MentalHealthCounselor acts as a case manager and may conduct individual/group rehabilitation, collateral services, and linkage to formal and informal resources. ESSENTIAL DUTIES AND RESPONSIBILITIES: Works with the Therapist, Family and the treatment team to develop an individualized Family Plan taking into account each family's strengths, goals and needs. Assists families in obtaining appropriate services within their communities. Assists families in obtaining appropriate activities to strengthen family relations. Is flexible and creative in providing support and services to families in their home, at school sites, in agency offices, or other off-site locations as outlined in the Treatment Plan or as needed by the families. Models appropriate social skills for family. Carries as caseload of CFSP families providing ongoing assessment of the family's basic needs as well as the assessment of needs for services. Provides and/or arranges transportation services for families as appropriate. Schedules intakes and completes all appropriate documentation needed for opening cases. Responsible for documentation including Progress Notes/ Unit of Service logs, Case Management Needs Assessment, relevant portions of DMH and agency's documentation. Participate in weekly staff meetings, Treatment Team meetings clinical update meetings, family meetings and individual and group supervision sessions. Assist with crisis intervention as needed including being available to families after hours and on weekends when appropriate Is respectful and sensitive to each family's cultural and individual needs and customs as appropriate. Is expected to become familiar and establish relationships with community agencies and service providers in order to be aware of possible resources available to CFSP families within their community. Complete all documentation in a timely and comprehensive manner. Performs other duties as assigned
Qualifications
Bachelor's degree in human services or related field or equivalent, bilingual, and valid ID and at least 2 years of experience working with SED children and their families. Bilingual (Spanish) Strong writing skills, organizational skills, computer skills. Knowledge of community resources. Possess valid Class C California Driver's License and driving record meets SFVMHC, Inc. insurance requirements. How You Will Contribute:
The Therapist is responsible for providing individual and group therapy sessions for clients in a specialized Outpatient facility. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment. Therapists act as the primary contact for clients and families throughout the treatment process and serve as a client advocate with the treatment team. Position acts as a liaison with referral sources, clinical partners, and individuals to coordinate care throughout the treatment process.
Essential Responsibilities:
Demonstrates a high level of clinical skill in assessing the clinical needs of clients, determining appropriate interventions, and conducting individual and group therapy sessions in accordance with these needs.
Completes psychosocial evaluations, treatment plans, contact logs, discharge plans and other reports required for each client in compliance with State, accrediting bodies, quality guidelines and payor source guidelines.
Conducts weekly individual and family therapy sessions for assigned caseload.
Monitors behavioral interventions for each client and provides skills coaching as needed.

Qualifications
What We Are Seeking
Position requires a master's degree in social work, marriage and family therapy or psychology. Must hold CA licensure (LCSW, LPC, LMFT, or LMHC), active associate number or intern number from the Board of Behavioral Sciences, in good standing. Position requires a minimum of one (1) year experience working with clients with substance use disorders and/or borderline personality disorder. Certification or significant experience in providing Dialectical Behavioral Therapy (DBT), strongly preferred.
Other Requirements
Clearance of TB test, fingerprinting and state clearance, and any other mandatory state/federal requirements.
Current CPR and First Aid certification.

Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program). Experience: Minimum of 2-3 years of relevant experience providing behavioral health services in a clinical or community setting. What We Are Looking For
The Behavioral Health Specialist will work under the direction of the Department Director and Clinical Manager to deliver high quality psychotherapeutic services to children, adults, and families within a community clinic setting. This role involves assessing, diagnosing, and treating individuals with behavioral health concerns, including mental illness and substance abuse disorders. The Behavioral Health Specialist collaborates with other healthcare professionals to develop and implement individualized treatment plans tailored to each age group and family dynamic. Behavioral Health Specialists conduct therapy sessions, provide crisis intervention, and offer support and education to patients and their families, ensuring a holistic approach to care that integrates behavioral health with overall wellness.

