Position Summary:
Seeking a full time RegisteredNurse for our Maternity Support Services (MSS) Program in University Place, WA. Maternity Support Services (MSS) community health nursing reflects a value for self-efficacy, where the nurse supports women in accomplishing self-care and decision making for the health and well-being of herself and the infant during pregnancy and the post-pregnancy period. This person is responsible for triaging pre and postnatal needs of each client and scheduling visits, including members of the Sea Mar MSS team and non-Sea Mar providers if needed.
Duties and Responsibilities:
Assess and observe client and family needs with regards to understanding of pre and postnatal care and educate accordingly.
Ongoing demonstrated ability to communicate effectively with people from diverse backgrounds
and to recognize cultural, environmental and socio-economic factors that may affect a client's
ability to receive, comprehend and retain information is essential to performance
Participate in monthly provider meetings.
Attend Community First Steps meetings quarterly.
Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
Perform independently, effectively, and professionally as an interdisciplinary team member.
Able to identify and categorize each patient's age-specific grouping of needs, such as those for
infant, adolescent, or geriatric patients.
Demonstrate knowledge of the principles of growth and development over the life span.
Must be comfortable yet professional in the client's home setting with ability to set and maintain personal boundaries.
Must document all nursing visits in the client MSS E H R.
Qualifications:
BSN/RN with recent maternal/infant, community, or public health experience.
Experience in patient counseling and education, lactation consultation experience and training and/or certified Childbirth Educator is preferred.
Effective oral/written communication skills, assessment and intervention skills, ability to recognize and support health behavior change.
Must obtain Basic Life Support (BLS) CPR within 90 days of hire date and maintain current BLS CPR throughout employment.
Must have access to an insured vehicle, and be able to make home visits.
Bilingual in Spanish required.

Position Summary
This position is responsible for coordinating pre and postnatal risk assessment, teaching, and appropriate referral, for Sea Mar OB clients and eligible First Steps clients who receive OB care outside of Sea Mar. Teaching may include the following: fetal development, normal changes in pregnancy, warning signs, importance of and expected course of medical care, sexuality/reproductive, nutrition, elimination, activity/exercise, sleep/rest, self-perception, role/relationship, coping/stress, values /beliefs, family planning, breastfeeding, and childbirth preparation. Contact is initiated prenatal and a thorough nursing assessment is completed including referrals as needed to nutrition, social work, mental health, community service agencies, and/or medical providers. Assessments, health messages, and education continue throughout the pregnancy with a minimal goal of one face to face visit each trimester and postpartum. The MSS RN performs early postpartum home visits or office visits. This person is responsible for triaging pre and postnatal needs of each client and scheduling visits and including all members of the Sea Mar MSS team and may include non-Sea Mar providers as needed. Active participation in community-wide efforts/coalitions to provide ever-improving comprehensive interdisciplinary care to pre and postnatal clients is expected. The productivity expectation for this position is 18 to 20 encounters per week for full time.

The person in this position shall have no history or evidence of alcohol or other drug misuse for a period of three (3) years prior to the date of employment at the facility, and no misuse of alcohol or other drugs while employed at this facility. This individual cannot be a person who has been convicted of a felony within the last seven years or ever been convicted of child abuse and / or any crime involving physical harm to another person nor be a perpetrator of substantiated child abuse. Bilingual (Spanish/English), bicultural preferred. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. EDUCATION and EXPERIENCE

Prior experience and training in one or more areas of health education. The incumbent must have experience in patient counseling and education, and recent perinatal nursing. Washington state RegisteredNurse license by D O H under Chapter 246-840WAC. This position must obtain CPR within 90 days of hire date and is required to maintain current CPR throughout employment. This person must pass a Washington State Patrol background check.

JOB SUMMARY: The Nurse Practitioner's primary job function is to be responsible for provision of comprehensive primary care services for the patients of Family Health Centers. Works independently and interdependently with other clinic providers. Job performance is evaluated through direct observation, consultation and written documents. .FTE: 1.0 Bilingual English/Spanish preferred.
Access & Quality
1. Provide comprehensive primary care services in a primary medical care ambulatory clinic setting, working independently and interdependently with other FHC providers.
2. Provide consultation to other clinic providers either in person or by telephone.
3. Consult with clinic physicians, specialists, or other health care professionals as appropriate.
4. Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
5. Participate in a shared call system with other providers.
6. Assist with meeting clinic patient care team productivity and quality care standards.
7. Participate in development and utilization of FHC practice guidelines and procedures.

