Position Summary:
Sea Mar Community Health Centers is seeking a full-time Certified Diabetes Care Education Specialist for the Vancouver Medical Clinic. The certified diabetes care and education specialist (CDCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and endocrinology care. The CDCES is an integral part of the care team and provides collaborative, comprehensive and person- centered care, and education conducive to behavior change and improved quality of life across the lifespan. The CDCES supports and advocates for people affected by diabetes to optimize quality care. The CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. Candidate will work closely with Endocrinologist, bariatric surgery patients, and diabetes technology (CGM, insulin pens, etc.).
ADMINISTRATIVE
Maintains complete and up-to-date charts (documenting counseling session as specified by program guidelines in legible form), and statistical program records but not limited to: Misys EMREHR records, WIC/CIMS Flow sheet, WIC diet records, certification sheets weight/height grids, diet evaluation sheets, infant birth outcomes, appointment schedules and encounter forms.
Establishes and maintains professional working relationships with team members and other Sea Mar staff. Serves as a liaison between Nutrition and/or WIC and other clinic and community services and makes referrals as appropriate.
Assists with the clerical and statistical program work as needed.
Attends selected clinic, MSS, PHS Department and WIC meetings and participates in continuing education as needed.
Acts as a preceptor for dietetic interns, as appropriate.
The Registered Dietician shall follow all organizational and departmental policies and procedures.
CLINICAL Medical Nutrition Therapy (MNT)
Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served.
Demonstrates knowledge of the principles of growth and development over the life span.
Able to identify and categorize each patient's age-specific grouping needs, such as those for infant, adolescent, or geriatric patients.
Interviews clients from diverse populations, reviews and documents client history, anthropometric and laboratory data, and reviews current eating habits.
Uses standard assessment tools and follows department policies and practices to perform screening procedures such as weighing and measuring, taking hematocrits, plotting growth grids, and reviewing dietary evaluations.
Maintains patient rights and confidentiality: documents vendor and participant complaints and infractions.
Provides program orientation, nutrition requirements, and assists in scheduling client appointments, considering all programmatic and client needs.
Provides individual and group nutrition education according to program guidelines, utilizing client appropriate methods and materials.
Provides Medical, MSS, and patient nutritional assessment and nutrition counseling. Facilitates appointment time through scheduling and following up on failed appointments according to protocol, coordinates services with other health professionals, and cooperates with other departments. Documents assessment, treatment, and plan of action from counseling session in Medical record according to protocol.
Education and/or Qualification:
Possess a Bachelor's or Master's degree in nutrition from an accredited university.
Completed an accredited dietetic internship program or Coordinated Undergraduate Program in Dietetics.
7 hours of HIV/AIDS course.
WA State Registration and Certification for Dietitian or RD/CD eligible with attainment of these credentials within time period stated in the Sea Mar Nutrition Program Policies and Procedures Manual.
Medical Assistant-Registered Certification required for Hemoglobin testing for WIC RD. WIC staff who do not have the Medical Assistant-Registered Certification are not permitted to perform the Hemoglobin testing. Staff are required to obtain the Certification after hire or promotion.
The experience should reveal demonstrated interest in nutrition services and nutrition education.
Experience in community nutrition setting with an interest in basic nutrition and childhood development is desirable.
Bilingual English/Spanish or English/Russian is preferred but not required.

