As the Transitional Case Manager (TCM), you will be facilitating seamless transitions for patients from facility settings to post-acute care. You will verify home health orders, assess care requirements, and ensure continuity of care. Your role includes assessing patients' health literacy, involving patients and families in care planning, and providing education to improve outcomes and promote self-management. You will implement rehospitalization reduction initiatives for patients at risk and communicate with healthcare providers throughout the transition.

Primary Responsibilities:
Educate patients on post-discharge follow-up, homebound criteria, and obtaining prescriptions
Assess readmission risk using the LACE tool
Ensure patients and families have agency contact information
Coordinate ancillary services (DME, Infusion) as needed
Assist in preparing for patient care post-discharge
Liaise between the agency and healthcare providers
Communicate patient transfers and coordinate resumption of care
Provide feedback on readmissions and non-admit decisions
Perform other duties as assigned

Required Qualifications:
Current, unrestricted RN, LPN, or SW licensure in state of practice
Current CPR certification
1+ years home health experience or 1+ years of hospital case management experience
Current driver's license, vehicle insurance, access to a dependable vehicle or public transportation

The Trainer/Certification Caseworker is responsible for the organization and delivery of the Pre-Certification Training Series and ongoing training for the program. This position is also responsible for annual home study updates, training, and consultation for foster families. The candidate assesses the physical safety of the home, character and background of the family, their experience in working with children, their knowledge of child development, and any risk factors to children who may be placed in their home. Ongoing support is provided to the family after certification to assist the caregiver in meeting children's needs. Families are also monitored for meeting continued certification requirements. The ideal candidate must familiarize themselves with the foster care policy, rules and regulations, as well as Volume 7 regulations for foster care certification. This position will be responsible for the presentation of applicants to the Certification Review Team for approval. Employee will match foster children and providers, consult with staff from the Collaborative Foster Care Program and provide placement information to ensure children are placed in levels of care to meet their needs.
Work involves conducting bi-weekly community information meetings (“Information Night”) regarding the Collaborative Foster Care Program. This will require evening and weekend work.
Makes collateral contact with community resources to learn about and secure information or services for the foster care program. This position is available to present at other agency meetings, such as investigation reviews, family engagement meetings, matchings for adoption, group supervision, etc. This position participates in staffings as needed within the agency and outside the agency with other professionals.
Manage a workload of up to 8 certification cases at any given time while in this role.

Our partner school provides alternatives to traditional schooling that utilize 21st century technology. The school provides students with computer technology, allowing them to progress at their own pace. The Intervention Specialist is responsible for overseeing assigned e-learning classes; students may be working at home or working in school computer labs. The Intervention Specialist is responsible for maintaining compliance of student special needs records including IEPs and ETRs; monitoring and reporting student progress for caseload students as well as in classes taught; and providing instructional resources to students per IEP specifications. This position will assist the TIS administration in providing effective and efficient educational services to any partner school affiliates. The use of technology to develop these relationships is essential. This position works mainly from a home office but reports to the main office in Marion, Ohio, or other regional locations at dates and times as assigned by the administration.
PRIMARY RESPONSIBILITIES INCLUDE:
Maintain compliance of student special needs records including IEPs and ETRs.
Use technology to identify, select, and modify instructional resources to meet the needs of the
students with varying backgrounds, learning styles, and special needs.
Monitor and report student progress on all IEP goals and objectives for student caseload and
in courses taught using IEP reporting software as well as Student Information Systems.

The Role
You will be responsible for supporting and contributing to the overall brand social strategy that acts as a vehicle to grow, support, engage, and strengthen the relationship between Love, Bonito, and our customers and the greater community of women, all over the world. You will deep-dive into our Social Media pillar and function as a full member of the team to achieve business objectives through your daily responsibilities and your team-specific projects.
Main Responsibilities
Brainstorm, manage, and develop the content schedule, briefs, and assets for social channels
Support in timely content delivery, scheduling, and postings
Write creative copy with compelling calls to action to generate traffic and conversions
Support with social reports, insights, and payment to partners/vendors/creators
Participate in brainstorming sessions/discussion within the team on how to improve in the delivery of content on all social platforms
Assist in market research and competitor analysis
Administrative support and daily upkeep of internal trackers, calendars, and influencer lists
Manage timelines and prompt follow-ups with influencers for various campaigns
Work closely with the team to plan, support and execute events
Conduct media monitoring for brand coverage across various channels including digital, social media, and online forums
Content Production
Develop a solid understanding of the Love, Bonito's brand, our vision and our target audience
Support in developing content for social channels (Facebook, Instagram and TikTok)
Stay on top of trending videos and sounds on TikTok and adapt them quickly for Love, Bonito's channel
Create a sound library that might be applicable for future Instagram Reels and TikTok videos
Participate in content creation discussion for social channels (Facebook, Instagram, TikTok)
Support in the daily maintenance and administrative upkeep of team assets
Community
Source and manage UGC database
Close tracking of campaign and BAU seeding performance to KOLs
Monitor for competitor and industry news, including potential issues that could impact the business
Work closely with other functional teams to troubleshoot or expedite KOLs orders
Provide community support to the Social Media Team - monitoring and responding to comments, questions, and DMs on the respective social media channel
Requirements
Ability to adapt quickly and respond to social media trends in a timely manner
Well-versed in content creation for Social Media channels like Instagram, TikTok, Facebook and Pinterest
Quick thinking and problem-solving attitude in tackling obstacles that may compromise workflow, capacity, and/or quality
Strong video content production skills
A good eye for balance and composition
Able to adapt to an ever-changing and dynamic environment with professionalism, positivity, and flexibility under pressure
Prior experience in social media content creation (especially TikTok) for lifestyle and fashion brands a plus

As an IHBT Family Therapist, you will take on a hybrid role of therapist, case manager, advocate, community builder, and leader-with the goal of helping families heal and thrive. You will provide individual therapy to parents, lead family therapy sessions, and supervise a team of two Quality Mental Health Specialists supporting the same household.
We are seeking individuals with LSW or LPC licensure who are community-driven, detail-oriented, and unapologetically passionate about racial and social justice. This role requires someone with a deep understanding of complex trauma and how it shows up in marginalized families-disrupting relationships, emotional regulation, and long-term stability. Our therapists must be ready to meet families where they are, with clinical skill and an unwavering belief in their capacity for change.
Key Responsibilities:
Serve as the primary therapist for the parent or guardian in the family unit
Lead and facilitate family therapy sessions using GRO's relational, phase-based model
Supervise and coordinate the care provided by two Quality Mental Health Specialists
Provide comprehensive case management-including housing, employment, education, and behavioral health support
Schedule and facilitate case consultations with GRO mentors and team members working with the same family
Collaborate with schools, probation officers, and other system partners involved in the family's life