ESSENTIAL FUNCTIONS AND DELIVERABLES
* Conducts thorough assessments of behavioral health needs for children, adults, and families.
* Diagnoses mentalhealth conditions and substance abuse disorders based on clinical evaluations.'
* Develops and implements individualized treatment plans in collaboration with patients, families, and multidisciplinary teams. Set measurable goals and objectives for treatment progress.
* Provides evidence-based individual and group therapy sessions tailored to diverse age groups
* Develops and maintains specialty-based clinics focusing on area of expertise for brief therapeutic services.
* Utilizes a variety of therapeutic techniques and approaches to address behavioral health issues.
* Offers timely crisis intervention and stabilization for patients in acute distress.
* Develops safety plans and coordinates emergency responses when needed.
* Works closely with medical professionals, social workers, and other healthcare providers to ensure integrated care.
* Participates in case conferences, group consultation, and treatment team meetings to review patient progress and adjust treatment plans.
* Participates in utilization review activities as assigned.
* Educates patients and their families about mentalhealth conditions, treatment options, and coping strategies.
* Provides resources and referrals for additional support services as needed.
* Maintains accurate and up-to-date patient records, including assessment results, treatment plans, and progress notes.
* Monitors and evaluates the effectiveness of therapeutic interventions and treatment plans.
* Prepares reports and documentation for internal use and external communication as required.
* Participates in community outreach programs and educational events to promote mentalhealth awareness as assigned by department leadership.
* Contributes to quality improvement initiatives and adhere to clinic policies and standards.
* May provide psychotherapeutic services and support to various patient populations and clinics based on operational and programmatic need.
* Will provide individual clinical supervision for associate level therapists and/or student trainees as assigned.
QUALIFICATIONS:
* Education: Master's degree in psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field from an accredited institution.
* Licensure/Certification: Valid and current license as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) issued by the State of California Board of Behavioral Sciences. Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program). As a requirement of this position, you must receive EPIC certification for the module you have been hired into. Valid California driver's license, and auto insurance.
* Experience: Minimum of 2-3 years of relevant experience providing behavioral health services in a clinical or community setting. Demonstrated experience working with children, adults, and families, including experience in individual and group therapy. Previous positions should include responsibilities related to assessment, diagnosis, and treatment planning. Experience in crisis intervention and management is highly desirable. Relevant experience from previous positions which demonstrate a strong foundation in behavioral health services and includes specialized training or certification will be considered. Preferred experience: Certification in specialized therapeutic techniques (e.g., CBT, DBT, play therapy, CADC, EMDR) and experience with electronic health records (EHR) and documentation systems.
* Essential Technical/Motor Skills: The ability to use a computer on a routine basis and complete required documentation. Use of word processor, spread sheets and internet. Ability to transition to Electronic Health Records.
* Interpersonal Skills: Strong clinical assessment and diagnostic skills. Proficiency in individual and group therapy techniques. Ability to develop and implement effective treatment plans. Excellent crisis intervention and management abilities. Strong communication and interpersonal skills, with the ability to collaborate effectively with patients, families, and multidisciplinary teams. Fluency in English and Spanish preferred. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team environment.
* Essential Mental Abilities: In-depth knowledge of mentalhealth conditions, treatment modalities, and evidence-based practices. Understanding of child and family dynamics, as well as developmental and cultural considerations. Familiarity with community resources and support services. Must have critical thinking for comprehensive treatment planning, empathy for building therapeutic rapport, cultural competence for engaging diverse populations, emotional intelligence for effective communication, adaptability for navigating complex environments, and strong communication skills for collaboration with interdisciplinary teams.

