Position Summary:
Full-time position available for our Everson Medical Clinic. Applicant will be responsible for the coordination of care for patients with chronic conditions and behavioral health needs at point of care. As a member of the Clinical Care Team, the Care Coordinator will participate in daily huddles, identify the patient's needs according to protocols, and provide point of care services. The Care Coordinator is responsible for enhancing quality and patient-centered care at Sea Mar Community Health Centers.
Duties and Responsibilities:
Participate in morning huddles to anticipate the patient's clinical, social and behavioral health needs.
Work with the care team to identify gaps in care and work to resolve them using process improvement strategies.
Provide brief interventions at point of care to assist patients with management of their chronic illness, address any social needs and link patients to behavioral health.
Advocate for patient services with community, social service, and medical providers.
Participate and coordinate care transitions for patients who have been seen in an emergency room and/or have been discharged from a hospital/long-term care facility.
Track patient's adherence with plan of care in electronic or paper charts and communicate outcomes and recommendations to the primary care provider.
Function as a point person within the clinic care team regarding chronic disease management and improvement activities to improve clinical quality measures.
Organize monthly Health Home meetings by working with the Clinic Operations Team/Clinic Manager, create the agenda and help facilitate the meeting.
Collaborate with clinical care team to improve Patient-Centered Medical Home processes and provide documentation demonstrating performance.
Review the medical record for quality and utilization indicators according to the Quality Improvement Plan.
Generate reports for care teams to identify areas of improvement and monitor sustainability of each quality measure.
Other duties assigned as needed.

Education, Certificates, Licenses, and Registrations
LPN with experience in ambulatory care and/or BA/BS/BSW in health-related field with one year of experience working in community health, or, 4 years of equivalent experience. The LPN does not have to have an active license; this is a non-licensed position.
This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
NCQA (National Committee for Quality Assurance) Certification is a plus.
Valid WA State Driver's License and proof of liability insurance.

ESSENTIAL DUTIES and RESPONSIBILITIES
Performing this job includes the following, and other duties may be assigned.
Supervises front desk, switchboard, and medical records and managed care staff. (Recruitment, orientation and training, provide point of contact to front desk and dental front desk staff, resource allocation, task definition, performance standards, performance feedback, cross training, performance evaluation, corrective action, shift scheduling, coordinate training, changes in policy that affect their department, and interface between management.)
Maintains high degree of confidentiality and professionalism.
Oversee access, customer service; check in procedures, patient confidentiality, registration, and patient financial screening procedures. Ensures that all policies and procedures are being followed and are efficiently implemented.
Works closely with the Nursing and Lab departments to ensure the delivery of efficient and effective customer service to staff and clients.
Will be responsible for delegating tasks that will ensure necessary completion of daily assignments. (Providing interoffice phone directory, maintaining accurate changes in providers schedules, keeping postage machine updated, ordering and receiving supplies, providing support for office equipment and operations, overseeing collections and deposits, ensuring all encounters are reconciled and correctly processed and mail distribution).
Assists in the preparation and implementation of departmental policies and procedures.
Will work closely with supervisor/managers/directors to keep staff informed on any changes in policies.
Works closely with clinic management on problems and issues related to patient flow and operations.
Responds to patient complaints and concerns in coordination with clinic management when necessary.
Will be diplomatic and offer solutions that will benefit not only the staff but the organization as well.
Will offer leadership to prioritize and organize the departments in the most efficient manner.
Assists with compiling data and preparing reports relative to various phases of health center activities.
Assists with implementing and conducting audits relative to quality improvement activities.
Develops and maintains collaborative working relationships with Sea Mar employees and the community.
Demonstrates knowledge and skills necessary to provide services appropriate to the age of patients served and identifies age-specific needs.

What You'll Do:

Additionally, residents will have opportunities to engage in research and gain teaching experience through our partnership with the WSU College of Pharmacy and Pharmaceutical Sciences.

What You'll Do:
Provides care during patient visits, discussing chief complaint and obtaining vital signs and complete health history for the patient. Develops a baseline physical assessment.

Position Summary:
Full-time Integration Specialist position available for the Care Management department in Anacortes, WA. The Integration Specialist provides Health Home services and similar support for patients whose complex medical, behavioral health and social concerns impede their ability for self-care. Must have knowledge of Community Outreach or Case Management. Knowledge of Community Resources preferred. Willingness to commute to meet with clients at various settings including clients home, clinic and/or hospital.
The Integration Specialist is a member of the patient-centered inter-disciplinary Care Management team, and has a strong understanding of chronic conditions and how each condition can compound another, leading to poor health outcomes.
The Integration Specialist meets with patients in the location of their choice; their homes, in the community, at in-patient settings or in clinics. This individual's work will include timely and effective screenings and appropriate referrals to internal Sea Mar service providers, community-based resources, and emergency services when indicated.
Screenings may pertain to functional abilities, daily medical self-management skills, fall risk, depression, anxiety, drug and alcohol use, and other screenings when indicated. Through the use of motivational interviewing and other techniques, the Integration Specialist will work with the patient to create a Health Action Plan which includes long and short term goals with actionable steps that will help the client self-manage their chronic health conditions.
As part of ongoing services, the Integration Specialist will follow up with the patient regularly to evaluate progress made towards completing their Health Action Plan goals. As part of the Care Management/ Health Home six core services, the Integration Specialist provides care transition assistance from in-patient settings, follow-up in the home, as well as community based care coordination, health promotion, patient and family support, referral to community and social support services, and comprehensive care management. As part of the clients' interdisciplinary team, the Integration Specialist will provide information and recommendations regarding the client's care. Must have experience working in the community with underserved populations required.