ESSENTIAL DUTIES and RESPONSIBILITIES include the following. Other duties may be assigned.
The MSS RN assesses and observes client and family needs with regards to understanding of pre and postnatal care and educates accordingly.
Consultation and coordination of client care is done on a regular and as needed basis with other professionals involved utilizing a team approach to comprehensive services.
Ongoing demonstrated ability to communicate effectively with people from diverse backgrounds and to recognize cultural, environmental and socio-economic factors that may affect a client's ability to receive, comprehend and retain information is essential to performance.
The MSS RN must be comfortable yet professional in the client's home setting with ability to set and maintain personal boundaries.
Must be able to perform independently and at the same time perform effectively and professionally as an interdisciplinary team member.
Must at all times preserve client confidentiality.
Participation in monthly providers meetings.
Attend Community First Steps meetings quarterly.
Be available via pager for urgent needs.
Have a valid Washington State Driver's License and own or have access to an insured vehicle.
Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
Demonstrate knowledge of the principles of growth and development over the life span.
Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients.
Must document all nursing visits in the client MSS E H R.
Must be able/willing to work with translators if not bilingual (Spanish).

Bilingual (Spanish/English), bicultural preferred. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before clients or employees of the organization.
Education, Certificates, Licenses, Registrations, and Medical Screening
EDUCATION and/or EXPERIENCE
BSN/RN with recent maternal/infant, community, or public health experience. Preferred experience in patient counseling and education, lactation consultation experience and training and/or certified Childbirth Educator.
Prior experience and training in one or more areas of health education.
The incumbent must have experience in patient counseling and education, and recent perinatal nursing.
CERTIFICATES, LICENSES, REGISTRATIONS
This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.

Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all time during the flu season.
Work Environment
This job requires to work off site of the office doing Home visits, this position share office with the MSS team, be flexible with schedule as some of our clients can only been seeing evening and or weekend.
The noise level in the work environment is usually moderate.

PRIMARY ROLE OBJECTIVE:
Works with Clinic Supervisors in conjunction with Operations, Medical Providers and the Quality Improvement Team to ensure quality, comprehensive and coordinated care is delivered to patients in a PCMH/Team Based Care environment.
MAIN DUTIES & RESPONSIBILITIES:
* Assists care teams with identification of complex chronic diseases and conditions to ensure the patient population receives appropriate care.
* Coordinates care with the medical, behavioral health and dental teams and patients to assess needs, identify barriers and coordinate plans of care and communication among multiple providers.
* Coordinates transitions of care with patients, PCP and other service providers.
* Relays appropriate information between patient and care team by documenting in EHR.
* Monitors and processes voicemail, EHR messaging and email in a timely manner.
* Works to the level of their license to maximize their contribution to Team Based Care
* Acts as a resource to MAs to help them work to the level of their license
* Acts as a leader in integrated teams to establish and maintain a positive work environment and strengthen relationships between teams and departments.
* Functions as an RN Case Manager in direct patient care when needed for complex patients
* Assists with Case Management in a Team Based Care environment as needed under the direction of the PCP
* Supports and contributes to Quality Initiatives that advance the goals of Family Health Center.
* Follows the direction of the Infection Control Manager to uphold the highest standards of patient and staff safety.
* Participates in PDSA cycles, capturing pertinent information to maximize the effectiveness of Process Improvement
* Provides professional nursing care for FHC patients following established standards and practices and acting within the scope of the RN license.
* Interacts frequently with high risk patients regarding care plans, by phone and in the clinical setting.
* Acts as an educational resource for patients and staff.
* Develops and discusses patient self-management plans in conjunction with PCP.
* Follows up with patients to assess compliance and progress with identified goals.
* Provides decision support and encourages treatment adherence
* Assists in PCMH strategies to identify high risk patients based on diagnosis and other factors; use reports to access data on high risk patients; work with Clinic Supervisor & Operations Manager to help develop and implement processes to ensure high risk patients received appropriate care at FHC. Triages patients to level of employee licensure: assess patients as needed and provide appropriate nursing care.
* Supports empanelment and helps to maintain accurate and appropriate Provider panels.
* Support Population Health to proactively improve the health of our community
* Participates in Team Based Care and Whole Person Care initiatives
* Works with QIP Leadership and the Clinic Nurse Supervisor to maintain compliance
WORK ENVIRONMENT Work is performed primarily in an ambulatory care setting, which may include working at sites other than their home clinic site. We may be required to work outside of the clinic when necessary to meet the needs of our community (such as at community events and at alternative care sites for infection control purposes).
