We are seeking a Surgical Technologist in our Duke GI-Endoscopy at Brier Creek clinic located in Raleigh, NC to be an integral member of the healthcare team to provide direct and indirect care to patients.
JOB DUTIES AND RESPONSIBILITIES:
Has basic knowledge of instrumentation. Prepare and set up medical supplies and equipment for diagnostic and therapeutic endoscopic procedures.
Demonstrates correct and safe instrumentation passing and assists provider in a safe and accurate manner
Maintains technical skills for diagnostic and therapeutic procedures to include obtaining specimens, attaching vital sign equipment, and positioning the patient.
Assist physician and nursing personnel during diagnostic and therapeutic endoscopic procedures to include, handling instruments and equipment, calling out equipment readings, and collecting and labeling specimens while maintaining infection control standards.
Manually clean endoscopes and then perform the proper process to complete High Level Disinfection of these scopes.
Assist with the teaching of newly hired technologists and student technologists.
Assist nurse with transporting the patient into the procedure room and to recovery after the procedure.
Anticipate the needs of the Endoscopist and the procedural nurse.
Clean and dress stretchers in preparation for next patient.
Clean procedure rooms, including procedural tables, countertops, floor mats and wall suction systems.

Graduate of an accredited Surgical Technology program or maintains an accredited Surgical Technologist certification (NCCT or NBSTSA). Or, provides evidence of the successful completion of an appropriate training program for Surgical Technology in the United States Military.

General Description of the Job Class
Surgical Technologists (ST) are allied health professionals who assist in the preparation and care of surgical patients ranging from infancy to geriatrics under the general direction of a professional nurse. The ST possesses expertise in the theory and application of sterile aseptic technique and combines the knowledge of human anatomy, surgical procedures, and implementation tools and technologies to facilitate a surgeon's performance of invasive therapeutic and diagnostic procedures.
The ST who has a dual function in the operating room and the ambulatory setting demonstrates competency in the care of surgical patients preoperatively and postoperatively in the ambulatory surgical clinic setting.
Duties and Responsibilities of this Level
Level I - New graduate with up to 18 months of full-time equivalent experience.
Perform all responsibilities in a manner that demonstrates Service Excellence towards patients, physicians, peers and other departments. Assume responsibility for upholding the policies and procedures of the department and organization.
Demonstrates eagerness to learn, takes direction, adapts to changes within unit, demonstrates consistent progress toward Level II.
Establishes and maintains sterile field and assures team maintains sterile environment. Demonstrate and practice a good understanding of sterile technique.
Has a basic knowledge of instrumentations. Prepare Operating Room with appropriate instrumentation, equipment and supplies.
Demonstrates correct and safe instrumentation passing and performs sponge, needle and instrument counts in a safe and accurate manner.
Maintain technical skills to serve a patient population comprised of infancy to geriatric clients.
Take calls and rotate shifts as required under the direction of a registered nurse.
For Surgical Technologists with a dual role in an ambulatory environment, additional responsibilities may include: prepare patients for examinations and procedures in the surgical clinic setting; assist the physician during patient examination and procedures in the clinic; collect and label laboratory specimens for processing; reinforce postoperative instructions under direction of physician or RN; assist with incoming and outgoing patient calls under supervision of physician or RN; remove sutures/ staples, change sterile and non-sterile dressings, following DUHS policies and procedures, under direction of RN or physician; perform other clinical functions as delegated and supervised by the RN or physician; schedule and post surgical cases; coordinate complex scheduling of cases which require additional surgical support; update and file patient medical records.
Must advance to level II within 18 months.
Perform other duties and responsibilities as required.
Level II - Perform all the duties and responsibilities of Level I, plus the following:
Assist with the teaching of newly hired technologists and student technologists.
Serve as a role model to new employees and other staff members.
Establish and maintain sterile field and assure team maintains sterile environment independently.

Apply basic knowledge of instrumentations to several cases.
Demonstrate appropriate and correct case preparation.
Competent at unit level.
Corrects deficiencies at the case level.
Communicate effectively with patients, staff members and physicians.
Adherence to standards including but not limited to unit and hospital policies and procedures, regulatory standards, and patient safety goals.
Assume responsibility for self and ensures that the policies, procedures and safety practices of the Operating Room and Hospital are reinforced.

**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.

+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.

What We're Looking For Education: Graduate Degree, required.
Master's degree in NP program or post Master's NP program.
As of January 1, 2005, all new NP graduates must have a Master's Degree.
National certification required after January 1, 2000.
Continuing education is mandatory for recertification and for continuing regulatory recognition to practice in the State.
Graduate Degree, required.
Graduate of an NCCPA accredited physician assistant program, required.
Experience: One to three years of experience working in a physician practice setting, preferred.
Licensure/Certification: Current NP or PA license in appropriate state, registry w/state medical bd, current DEA registry, required.
