We are recruiting for the following position: Sea Mar is a mandatory COVID-19 and flu vaccine organization Care Coordinator II - Posting #25570 Hourly Rate: $22.63 Position Summary: Full-time position available for our Everson Medical Clinic. Applicant will be responsible for the coordination of care for patients with chronic conditions and behavioral health needs at point of care. As a member of the Clinical Care Team, the Care Coordinator will participate in daily huddles, identify the patient's needs according to protocols, and provide point of care services. The Care Coordinator is responsible for enhancing quality and patient-centered care at Sea Mar Community Health Centers. Duties and Responsibilities: Participate in morning huddles to anticipate the patient's clinical, social and behavioral health needs. Work with the care team to identify gaps in care and work to resolve them using process improvement strategies. Provide brief interventions at point of care to assist patients with management of their chronic illness, address any social needs and link patients to behavioral health. Advocate for patient services with community, social service, and medical providers. Participate and coordinate care transitions for patients who have been seen in an emergency room and/or have been discharged from a hospital/long-term care facility. Track patient's adherence with plan of care in electronic or paper charts and communicate outcomes and recommendations to the primary care provider. Function as a point person within the clinic care team regarding chronic disease management and improvement activities to improve clinical quality measures. Organize monthly Health Home meetings by working with the Clinic Operations Team/Clinic Manager, create the agenda and help facilitate the meeting. Collaborate with clinical care team to improve Patient-Centered Medical Home processes and provide documentation demonstrating performance. Review the medical record for quality and utilization indicators according to the Quality Improvement Plan. Generate reports for care teams to identify areas of improvement and monitor sustainability of each quality measure. Other duties assigned as needed. Ability to utilize and document relevant patient information the Electronic Health Record. Knowledge of community resources. Ability to work in a fast-paced community health care setting. Ability to think analytically and problem solve in a multidisciplinary team and independently. Ability to deal effectively with difficult people and situations. Ability to communicate effectively with diverse communities. Ability to manage time effectively and prioritize tasks. Ability to analyze patient care data. Ability to identify client learning needs and to assess client's knowledge, skill level and readiness for learning. Ability to maintain the privacy and security of sensitive and confidential information in all formats including verbal, written and electronic; and adhere to policies and procedures related to local, state, and federal privacy requirements. Excellent communication and customer service skills. Critical thinking skills. Ability to understand and implement process improvement activities. Bilingual in Spanish is strongly preferred. Other language skills may be considered depending on site needs due to the population that is being served. Education, Certificates, Licenses, and Registrations LPN with experience in ambulatory care and/or BA/BS/BSW in health-related field with one year of experience working in community health, or, 4 years of equivalent experience. The LPN does not have to have an active license; this is a non-licensed position. This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment. NCQA (National Committee for Quality Assurance) Certification is a plus. Valid WA State Driver's License and proof of liability insurance. The Child and Family Therapist provides individual and/or family therapy, case management, crisis stabilization, treatment planning, and may lead groups. It is suggested that this individual communicate with the Primary Care Physicians, Substance Abuse Counselors, schools, case workers, or others involved in the care of the patient. Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, specifically children and families. Provide information about assessments and involve family members as active participants in the assessment process and in treatment planning, where appropriate. Collaborate with other professionals in conducting family-centered assessments and treatment planning, when appropriate. Demonstrate knowledge of the principles of growth and development over the life span. Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients. Refer patients to psychiatric and medication management services as appropriate. Gather record materials and background information as needed by the community mental health center (See Policies and Procedures). Actively collaborate with the psychiatric service provider to insure accuracy of presentation and that identified needs are addressed. Provide monitoring regarding the effects of medications, and feedback to the psychiatrist regarding effects/side effects. Provide individual and/or family counseling, in coordination with other system as needed. Provide suicide risk assessment and develop treatment plans to address suicidal ideation or gestures. Use case management as needed to provide follow through and build community support as follow through. Provide other crisis stabilization as needed. Gather information regarding past mental health services to be used in the individual's treatment plan development. Provide mental health services referrals to patients prior to or following admission, and exchange information with outside service providers. Support other staff regarding mental health needs of staff or patients. Maintain confidentiality, prevent splitting between staff and patients, and offer concrete ideas on handling challenging situations. PRODUCTIVITY STANDARD Produce a minimum of 80 encounters that will equal at least 80 hours of face-to-face contact. As part of the Care Management/ Health Home six core services, the Integration Specialist provides care transition assistance from in-patient settings, follow-up in the home, as well as community based care coordination, health promotion, patient and family support, referral to community and social support services, and comprehensive care management. As part of the clients' interdisciplinary team, the Integration Specialist will provide information and recommendations regarding the client's care. Must have experience working in the community with underserved populations required. Requirements and/or Responsibilities: Must be able to complete job responsibilities in various locations; client's home setting, community setting, or clinic. Ability to understand medical terminology pertaining to chronic conditions. Ability to work with an interdisciplinary care team including medical providers, nursing staff, care coordinators, behavioral health and support staff. Must be able to perform independently and at the same time perform effectively and professionally as an interdisciplinary team member. Must be able/willing to work with translators if not bilingual. May carry a caseload of 60 patients as assigned by Care Manager. Provides up to two contacts per month for high-intensity patients (one face-to-face contact and one telephone contact with patient, providers, or caregivers) with a step down to telephone contact when the patient has demonstrated stability. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Education and/or Experience: BSW or BA/BS in Human Services, Health Sciences or related field with experience either in social service case management, or care coordination. Will consider other bachelor's level applicants who have similar experience, but only in the circumstance when doing so is approved by contractors and/or state certification entities (ex: HCA approval of WA Health Home Program care coordinator status). When contractually necessary for provision of work for which individual is hired, this person will complete WA State Health Homes Care Coordinator certification training within sixty days of hire. Will acquire and maintain state Health Homes Care Coordinator certification. Must complete agency and State mandatory trainings. This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment. Experience working with substance use disorders, chronic mental illness, and chronic medical conditions, and people in underserved and transient populations. Bilingual in English/Spanish preferred. Background in community outreach or case management preferred. Knowledge of Community Resources preferred. Willingness to commute to meet with clients in various settings including in the client's home, clinic, and/or hospital.
