THE HEALTH CENTER PROVIDES A WIDE RANGE OF MEDICAL, DENTAL AND BEHAVIORAL HEALTH SERVICES. CLINICAL SERVICES ARE DELIVERED IN A PATIENT-CENTERED MEDICAL HOME MODEL OF CARE. THIS MODEL IS A WAY OF ORGANIZING PRIMARY CARE THAT EMPHASIZES COORDINATION AND COMMUNICATION. THE HEALTH CENTER PROVIDED SERVICES TO ROUGHLY 19,100 UNIQUE PATIENTS WITH APPROXIMATELY 35,100 MEDICAL PATIENT VISITS, 22,900 DENTAL PATIENT VISITS, AND 6,300 BEHAVIORAL HEALTH PATIENT VISITS.