Job Description

ADMINISTRATIVE DUTIES
Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws,
regulations, and contracts.
Ensure that charts, prescription refill requests, and all information or paperwork the
psychiatric prescriber will need is prepared ahead of time and organized in the fashion
preferred by the prescriber.
Ensure medication storage, maintenance, dispensation and waste logs are maintained in
accordance with Sea Mar policies and procedures. Any area of non-compliance should be
reported immediately to the supervisor/manager, if applicable.
Ensures that all medical, nursing, health & safety and personnel policies and procedures are
being followed and implemented.
CLINICAL DUTIES
Assists psychiatric staff with client flow and provides quality of care in a timely manner.
Completes routine checks and monitors clients to detect unusual or harmful behavior and
reports observations to psychiatric staff.
Prepare the patient for their office visits with the provider including obtaining subjective data
and vital signs, ensuring charting is in accordance with Sea Mar's policy.
Works as part of an integrated care team that may include psychiatrists, social workers,
behavioral health professionals, chemical dependency professionals and case managers.
Provides mentally impaired or emotionally disturbed clients with routine physical, emotional,
psychological, or rehabilitation care under the direction of psychiatric staff.
Encourages client participation in social, educational, recreational and employment activities.
Receives and processes medication refill requests from pharmacies by preparing refill requests
for provider review and approval, and ensures prescriptions are faxed to the pharmacy.
Checks EHR to confirm requests are valid. If valid, the aide will send the prescriber a refill
request through EHR. If invalid, the aide will communicate with the pharmacy.
Checks medication history in EHR as requested by prescriber.
Collaborates with prescriber to determine next steps.
Fields phone calls from clients regarding questions about side effects and/or status of refills.
Facilitates client enrollment in prescription assistance programs.
Demonstrates knowledge and skills necessary to provide care appropriate to the age of the
clients served.
Demonstrates knowledge of the principles of growth and development over the life span.
Able to identify and categorize each client's age-specific grouping of needs, such as those for
infant, adolescent, or geriatric clients.
Provides medication monitoring including effects on the client and provides feedback to the
psychiatrist regarding positive and negative side effects.
Screen for suicide risk and inform psychiatric prescriber and mental health therapist. Use case
management and build community support as needed to provide follow-up care.
Maintain confidentiality; prevent splitting between staff and clients, and offer concrete ideas on
handling challenging situations.
Provide advocacy as needed and encourage cooperation when implementing the Service Plan
or to resolve problems which are interfering with active participation in plan (such as
transportation or language barrier).
Assists client in accomplishing necessary tasks in service accessibility such as filling out
appropriate forms, obtaining necessary documentation or authorization, and finding
transportation.
Makes referrals based on identified needs to appropriate agencies and/or community services.
Provides information and assists clients for self-referral.
Maintains files that document case management activities as required by the State and the
agency.
Participates in regular interdisciplinary team case reviews.
Promotes a positive team approach. Collaborates with all staff members to provide
comprehensive services that are individualized to best serve each client.
Welcomes and orients all new staff and visitors in a positive manner.
Maintains professional standards and follows the treatment policies and procedures of the
agency, as well as applicable federal, state, and local regulations.
Completes daily record keeping as directed by the Program Manager.
Reports any unusual incidents to the Program Manager and agrees to report to the State any
suspected case of child/elder abuse or neglect as mandated by WAC.
Must be able to support a full-time provider (MD/ARNP) to see an average of 14 patients per
day, see attached productivity requirements.
Assist providers with GYN exams, I & D's, and other minor procedures.
Demonstrate clear and concise written and verbal communication. When patients are
determined a priority due to abnormal vital signs, the Psychiatric Aide will contact the
provider immediately.
Utilize aseptic (sterile) technique.
Complete point of care referrals for patients including specialty care, care coordination and
other services. Informing patients regarding specialty referral, faxing information to specialist's
offices for point of care referral processing will be made by the Medical Assistant.
Assist with obtaining laboratory tests and results.
Maintain positive collaborative working relationships with co-workers, specialists and
community organizations.
Ensure customer service is delivered at each patient visit including escorting each patient to
and from the treatment rooms.
Responsible for communicating provider delays and delays in patient flow to the front desk
staffs, as well as the patient.
Demonstrate proficiency in the nursing parameters of the Electronic Health Record and
Practice Management Application programs.
Demonstrates knowledge, proficiency and compliance with regulatory requirements including,
but not limited to: Joint Commission, NPSG, Department of Health, HEDIS, PCMH, QI and
various contractual obligations.
Demonstrates proficiency in cardiopulmonary resuscitation (CPR) and can perform CPR
independently.
Measures and records fluid and food intake and output.
Recognizes, responds to and reports client's or resident's emotional, social, cultural and
mental health needs.
Addresses individual behavioral needs of the client or resident.
Depending on the work environment, the Psychiatric Provider Aide will perform all duties
under WAC 246-841-400.

end of probationary period.

The Crisis Intervention Specialist (CIS) provides behavioral health crisis intervention services to individuals who are referred to Sea Mar's Adult Mobile Crisis Intervention (AMCI) team through the Southwest Washington Crisis line, 911 or CRESA. A Sea Mar Mental Health Professional (MHP) will always be available to the CIS for consultation. The CIS provides outreach, assessment, referral and follow up supportive services for these individuals as the primary responder. The outreach services would always require a second staff for safety. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
ADMINISTRATIVE
Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws, regulations, and contracts.
Assists in developing and implementing policies and procedures as directed by the Program Director.
Educates staff regarding the process for crisis behavioral health service referrals and regarding "psycho educational topics (suicide risk assessment, behavior modification, etc.) as directed by the Program Director.
Maintains documentation of crisis services in EPIC.
CLINICAL DUTIES
Receives, processes and responds in person to mobile crisis referrals.
Documents nature of crisis referrals, outreaches and outcomes.
Demonstrates knowledge and skills necessary to provide adult crisis outreach services.
Demonstrates knowledge of the principles of risk assessment, de-escalation, safety planning and referral to more restrictive alternatives (DCR, Inpatient, 911).
Refers patients to psychiatric and other health care services as appropriate.
Maintain professional standards and follows the treatment policies and procedures of Sea Mar, and of the federal, state, and local regulations. Has a thorough working knowledge of appropriate WAC and RCW's.
Attends AMCI daily crossover meetings and shares relevant information.
Provides suicide risk assessment and develops safety plans to address suicidal ideation or gestures. Provide other crisis stabilization and de-escalation servcies as needed.
Perform opioid overdose risk screening and appropriate follow up to include distribution of narcan.
Gathers information regarding past mental health services to be used in the individual's treatment plan development.
Maintains an active Department of Health Credential.