Summary

A MentalHealth Rehabilitation Specialist provide interventions in homes, community, or other locations depending on client needs.
Education and/or Experience
AA Degree in psychology, social work, child development or related field plus 6 years' experience providing behavioral intervention to children and adolescents with emotional disturbances or intensive needs. POSITION RESPONSIBILITIES:
Identify individual client strengths, needs and resources. Assesses and evaluates client's level of psychosocial impairment, self-care potential, family and support network availability, independent living needs, financial status, and social/recreational needs. Develop and coordinate service plans.
Assist clients in developing independent living skills and resources, including residential site visits on a weekly basis.
Transport clients to and from appropriate services as necessary. Assist clients in navigating the transportation system within the San Fernando Valley, apply for bus cards and transport to doctor appointments, SSI/SSA, and other locations as deemed appropriate by Program Manager.
Work as part of a multi-disciplinary treatment team, participate in all-staff, team meetings, and community meetings as assigned.
Provide intensive field-based services for enrolled and potential clients.
Maintains current knowledge and familiarity of public entitlements.
Knowledge or professional experience providing services to clients with dual diagnosis and/or substance abuse disorders.
Ability to communicate effectively (and develop trust) with a culturally diverse population, including homeless and those suffering from mental illness.
Network and maintain strong working relationships with community resources, police, businesses, churches and other agencies serving the homeless population, to gain potential client referrals.
Will perform other duties as necessary, to be determined by the Program Manager.
Provide knowledge and understanding to staff and clients of resources and referrals for achieving increased income stability through attainment of SSI/SSA/SSDI/GR or other means of increase income. Provide vocational referrals (VES/DOR) and assisting clients with achieving these goals.
Complete Progress Notes/Unit of Services logs within the Centers/DMH 48 hour time frame. Maintain productivity standards as assigned by Program Manager and/or Director.
Oversee clients at program housing.
Perform other duties as necessary.
Qualifications
EDUCATION:
BA in the field of Human Services, or related degree in Social Services, i.e. Social Work preferred.
EXPERIENCE:
Minimum of two years' experience in a behavioral health care setting experience with homeless chronic mentally ill.
Knowledge of DSM-V-TR required.
Chemical dependency experience preferred.
ADDITIONAL REQUIREMENTS:
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Must successfully complete the Center's HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.

The MentalHealth Associate/Intern will provide on-site mentalhealth services to formerly homeless children and their families living in permanent supportive housing in Pleasant Hill. The position can be part or full-time depending on your needs. Individual supervision, group supervision and training are all provided. Associates provide services in an on-site play therapy room, in family homes and in the community. Some evening and occasional weekend hours will be required.
Applicants of diverse backgrounds who have experience working with low-income families with multiple challenges are encouraged to apply. Applicants with competency working with culturally diverse populations are strongly desired.
WHAT YOU'LL DO
Support formerly homeless children to reach their full potential (75%)
:
Provide milieu-based mentalhealth services to
individual children
living in permanent supportive housing who are experiencing emotional challenges. Many of these children may be receiving EPSDT (Early and Periodic Screening Diagnosis and Treatment) services.
Provide on-site individual and group mentalhealth therapeutic services to children.
Assist in delivering on-site parenting support groups to families.
Collaborate with case managers and the youth enrichment coordinator to provide support for individual children and their families, using a team approach. Assist with staff supervision of volunteer programs and community activities delivered to families served by CCIH. These may include evening and occasional weekend commitments.
Work with other staff members to provide community resources for families and individuals, such as social and health related activities, life-skills and employment support, and youth enrichment.
Stay on top of the paperwork and administrative details to keep the program running (15%)
:
Complete accurate and timely EPSDT chart notes for all services provided to children who are assigned under this contract.
Meet individual EPSDT contract goals for hourly billing, including averaging a minimum of 10 - 12 hours per week (depending on work schedule) of direct service to clients who are assigned under this contract.
Assist the services team with appropriate clinical documentation, including progress notes, service delivery documentation, reports, and forms.
Continuously improve your skills and work with team members and community partners to provide excellent services (10%)
Participate in weekly group and individual supervision, using that time to work on developing therapeutic skills and clinical knowledge, as well as exploring growth opportunities as a mentalhealth provider.
Participate in clinical training as possible/needed.
Represent CCIH in a professional manner in all circumstances.
Maintain awareness of culturally diverse consumer populations and perform duties in a culturally competent manner.
Attend all required meetings, including but not limited to: CCIH staff meetings, Internal team meetings, and linkage meetings with other agencies.
Qualifications
Position Qualifications:
Master's degree in Social Work, Marriage and Family Therapy, or Counseling.
Registered with the Board of Behavioral Sciences as an intern working toward licensure as an LCSW, LMFT, or LPCC.
Experience with low-income and disenfranchised populations desired.
Outstanding written and verbal communication skills.
Computer proficiency in the use of Microsoft, and database applications.
Must pass LiveScan screening and TB test.