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Education and/or Experience:
BSW or BA/BS in Human Services, Health Sciences or related field with experience either in social service case management, or care coordination. Will consider other bachelor's level applicants who have similar experience, but only in the circumstance when doing so is approved by contractors and/or state certification entities (ex: HCA approval of WA Health Home Program care coordinator status).
When contractually necessary for provision of work for which individual is hired, this person will complete WA State Health Homes Care Coordinator certification training within sixty days of hire.
Will acquire and maintain state Health Homes Care Coordinator certification.
Must complete agency and State mandatory trainings.
This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
Experience working with substance use disorders, chronic mental illness, and chronic medical conditions, and people in underserved and transient populations.

Licensed Mental Health Therapist: This individual provides diagnostic assessments, referrals for psychiatric services, and case management in coordination with other teams or parties involved in treatment. The Licensed Mental Health Therapist is responsible for providing care to chronically ill patients, patients who are part of the Intensive outpatient program, or patients who are considered high utilizers. The Licensed Mental Health Therapist is responsible for working with CDMHP's, inpatient facilities, including Western State Hospital, and ensuring proper protocols are followed for hospitalization authorization and during discharge planning. In addition, the Licensed Mental Health Therapist provides individual counseling, crisis stabilization, suicide risk assessment and treatment plans, and leads groups. ESSENTIAL DUTIES and/or RESPONSIBILITIES
The CNM provides primary well woman, family planning, antepartum, intrapartum, post-partum, and
newborn care services to patients of Sea Mar Community Health Center in accordance with
training, experience and licensure, in compliance with the principles of practice of Sea Mar and its
mission.
The CNM will consult with an appropriate physician (family practice, OB/GYN, pediatrics, or other
specialist) as needed when patients care needs arise outside the scope of his or her training.
The CNM will provide appropriate hospital care as required.
The CNM may refer patients to medical specialist or other sources of service when necessary.
The CNM prescribes and/or dispenses required medication to patients according to the individual
prescriptive authority granted by the Washington State Board.
The CNM provides professional supervision to the assigned staff of nurses and other
paraprofessionals.
The CNM participates in preventive health programs such as immunization, disease detection and
disease prevention activities. Including education programs for patients and community groups in the
service area.
The CNM works closely with administrative personnel including the Clinical and Medical Director,
preparing reports, grant proposals and other administrative activities when appropriate.
High risk antepartum and intrapartum conditions would necessitate the CNM to initiate the process
currently defined by the standards of the community.
Qualifications
Knowledge of and interest in the unique cultural aspects of the Hispanic people is needed.
Bilingual ability in English and Spanish is preferred.
Education, Certificates, Licenses, Registrations, and Medical Screening
Graduation from an accredited training program or an acceptable equivalent. Board Certified
with appropriate Nursing Specialty Board.
Washington State license to practice medicine.
One year of experience in a family practice setting is preferred.

Position Summary:
Full-time Child and Family Therapist position available at Sea Mar's Child and Family Clinic Oak Harbor and Anacortes, WA. The Child and Family Therapist provides individual and/or family therapy, case management, crisis stabilization, treatment planning, and may lead groups. Essential Duties and Responsibilities:
ADMINISTRATIVE
* Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws, regulations, and contracts.
* Assists in developing and implementing policies and procedures as directed by the Program Director.
* Educates staff regarding the process for mental health service referrals and regarding "psycho educational topics" (suicide risk assessment, behavior modification, etc.) as directed by the Program Manager.
* Maintains documentation of mental health services, and follow through in patient charts.
CLINICAL DUTIES
* Provide developmentally and culturally appropriate formal and informal diagnostic assessments for each patient during the program. Document outcomes, justify diagnoses, and communicate with the collaborative partners regarding outcomes and recommendations. It is suggested that this individual communicate with the Primary Care Physicians, Substance Abuse Counselors, schools, case workers, or others involved in the care of the patient.
* Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, specifically children and families.
* Provide information about assessments and involve family members as active participants in the assessment process and in treatment planning, where appropriate.
* Collaborate with other professionals in conducting family-centered assessments and treatment planning, when appropriate.
* Demonstrate knowledge of the principles of growth and development over the life span.
* Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients.
* Refer patients to psychiatric and medication management services as appropriate. Gather record materials and background information as needed by the community mental health center (See Policies and Procedures). Actively collaborate with the psychiatric service provider to insure accuracy of presentation and that identified needs are addressed. Provide monitoring regarding the effects of medications, and feedback to the psychiatrist regarding effects/side effects.
* Provide individual and/or family counseling, in coordination with other system as needed.
* Provide suicide risk assessment and develop treatment plans to address suicidal ideation or gestures. Use case management as needed to provide follow through and build community support as follow through. Provide other crisis stabilization as needed.
* Gather information regarding past mental health services to be used in the individual's treatment plan development. Provide mental health services referrals to patients prior to or following admission, and exchange information with outside service providers.
* Support other staff regarding mental health needs of staff or patients. Maintain confidentiality, prevent splitting between staff and patients, and offer concrete ideas on handling challenging situations.
PRODUCTIVITY STANDARD
Produce a minimum of 80 encounters that will equal at least 80 hours of face-to-face contact.

