I've never needed to work more than 40 hours a week, and occasionally get permission to leave for medical appointments and such

Zero autonomy, your boss is your king. No matter what you want to do, you need permission. If your boss says no, that's it, no appeals. Most bosses never ask questions and would rather slander employees than find out why they did(n't) do something.
