Maternity Support Services (MSS) community health nursing reflects a value for self-efficacy, where the nurse supports women in accomplishing self-care and decision making for the health and well-being of herself and the infant during pregnancy and the post-pregnancy period. This person is responsible for triaging pre and postnatal needs of each client and scheduling visits, including members of the Sea Mar MSS team and non-Sea Mar providers if needed. Duties and Responsibilities: Assess and observe client and family needs with regards to understanding of pre and postnatal care and educate accordingly. Ongoing demonstrated ability to communicate effectively with people from diverse backgrounds and to recognize cultural, environmental and socio-economic factors that may affect a client's ability to receive, comprehend and retain information is essential to performance Participate in monthly provider meetings. Attend Community First Steps meetings quarterly. Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Perform independently, effectively, and professionally as an interdisciplinary team member. Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients. Demonstrate knowledge of the principles of growth and development over the life span. Must be comfortable yet professional in the client's home setting with ability to set and maintain personal boundaries. Must document all nursing visits in the client MSS E H R. Qualifications: BSN/RN with recent maternal/infant, community, or public health experience. Experience in patient counseling and education, lactation consultation experience and training and/or certified Childbirth Educator is preferred. Effective oral/written communication skills, assessment and intervention skills, ability to recognize and support health behavior change. Must obtain Basic Life Support (BLS) CPR within 90 days of hire date and maintain current BLS CPR throughout employment. Must have access to an insured vehicle, and be able to make home visits. Teaching may include but may not be limited to the following: fetal development, normal changes in pregnancy, warning signs, importance of and expected course of medical care, sexuality/reproductive, nutrition, elimination, activity/exercise, sleep/rest, self-perception, role/relationship, coping/stress, values /beliefs, family planning, breastfeeding, and childbirth preparation. Contact is initiated prenatal and a thorough nursing assessment is completed including referrals as needed to nutrition, social work, mental health, community service agencies, and/or medical providers. Assessments, health messages, and education continue throughout the pregnancy with a minimal goal of one face to face visit each trimester and postpartum. The MSS RN performs early postpartum home visits or office visits.
