Ensures compliance with regulatory, accrediting and company policies and procedures. May assist in problem solving with provider, claims or service issues.
Demonstrates the minimum knowledge, skills and abilities to care for the individualized needs of the patient to include physical, psychological, socio-cultural, spiritual and cognitive needs as well as functional abilities including the need for diversified use of such practices. What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).

Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.

The program will help Black Fathers in Washington, DC to improve overall wellness, economic well-being, and reduce child support arrears for hundreds of families in the District of Columbia, now and in future generations. After a four-month ramp-up phase, the program will serve 150 families over a 2-year period.
The Family Therapist will play a critical role in transforming and shaping the lives of Black fathers and their families by providing therapeutic services to individuals, couples, and families, focusing on improving communication, resolving conflicts, and fostering healthier family dynamics. This role involves conducting assessments, creating personalized treatment plans, and delivering evidence-based interventions tailored to the unique needs of each family. The Family Therapist will work closely with the fathers to address issues such as trauma, mental health disorders, substance abuse, and behavioral challenges. Additionally, the therapist will collaborate with multidisciplinary team members to ensure holistic support and progress tracking. Strong therapeutic skills, cultural competence, and the ability to work with diverse family structures are essential for success in this role.

Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for ones discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM certification upon entry into this position OR within two years of entry into the position.
CCM or ACM certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications :
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an a ssociate d egree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelors degree and graduate degree is preferred.

Opportunity time frame:

Learning model:
