Certified Home Health Aide
You will provide a range of personal care services to patients such as bathing, dressing, assisting with grooming, hygiene and household chores.
RESPONSIBILITIES:
Provide patient care as directed on aide care plan, reports and documents observations and care performed
Observe and report changes in client's condition to Supervising Clinician
Assist the patient in ADL's (Activities of Daily Living) as directed on aide care plan
Communicates patient's supply needs to Supervising Clinician or Health Care team
Display professional behavior at all times
Comply with all areas of corporate compliance program and HIPPA regulations
Document thoroughly and accurately in format prescribed by the agency
Provide companionship and patient care as directed on care plan
Help patients and their families understand instructions given by medically trained staff
QUALIFICATIONS:
Must have vehicle, valid current driver's license and automobile insurance
Minimum 1-year experience
High School Graduate or equivalent
Current certification as a Certified Home Health Aide by the State of California required
Current CPR card
Documentation to verify work experience and legal right to work in the U.S.
Report to work in a timely manner and adhere to call-in times for schedule changes
Able to provide two verifiable references

Registered Nurse (RN)

As a Home health RN, you will develop a plan of care in collaboration with the patient, family/caregiver, and physician orders including frequency and duration of nursing visits, specific care or treatments to be provided, appropriate goals, medications, and other disciplines needed. You will make regular visits to monitor the patient's conditions, illnesses, or injuries.
RESPONSIBILITIES:
Perform comprehensive assessments in the patients' homes and identifies patient's needs.
Develop a care plan in collaboration with patient/CGs, healthcare provider, case manager, and/or supervisor as assigned and document appropriately.
Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis.
Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects.
Take verbal orders from the healthcare provider as necessary promptly and documents appropriately.
Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly.
Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues.
Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required.
Serve as a preceptor when needed

Must have a vehicle and current driver's license and automobile insurance.
Interpersonal and communication skills necessary.
Ability to make home visits and provide skilled nursing assessments and care in the home
Home Health and Home Care Home Base (HCHB) documentation experience preferred but not required.

Speech Language Pathologist
You will make home visits to assess and treat speech, language, social communication, cognitive communication, and swallowing disorders. You'll be responsible for providing detailed notes and documentation regarding your home therapy visits and patient progress.
RESPONSIBILITIES:
Knowledge of the principles and practices of speech pathology and provides speech therapy services in accordance with state practice act
Perform an initial assessment and evaluation of the patient and establish a plan of care, including scheduling subsequent visits determined by the SLP to be clinically appropriate.
Coordination of care by care conferencing with other healthcare team members
Observe, record, and report to the Primary Care Physician the patient's treatment, response, and progress including significant changes
Obtain orders to revise the plan of care as clinically appropriate
Perform re-evaluations of the patient's progress at least every thirty (30) days and update the plan of care as necessary
Complete comprehensive assessments and OASIS data as necessary
Teach patients and caregivers about the patient's impairments and provide support and educational materials to support interventions and foster independence

As a Home Health Occupational Therapist, you will evaluate and treat patients with injuries, illnesses, or disabilities in their homes. You will help patients recover, improve, and maintain skills needed for daily living.
RESPONSIBILITIES:
Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
Evaluate home environment for hazards or barriers to more independent living
Identify equipment needs
Provide supervision to the Occupational Therapy Assistants
Conduct an initial evaluation/assessment and report findings to the Health Care Provider and RN and develop a plan of care promptly for patients
Document initial evaluation and treatment plan and re-evaluations of patient's progress at least every thirty (30) days and update plan of care as necessary.
Complete comprehensive assessments and OASIS data, as indicated on a timely basis.
Observe, record, and report to the Health Care Provider the patient's treatment, response, and progress and promptly report significant changes
Obtain orders to revise the plan of care
Instruct patient/CG in the use of alternate methods and devices for upgrading independence in activities of daily living
