Home Health Aid Job in Alamosa, CO
Cares for elderly, convalescent, or disabled persons in their home as assigned by the RN.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Performs personal care /activities as assigned on a written aide care plan under the direction and supervision of a registered nurse or physical therapist.
Reports any change in patient condition to the case manager or an office RN promptly.
Communicates any unusual demands or requests (not on care plan), immediately to supervisory RN/PT so that care plan may be redefined if necessary.
Is available for on-site RN/PT supervisory visits per CMS regulations.
Attends 80% of scheduled in-services and monthly staff meetings. Ensures that the required 12 hours of annual CEUs are obtained and proper documentation of attendance is submitted to the office.
Maintains current and ongoing certification with the Colorado State Board of Nursing and current CPR certification.
Participates in QA/UR Review activities as requested.
Promptly records visit and pertinent information in the patient record and submits paperwork per agency guidelines. Submits all paper documentation on the day the service is provided.
Must be knowledgeable of patient rights and be respectful of patient and family needs including confidentiality at all times.
SUPERVISORY RESPONSIBILITIES This job has no supervisory responsibilities.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. HHA must be able to pass, upon hire, a competency skills lab, including (not limited to) VS measurement, skin care, bathing and bowel program as delegated.
12 hours of CEU must be maintained annually, and current CPR certification. The

EDUCATION and/or EXPERIENCE
Current certification through the Colorado State Board of Nursing and one to three months related experience.
LANGUAGE SKILLS Ability to read and comprehend at an adult level. Ability to effectively communicate verbally and in writing with nurses, patients, family members, and other individuals involved in the patient's care.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
