Performs medical procedures including, but not limited to: laceration repairs, nail removal, pap tests, injections, splinting and abscess incision and drainage.
Establishes and monitors a medically appropriate level of care for clinic patients.
Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable.
Maintains clinical core competency.
Works with Center Leadership and other staff to ensure operations are consistent with medical policy and professional standards.
Cooperates and participates in the development, implementation and revision of policies affecting medical practice, quality of care and patient satisfaction.
Participates in and assists with in-service and continuing education programs for physicians, nurses and other personnel employed by or associated with Company.
Recommends potential means of growth and development for new and/or existing services within the Center.
Assures compliance with licensing, certification and accrediting bodies.
Participates in development and presentation of education programs for staff.
Represents Company and the clinic as reasonably required at internal and external meetings or events.

What is the Telehealth NursePractitioner role?
We are looking for a full-time NursePractitioner specializing in cardiology and heart failure with experience overseeing complex chronic care programs. As the NursePractitioner leading the GUIDE Program, you'll integrate comprehensive dementia treatment with essential community resources. Your days blend meaningful patient interactions with strategic program building:
Clinical Care (Average 8 Patients Daily, 4 Days/Week)
Provide dementia assessments using validated tools (Clinical Dementia Rating Scale, Functional Assessment Staging Tool)
Develop person-centered care plans addressing root causes: nutritional deficiencies, infections, toxins, vascular health, hormonal imbalances
Work within the Bredesen Protocol framework (comprehensive lifestyle medicine proven to reverse cognitive decline)
See Medicare-eligible patients (65+) with mild cognitive impairment through moderate dementia, typically accompanied by supportive family members
Conduct follow-ups every 6-12 weeks, building healing relationships where patients and families are genuinely grateful
Program Leadership & Community Development (Especially Heavy in First Months)
Deliver presentations at senior centers, assisted living communities, and adult day programs
Build formal partnerships with Area Agencies on Aging, respite providers, meal services, and transportation organizations
Establish CMS-approved contracts with community partners to create comprehensive support infrastructure
Oversee Care Navigators who provide 24/7 helpline support for patients and caregivers
Position the practice as San Diego's destination for evidence-based cognitive health
Conduct provider education and professional networking events

Required Qualifications:

The GUIDE Program You'll Lead
You'll implement nine CMS care delivery domains creating comprehensive support for dementia patients and their caregivers:
Comprehensive Assessment: Dementia staging, medical/psychosocial needs evaluation, caregiver burden assessment (Zarit Burden Interview)
Person-Centered Care Planning: Individualized roadmaps addressing all identified needs
Care Coordination: Interdisciplinary team communication across all providers
Care Navigation: Supervise team members who connect patients to medical care AND community resources (meals, transportation, respite, home health)
24/7 Helpline: Human support (not AI) for patients and caregivers during crises
Caregiver Education & Support: Training programs, support groups, one-on-one guidance
Respite Services: Coordinate Medicare-funded respite care (up to $2,500 annually per patient) giving caregivers critical breaks
Health-Related Social Needs: Screen and address food insecurity, housing instability, transportation barriers, utility difficulties
Community Partnerships: Maintain CMS-approved contracts with respite providers, meal services, transportation organizations, Area Agencies on Aging

We are looking for a highly motivated full-time Bilingual Family NursePractitioner who is willing to think creatively and empathically to help our team change the way people access healthcare.
We are excited to partner with Federally Qualified, Non-Profit Health Centers in California to enable their participation in CalAIM's new Enhanced Care Management Medi-Cal benefit program, which provides long-term, whole-person care coordination, inclusive of behavioral health and social needs supports.
We seek a full-time Bilingual Family NursePractitioner to play a critical role in our whole-person, interdisciplinary care model. This person would be responsible for directly engaging and caring for individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. Focused on building relationships with and providing support to individuals whose quality of life can be improved with the Enhanced Care Management benefit, the Lead Care Manager has lived experience working with these populations, is an empathetic problem-solver, and works closely with our partner clinics, community organizations, and Pair Team's Lead Care Managers and Clinical Team.

What You'll Need
2+ years of clinical experience in serving patients with complex social and healthcare needs and passionate about building a more equitable healthcare system

Technology-savvy and experienced in digital-first healthcare (e.g. telemedicine) and comfortable working in a variety of different tech platforms
Board certification or eligibility in Family Medicine
Comfortable and able to work with patients across the lifespan, from pediatrics to geriatrics