Position Summary:
Full-time Certified Peer Counselor (CPC) position available for our Behavioral Health Program in Vancouver, WA. The CPC provides behavioral health crisis intervention services to individuals who are referred to Sea Mar's Adult Mobile Crisis Intervention (AMCI) team through the Southwest Washington Crisis line, 911, or CRESA. A Sea Mar Mental Health Professional (MHP) or Crisis Intervention Specialist (CIS) will always be with CPC on outreaches. The CPC provides outreach, referral, and follow up supportive services for these individuals as a responder. The outreach services would always require two staff outreach. The assessment, follow up and referral activities, and collaboration will occur with natural supports, community partners, service providers, and other community organizations.

Responsibilities and Duties: Assists therapist/case managers in the following areas:
Provides case management services to assigned consumers. These services may include collaborating in the provision or actually providing assistance to the consumer and their family or significant others to obtain and/or maintain housing, income, employment, medical and mental health treatment, and meaningful social and recreational activities in the community. It is expected that the provision of these services will constitute at least 50% of the CPC's work.
Assists the recovering person to identify their personal interests and goals in relation to recovery and integrating into the community.
Educates clinical staff and others about the recovery process and the damaging role that stigma can play in undermining recovery.
Assists recovering persons' abilities, strengths and assets to assist them to recognize these strengths and use them to achieve their goals.
Has an understanding of community resources such as vocational, social, cultural, spiritual, as well as funding and how to help consumer's access services as appropriate.
Assures appropriate documentation of the above activities.
Assists with case management for clients from the Substance Abuse and Mental Health programs; assists with obtaining records and maintaining records as needed; assists with administrative work as assigned by Program Manager.
Participates in clinical consults with either Program Manager or assigned Therapist as well as seeks additional consultations as needed.
Attends and participates in meetings as directed.
Helps to create an environment in which good mental health and a climate of teamwork is fostered.
As needed, provides on-site services to acute-care consumers, including groups, one-on-one support, and coordination with other clinic services and staff, including medical prescribers and crisis staff.

Collaboration
Collaborates with the primary Clinician and fosters the ability of the program to assist the client in obtaining appropriate community resources, natural supports, and entitlements as identified through a comprehensive assessment and treatment plan, by providing case management to facilitate the arranging of these services and to assist the client in learning the skills necessary to obtain and keep these supports and benefits.
Assists the Primary Clinician for assigned consumers in the accomplishment of the following duties:
Development with the consumer of an outcome based treatment plan;
Development of an individually tailored crisis plan if needed;
Provision of needed treatment services to assigned clients,
Assistance to client in management of the authorized treatment benefit (Medicaid or other insurance) to maximize clinical effectiveness and benefit;

Job Summary

Responsibilities and Duties

Qualifications and Skills
Licensed MD or DO
Successful completion of an Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA) approved post-graduate residency; board certification in the appropriate specialty or actively seeking board certification.

ADMINISTRATIVE

CLINICAL

CONDITIONS OF EMPLOYMENT

Must have and maintain CPR certification bi-annually and TB test annually.

Position Summary:
Full-time Social Worker II position available for our Maternity Support Services program in Washougal, WA. This is a great opportunity to work with expectant moms and babies in a community health setting. The Social Worker is to provide brief, culturally relevant counseling interventions, education, case management and crisis intervention to clients based on areas identified in screening/assessment and in keeping with the core services of the First Steps program. Duties and Responsibilities:
Introduces clients to integrated MSS services (either in home or at clinic) and performs

Complete thorough, culturally sensitive, psychosocial assessments which include: Personal history (cultural background, health status, coping strategies, mental health status, substance use, abuse and violence); current family functioning; review of support systems (formal and informal); and basic physical needs. This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning.
Other duties will be assigned as necessary.
Qualifications:
Master's degree from an accredited university and licensure in the state of Washington as a
Licensed Mental Health Counselor, Licensed Independent Clinical Social Worker, Licensed
Social Worker, Licensed Marriage and Family Therapist, or Licensed Psychologist.
Experience working with underserved populations such as, but not limited to, those who have

