School-Based Site Coordinator

Salary Information: Full-time salaried position
Hours: General Hours are 10:00 am to 6:30 pm, Monday-Friday (may adjust to meet specific site hours)
Reports to: 21st Century Division Executive
Job Description:
Responsible for coordination and execution of afterschool programming within the partner school
Program is located at an independent school-based site; on days the school is off, work hours will be at assigned partner Horizon Center or the Administration building
Must maintain collaborative relationship with site school principal and personnel, regular meetings required
Identify and organize program spaces with school personnel; work with the school-based staff to ensure spaces are kept clean and organized for both school staff and the afterschool program
Provide academic coaching with identified students during school day
Data collection and good record keeping for CACFP or Cleveland Food Bank and 21st Century programs
Recruitment, hiring, supervision and development of all staff, including onboarding, orientation and payroll, staff schedules, and maintaining staff longevity
Responsible for site's monthly budget and ordering necessary supplies within Horizon's parameters
Plan and implement process for academic success of afterschool students, working with families and school day staff
Meet grant goals for academic success, youth development and parent engagement
Support and help Lead Teachers in creating and implementing lesson plans and activities
Complete a CAPA portfolio for 21st CCLC grant program
Coordinate snack program for participants
Meet program goals identified by the school
Develop working relationships with partner Horizon Center and Team 21
Identify, contract and work with community partnerships to enrich and strengthen program
Attend monthly Team 21 meetings at Horizon admin building, monthly SAC meetings at North Olmsted Library, and monthly Summer Planning meetings at Horizon admin building
Occasional travel required for professional development, marketing, and monthly meetings
Maintain professionalism and confidentiality with personnel and family information
All other duties as needed/assigned
Qualifications
Qualifications:
An associate's degree or higher is required in education or a related field; experience with 21st Century and/or education is preferred
Must be able to adapt quickly to changing environments and assignments as required
Has the ability to build positive relationships with coworkers, families, vendors, and the community through prompt and accurate response and service, cheerful and professional attitude, and problem-solving spirit
Must have strong interpersonal and customer service skills; and be able to expertly deal with internal and external individuals in a positive manner

School-Based TBS Specialist

Summary/Objective:
Primary job responsibilities involve service linkage and care coordination to assist individuals in addressing their behavioral health needs and tracking relevant health outcomes. School-based Therapeutic Behavioral Support (TBS) Specialist is primarily school and community-based, providing support, case management, and service linkage within Boundless and outside providers that identify and address behavioral health needs, developmental issues, and barriers to care. School-Based TBS Specialists will work with individuals, parents/guardians, Boundless staff, and outside providers that support treatment and contribute meaningfully whole health outcomes of the individual served.
Essential Functions:

• Provides case management support to individuals engaged in behavioral health services, including community psychiatric support and therapeutic support, to promote the coordination and implementation of treatment goals.
• Serve as a liaison between families and agencies to positively promote collaborative processes in client treatment planning by encouraging parent/guardian participation in treatment.
• Identify and assess psychosocial and life skills needs through analysis of factors impinging on client success , including home, school, and community factors.
• Provide parent/guardian educational workshops on identified issues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication.
• Manage phone calls from families, guardians, agencies, etc. Set up meetings, obtain information, etc.
• Develop programs to address parent/guardian participation in agency services and client engagement in the treatment process.
• Attend meetings, training, and professional development activities as required.
• Completes mandatory documentation within timelines established by applicable regulations and policies.
• Mediates issues that arise among clients, families, and staff.
• Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational, transportation, healthy/safety, employment, etc.).
• Provides clients and families with crisis management services as needed.
• Work closely with an interdisciplinary team to support clients and coordinate care.
• Develops a comprehensive discharge plan in collaboration with clients, client's families, multidisciplinary staff, and referral agencies.
• Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.
• Advocates on a case, policy, and program level.
• Meet productivity requirements as established by management.

• Two years of experience in a case management or care coordinator role.
• Experience working in a school-based setting.
Additional Eligibility Qualifications:
• Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record.
• Valid car insurance.
• Ability to obtain and maintain First Aid, CPR - adult and child. (Training will be provided if not already certified).
Supervisory Responsibility:
No supervisory responsibilities are required for this role.
Work Environment:
This job regularly works in the community providing services outside of the office, in schools, client homes, and other community settings. This role routinely uses standard office equipment such as computers, phones and photocopiers.

Position Type/Expected Hours of Work:

Occasional evening and weekend work may be required as job duties demand.
Travel:
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion. The position may require transporting clients as appropriate to support their treatment needs.
