Acts is currently seeking qualified candidates for our Wellness Home Services Provider (GNA) position.
In this role, you will be responsible for delivering compassionate, high-quality care to residents within all levels of care. Provides direct, hands-on assistance with daily living activities (bathing, grooming, dressing, eating, toileting, mobility, etc.) while prioritizing resident safety, comfort, and dignity at all times. Monitors residents' physical and emotional well-being, promptly reporting any changes to the appropriate supervisor to support proactive and personalized care.
Requirements The ideal candidate will meet the following requirements:
Completion of an approved Geriatric Nursing Assistant (GNA) program; or equivalent verifiable training and experience that meets all applicable state requirements
Current CPR certification
(or obtain within three months of employment)
If responsible for transporting a resident, must maintain a valid state driver's license and provide proof of current automobile liability insurance

Responsibilities
Responsible for assisting in the delivery of resident care through assessment, planning, implementation, and evaluation
Review resident medical records to ensure nursing practice provided is within State and Federal guidelines
Review all referrals for consultation, treatment or resolution of problems are valid and applicable to the documented nurse plans
Evaluate and monitor resident responses to daily implementation of nursing care to ensure the provision of resident care plans are met
Implement physician's orders, nursing interventions and procedures as needed for the care of the resident
Seeks opportunities for professional growth and expansion of responsibilities to maintain competency levels
Set and implement goals and policies consistent with the philosophy and purpose of FutureCare's nursing facility
Qualifications
Must hold an active Registered Nurse license issued in the state of Maryland
1 year of nursing experience preferred
Previous experience caring for residents in an acute or long-term health care facility a plus
Must possess a current CPR license