The Child and Family Therapist provides individual and/or family therapy, case management, crisis stabilization, treatment planning, and may lead groups. In addition, will provide referrals and coordination with medication providers, primary physicians and other community partners. Must have the ability to accurately diagnose mental health issues and develop treatment plans based upon diagnosis.
Essential Duties and Responsibilities:
ADMINISTRATIVE
Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws, regulations, and contracts.
Assists in developing and implementing policies and procedures as directed by the Program Director.
Educates staff regarding the process for mental health service referrals and regarding "psycho educational topics” (suicide risk assessment, behavior modification, etc.) as directed by the Program Manager.
Maintains documentation of mental health services, and follow through in patient charts.
CLINICAL DUTIES
Provide developmentally and culturally appropriate formal and informal diagnostic assessments for each patient during the program. Document outcomes, justify diagnoses, and communicate with the collaborative partners regarding outcomes and recommendations. It is suggested that this individual communicate with the Primary Care Physicians, Substance Abuse Counselors, schools, case workers, or others involved in the care of the patient.
Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, specifically children and families.
Provide information about assessments and involve family members as active participants in the assessment process and in treatment planning, where appropriate.
Collaborate with other professionals in conducting family-centered assessments and treatment planning, when appropriate.
Demonstrate knowledge of the principles of growth and development over the life span.
Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients.
Refer patients to psychiatric and medication management services as appropriate. Gather record materials and background information as needed by the community mental health center (See Policies and Procedures). Actively collaborate with the psychiatric service provider to insure accuracy of presentation and that identified needs are addressed. Provide monitoring regarding the effects of medications, and feedback to the psychiatrist regarding effects/side effects.
Provide individual and/or family counseling, in coordination with other system as needed.
Provide suicide risk assessment and develop treatment plans to address suicidal ideation or gestures. Use case management as needed to provide follow through and build community support as follow through. Provide other crisis stabilization as needed.
Gather information regarding past mental health services to be used in the individual's treatment plan development. Provide mental health services referrals to patients prior to or following admission, and exchange information with outside service providers.
Support other staff regarding mental health needs of staff or patients. Maintain confidentiality, prevent splitting between staff and patients, and offer concrete ideas on handling challenging situations.
PRODUCTIVITY STANDARD
Produce a minimum of 80 encounters that will equal at least 80 hours of face-to-face contact.
Qualifications:

An understanding of the Behavioral Health Organization, County, and/or MCO contracts with the ability to tier clients in a managed care system. The ability to work effectively with all persons and groups with an open mind towards cultural differences and knowledge of cultures.
The ability to accurately diagnose mental health issues and develop treatment plans based upon diagnoses. The ability to NOT diagnose behavioral problems as mental illnesses unless appropriate.
An understanding of chemical dependency treatment and an ability to coordinate mental health services with chemical dependency treatment providers if necessary.
Must complete an annual clinical competency test as a mental health therapist.
The Child and Family Clinical Therapist must sign a permanent oath of confidentiality covering all patient related information.
This person must pass a background check.
Education and Experience:
A Master's degree from an accredited college or university in psychology, counseling, or social work, which includes course-work in psychological diagnostics.
For a licensed therapist, must have a license in Marriage and Family Therapy (LMFT), Mental Health Counseling (LMHC), or Social Work (LICSW or LASW).
If not licensed, must hold and maintain an active Washington State Agency Affiliated Counselor License or mental health Associate License (includes Marriage and Family Therapist, Mental Health Counselor, and Clinical Social Worker).
Must possess a Child Mental Health Specialist credential or be working towards credential.
MUST meet WAC requirements for a Mental Health Professional.
Computer proficiency with Cerner and MS Office and other Medical software.
Experience with evidence based models and wrap around services preferred.
Experience working with children and families required.
Bilingual Spanish/English highly desirable.

Position Summary:
Full-time Receptionist position available for our Mount Vernon Medical and Dental Clinic. We are looking for customer service oriented candidates. The receptionist will be responsible for processing patient information. Making medical appointments and contacting patients via phone on behalf of Sea Mar, taking messages, etc. The ideal candidate must possess some problem-solving skills and must be able to communicate efficiently with our patients and be sensitive to their needs. Position Summary
