Role Overview:
The RN conducts high-quality, timely in-person clinical assessments in members' homes and in community settings across Massachusetts. Using clinical judgement, the position will complete intake assessments, notating SDOH needs, while maintaining compliance with MassHealth and Medicaid, SNP member guidelines. This role identifies unmet medical, behavioral, and social needs and collaborates with internal care teams to drive appropriate interventions. The position requires strong clinical judgment, field-based experience, and comfort with electronic health record systems.
Responsibilities:
Conduct in-person assessment visits and clinical exams, including relevant screenings within assigned territory.
Perform medication coaching and reconciliation, identify adherence concerns, and provide patient education on acute and chronic diseases.
Complete screeners such as PHQ-9, EASI, Audit, and DAST, and Minimum Data Set- Home Care as needed.
Prioritize and escalate members with urgent needs to RN Supervisor or manager promptly.
Use electronic health record and claims systems to document and track member interactions and care needs.
Ensure timely referrals to behavioral health and long-term services as appropriate.
Manage scheduling and tracking of assessments to ensure timely completion.
Participate in quality improvement activities and maintain clinical competencies.
Maintain effective communication and a constructive work environment with team members.
Comply with organizational policies, privacy regulations, and documentation standards.
Provide clinical assessments via telehealth when appropriate.
Continuously assess member's engagement within the designated care pathway, and determine program fit.
Collaborate with care, decision makers to ensure program alignment.

Requirements
Qualifications & Skills:
Associate Degree in Nursing required; Bachelor's in Nursing preferred.
RN license in good standing in the Commonwealth of Massachusetts.
Minimum of 1 year clinical experience (including clinical rotations).
Experience working in HIPAA-compliant or healthcare settings.
Experience with electronic medical records (EMR).
Prior experience in care management or managed care.
Ability to conduct independent assessments and escalate clinical concerns appropriately.
Strong organizational, communication, and interpersonal skills.
Intermediate to advanced proficiency in Microsoft Office and EHR platforms.
Able to work independently in the field and navigate shifting priorities.

The CLCM collaborates and communicates with the client's caregivers/family support persons, other providers, and others in the Care Team to promote wellness, recovery, independence, resilience, and member empowerment, while ensuring access to appropriate services and maximizing member benefit.
This is a hybrid position that requires traveling throughout the Sacramento County area up to 5 days per week. Candidates wishing to be considered must reside within 25-miles of the assigned territory due to frequency of travel.
What you'll do
Hybrid (in-person and remote) care management duties as described below:
Assess member needs in the areas of physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports
Oversees the development of the client care plans and goal settings
Offer services where the member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services
Connect clients to other social services and supports that are needed
Advocate on behalf of the client with health care professionals (e.g. PCP, etc.)
Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
Conduct outreach and engagement activities in order to facilitate linkage to the ECM program and log activity in the Client Relationship Management (CRM) system
Evaluate client's progress and update SMART goals
Provide mental health promotion
Arrange transportation (e.g., ACCESS)
Complete all documentation, including outcome measures within the timeframes established by the individual care plans
Maintain up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems
Complete monthly reporting to ensure program compliance
Attend training as assigned

Active California Licensed Vocational Nurse (LVN) license required
Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely
2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
Bilingual, Spanish/English, required
Working knowledge of government and community resources related to social determinants of health

At least Three (3) years of experience working with young adults (ages 18-25) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service settings(s) care for individuals experiencing homelessness, justice involvement or limited social support
Extensive knowledge of community treatment, support services, and resources
Highly organized and self-motived
Proficient in computers

Qualifications:
Associate's Degree (ADN) or above in nursing
Active and unencumbered Registered Nurselicense
1 year experience in radiology, ICU, or a high-acuity clinical setting preferred
BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications from the American Heart Association
Strong understanding of radiologic and imaging procedures and safety protocols, including the use of contrast media
Proficient in electronic medical record systems
Responsibilities:
Assist radiologists and other medical professionals during diagnostic imaging examinations and interventional radiology procedures
Administer and monitor intravenous contrast agents, ensuring that any allergic reactions or complications are promptly addressed
Collaborate with a multidisciplinary healthcare team to provide optimal patient care before, during, and after radiologic and diagnostic imaging procedures
Educate patients and their families on pre- and post-procedure care, explaining the risks and benefits of radiologic tests and contrast agents
Maintain accurate and timely documentation of all nursing care in accordance with hospital and departmental policies
