YOUR ROLE
Thyme Care Inc., the management company to Thyme Care Medical PLLC, is the employing entity with your duties to be performed for Thyme Care Medical PLLC, a medical practice, and its patients. As a Thyme Care Oncology Nurse Navigator, you'll be a vital clinical resource for our members and their care network, offering triage, support, and education during their cancer journey via phone, email, and video communication. Under the guidance of our Nurse Team Lead, you'll conduct comprehensive clinical assessments, oversee member health, and facilitate end-of-life care discussions. Within your first three months, you will:

SUMMARY:
A Triage Nurse is a professional registered nurse with oncology-specific clinical knowledge who offers individualized care and clinical guidance to patients, families, and caregivers to assist with ongoing healthcare needs.
PRIMARY TASKS AND RESPONSIBILITIES:
Under general supervision, following established policies, procedures, and professional guidelines, provides care to patients by triaging oncology patient calls regarding treatment, surgery, and appointment information.
Monitor and provide patient symptom management.
Manage high-risk, complex patient care with the goal of minimizing emergency department and inpatient readmission.
Assess barriers to care to address patient, caregiver, or family needs to achieve optimal patient outcomes.

Position Summary: Provides professional nursing care for clinic patients following established standards and practices. Collects specimens, including blood, urine, etc. Performs catheterization. Processes paperwork for appropriate specimen collections.
Contacts patients regarding missed appointments.
Performs in-office testing and treatment such as EKG, audiometry, nebulizer and oxygen.
Arranges for patient testing and admission which may include patient education.
Maintains exam rooms for necessary supplies and materials. Ensures safety and cleanliness. Prepares list of medical supplies needed and maintains equipment to ensure a clean and safe environment.
Prepares contaminated instruments and other related materials for transport to hospital for sterilization.
Maintains patient confidentiality.
Completes appropriate forms for managed care referrals and gets authorization when necessary.
Calls in prescriptions to pharmacy. Calls lab results and test results to patient or automated test system.
Checks encounter form for missed charges. Has Medicare waivers signed prior to service.
Acts as patient advocate in attempting to locate agencies appropriate to patient needs, i.e. Meals-on-Wheels, Department Services for Crippled Children, etc.
Required Qualifications:
Graduate of an accredited school of nursing.
One year of professional nursing experience in a clinic setting preferred.
Possession of a State Registered Nurse License.
CPR Certification.

By joining our nursing team, you will be helping thousands of employers better manage their workplace injuries and improve the quality of healthcare for their employees. Nurses who are successful in this position must be able to talk on the phone for long periods while typing and navigating through various software applications simultaneously. Our nurses must be able to visualize an injury while on the phone and clarify details about the injury while following our propriety algorithms to guide the triage of the injured worker.
Training:
Training for this role will last 5-6 weeks, with 2.5 weeks of classroom instruction and 2.5 weeks of precepting. These first 5-6 weeks of training are held Monday through Friday, from 8a-4p CST. The training schedule is non-negotiable, and all training must be successfully completed within the 6-week time frame. Following training, you will transition to your permanent schedule between the hours of 12p and 2a CST with an every-other-weekend requirement and holiday rotation. Changes to the permanent schedule are not allowed within the first 12 months of employment.

Establish rapport and a relationship with the patient and family in order to understand their needs and expectations and to assist them in setting realistic and mutual goals. Communicate plan of care, including changes and issues related to plan of care to patient/family, physicians and other members of the healthcare team.

Document all care management assessments and interventions.

The Role
The Field Resource Nurse (LPN/LVN) is part of Float's centralized Clinical Resource team. This role provides real-time phone support to independent contractor nurses in the field, helping them troubleshoot workflows, documentation, and therapy-related questions during active visits. You'll be the first line of contact for Float's field nurses, ensuring they feel supported, informed, and confident while providing patient care.
In this role, you'll also be reviewing clinical documentation and helping create plans of treatment.
This is a great opportunity for a licensed practical or vocational nurse with experience in infusion or specialty pharmacy who communicates clearly, stays calm under pressure, and enjoys helping other clinicians succeed.
This is a first-of-it's-kind role for Float Health as we scale our Clinical Resource Team. You'll be a part of our inaugural LVN/LPN team. We're looking for flexible, adaptable team members who will help us further define this role for the future.
What you'll do:
Provide real-time phone support for field nurses during patient visits.
Help troubleshoot common issues related to equipment, medication handling, documentation, and pharmacy protocols.
