North Platte Valley Medical Center, a Critical Access Hospital (CAH), is seeking a skilled and compassionate Physical Therapist (PT) to join our outpatient/inpatient rehabilitation team. This is an excellent opportunity to make a meaningful impact in a rural healthcare setting, where you'll provide individualized therapy and work closely with patients across the lifespan to help them restore function, improve mobility, and achieve their health goals.
Position Overview
The Physical Therapist will evaluate, plan, and implement comprehensive treatment programs for patients with a wide variety of musculoskeletal, neurological, and functional conditions. You will collaborate with physicians, nursing staff, and the interdisciplinary team to ensure continuity of care in both outpatient and occasional inpatient settings.
Key Responsibilities
Perform patient evaluations and develop individualized treatment plans.
Provides physician prescribed physical therapy.
Provide therapeutic exercises, manual therapy, modalities, and functional training.
Educate patients and families on home exercise programs and injury prevention.
Document all care in compliance with hospital and regulatory standards.
Collaborate with physicians, nursing staff, and other healthcare providers to ensure coordinated care.
Maintain knowledge of evidence-based practices and current treatment guidelines.
Supervise and mentor Physical Therapy Assistants (PTAs) and support staff as appropriate.
Maintain HIPAA complaint and follow policies pertaining to PHI.
Must work collaboratively as a team to ensure patient satisfaction and continuity of patient care.
Ensures a safe environment for both inpatient and outpatient patients
Assists with our acute swing bed patients and assessing their needs.
Must attend all mandatory departmental and facility meetings
Qualifications
Graduate of an accredited Physical Therapy program.
Current Wyoming Physical Therapist license (or eligibility).
Current BLS certification (or ability to obtain upon hire).
Strong communication and interpersonal skills.
Prior experience in outpatient or hospital settings preferred, but new graduates are encouraged to apply.

Full-Time | Monday - Friday, 8:00 a.m. - 5:00 p.m.
No weekends or holidays required (occasional flexibility for patient needs).
