other staff as assigned or needed. Protects residents and employees by adhering to infection control policies and OSHA requirements. Ensures responsiveness to residents' needs by restocking housekeeping cart at end of each shift. Keeps residence clean by removing trash, newspapers, recyclables, and returning Salemtowne property to its proper location. Keeps residents safe by following operating instructions; troubleshooting breakdowns; performing preventive maintenance; calling Maintenance for repairs. Completes other duties as assigned and enhances housekeeping reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.     The MDS Coordinator prepares individualized care plans for Health Care Center residents and patients according to State and Federal regulations; ensures that these care plans are followed and updated as needed. Monitors quality of care standards according to the CMS 5-star quality of care indicators. Essential Job Functions: Sets the MDS schedule and assures Minimum Data Set forms are completed in a timely manner by overseeing multi-disciplinary team. Ensures resident care is delivered as ordered according to Federal and State guidelines by making rounds and reviewing residents' charts periodically. Ensures individual care plan approaches are being carried out or changed as needed by interacting and communicating daily with the patient staff. Ensures weekly care plan meetings are performed on time by the multi-disciplinary team.
