    The Registered Nurse provides professional nursing care to patients, families and caregivers in the Medical Group. Core Job Responsibilities (25%) Delivery of Nursing Care - The registered nurse manages the nursing care process and delivers nursing care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to: Identify patient care requirements establishing personal rapport with patients, families and care givers and understanding of plan of care throughout the patient's experience; Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, families and care givers; Promote patient independence by establishing patient care goals; teaching patient, family and caregiver to understand disease process, health promotion and preventative care, medication management, and self-care skills, and provide time to answer questions and evaluate understanding; In collaboration with care team members and provider supports and adjusts plan of care as appropriate; Assure quality care by adhering to standards measuring health outcomes against patient care goals and standards; Follow the MVHS and Medical Group patient centered care model and standards of care set by the State Board of Nursing, state nursing practice act, and other governing agency regulations. (15%) Safety and Environment of Care - The registered nurse manages the nursing care safety and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to: Protect patients, families , employees by adhering to infection control policies and protocols, medication administration and controlled substance regulations; Utilize standard precautions and practice at all times; Maintain equipment, supplies, and inventory as appropriate; Maintain a safe and clean work environment by complying with procedures rules, and regulations; utilize safety devices at all times. (15%) Patient Care Administration - The registered nurse manages the nursing care safety and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to: Document patient care service by charting in patients' medical records accurately and timely in accordance with licensure, regulatory standards; Maintain continuity of care with the nursing team by communicating plan of care and follow through with multidisciplinary team ensuring that patient administration of care for all services provided are organized, planned and delivered; Maintain patient confidence and protect operations by keeping information confidential and maintaining a private environment; Maintain and anticipates needed supplies, equipment and medications verifying receipt utilizing MVHS ordering procedure. (10%) Continuing Education - The registered nurse manages the nursing care safety and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to: Maintain professional and technical knowledge by attending educational workshops, mandatory in-services; reviewing professional publications and participating in department committee meetings or work groups. As a member of our caring, compassionate health care team the RN MDS Specialist/Coordinator will enjoy: * Tuition assistance * Hourly position - eligible for great shift differentials * Multiple units providing long term care, memory care and/or med surg rehabilitation care * Non-direct care role, but still get to be around residents * Great co-workers * Supportive work environment and thorough orientation * Friendly and welcoming work atmosphere * Easy commute from Troy, East Greenbush, Colonie and Latham * Shadow experiences for those interested in getting additional insight on this rewarding opportunity *The RN MDS Coordinator will be responsible for, but not limited to:* * Oversee all aspects of Medicare requirements and MFD coordination * Responsible for ensuring that the MDS is submitted timely and accurately reflects the services provided * Work with others to coordinate communication among all clinical staff * Coordinates Medicare PDPM, PPS and OBRA care plan schedules for the facility * Work collaboratively with facility MDS RN Specialist and SPHP MDS Team * Responsible for ensuring completion of required documentation for Medicare residents (admission through discharge) * Monitor MDS data and other documentation for accuracy * Participate in determining Medicare Part A eligibility and other MDS considerations * Act as a MDS/PPS resource to the interdisciplinary team * Performs electronic submission of the facility-wide MDS information timely to the state of New York * Actively participates in resident care planning providing CAAs reports to the nurse managers in database, coordinating with medical records for updating face sheets * Enters and maintains residents' ICD-10 code *Minimum Requirements:* * Valid NYS RN license * Associate's degree in nursing * Current CPR and BLS or achieve with onsite training * MDS Certification or must achieve within designated time period with the site's assistance in obtaining *Preferred Requirements:* * Bachelor's degree in nursing * RAC-CT Certification preferred * 2+ years of experience as a RN in a skilled nursing facility or 5 + years in a direct care role (LPN, C.N.A) * 3+ years of experience as a MDS coordinator Pay Range:$34.50 - $48.45 Pay is based on experience, skills, and education.