Must be willing to work later hours (after 5 pm) on some days, and will require work within schools or other non-traditional settings.
Essential Duties and Responsibilities:
ADMINISTRATIVE
Assists in insuring agency compliance with WAC's, RCW's, and other pertinent laws, regulations, and contracts.
Assists in developing and implementing policies and procedures as directed by the Program Director.
Educates staff regarding the process for mental health service referrals and regarding "psycho educational topics” (suicide risk assessment, behavior modification, etc.) as directed by the Program Manager.
Maintains documentation of mental health services, and follow through in patient charts.
CLINICAL DUTIES
Provide developmentally and culturally appropriate formal and informal diagnostic assessments for each patient during the program. Document outcomes, justify diagnoses, and communicate with the collaborative partners regarding outcomes and recommendations. It is suggested that this individual communicate with the Primary Care Physicians, Substance Abuse Counselors, schools, case workers, or others involved in the care of the patient.
Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, specifically children and families.
Provide information about assessments and involve family members as active participants in the assessment process and in treatment planning, where appropriate.
Collaborate with other professionals in conducting family-centered assessments and treatment planning, when appropriate.
Demonstrate knowledge of the principles of growth and development over the life span.
Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients.
Refer patients to psychiatric and medication management services as appropriate. Gather record materials and background information as needed by the community mental health center (See Policies and Procedures). Actively collaborate with the psychiatric service provider to insure accuracy of presentation and that identified needs are addressed. Provide monitoring regarding the effects of medications, and feedback to the psychiatrist regarding effects/side effects.
Provide individual and/or family counseling, in coordination with other system as needed.
Provide suicide risk assessment and develop treatment plans to address suicidal ideation or gestures. Use case management as needed to provide follow through and build community support as follow through. Provide other crisis stabilization as needed.
Gather information regarding past mental health services to be used in the individual's treatment plan development. Provide mental health services referrals to patients prior to or following admission, and exchange information with outside service providers.
Support other staff regarding mental health needs of staff or patients. Maintain confidentiality, prevent splitting between staff and patients, and offer concrete ideas on handling challenging situations.
PRODUCTIVITY STANDARD
Produce a minimum of 80 encounters that will equal at least 80 hours of face-to-face contact.
Qualifications:

An understanding of the Behavioral Health Organization, County, and/or MCO contracts with the ability to tier clients in a managed care system. The ability to work effectively with all persons and groups with an open mind towards cultural differences and knowledge of cultures.
The ability to accurately diagnose mental health issues and develop treatment plans based upon diagnoses. The ability to NOT diagnose behavioral problems as mental illnesses unless appropriate.
An understanding of chemical dependency treatment and an ability to coordinate mental health services with chemical dependency treatment providers if necessary.
Must complete an annual clinical competency test as a mental health therapist.
The Child and Family Clinical Therapist must sign a permanent oath of confidentiality covering all patient related information.
This person must pass a background check.
Education and Experience:
A Master's degree from an accredited college or university in psychology, counseling, or social work, which includes course-work in psychological diagnostics.
For a licensed therapist, must have a license in Marriage and Family Therapy (LMFT), Mental Health Counseling (LMHC), or Social Work (LICSW or LASW).
If not licensed, must hold and maintain an active Washington State Agency Affiliated Counselor License or mental health Associate License (includes Marriage and Family Therapist, Mental Health Counselor, and Clinical Social Worker).
Must possess a Child Mental Health Specialist credential or be working towards credential.
MUST meet WAC requirements for a Mental Health Professional.
Computer proficiency with Cerner and MS Office and other Medical software.
Experience with evidence based models and wrap around services preferred.
Experience working with children and families required.
Bilingual Spanish/English highly desirable.

Duties and Responsibilities:
Follows Universal Precautions against biohazard contamination during blood drawing, handling, preservation and transportation of body fluids, keeping area clean and disinfected, disposing of contaminated material in proper containers, using personal protective equipment (PPE).

Requires by HIPAA to protect the privacy and security of the Personal Health Information to which you have access and follow OSHA and CLIA regulations.
Assist support personnel by assisting them during the day across various departments as requested, and working overtime hours as necessary.
EKG:
Prepares room for the patient.
Prepares the patient with cape, drape sheet or other supplies needed to maintain modesty and dignity.

Spirometry:
Run the calibration of the equipment every 24 hrs or day of use.
