Formulate and implement survivorship care plans for all curable cancer patients. Identify and initiate referrals to palliative care. Collaborate with the palliative care team for management of patient's ongoing care. Communicates positively and effectively with patients, families, physicians and the multidisciplinary oncology team. Coordinates patient care with PCP and specialty providers when appropriate. Provides detailed education and counseling to patients and families regarding diagnosis, goals of therapy, expectations, and treatment related side effects and management. Develop a comprehensive plan of care in collaboration with the attending oncologist to include selection of appropriate pharmacologic agents, referrals, and end of life care. Inpatient oncology and hematology acute care coverage to include rapid assessment and treatment of acutely ill oncology/hematology patients. Assist in the admission and discharge of patients receiving inpatient chemotherapy in collaboration with the attending oncology physician. Administration of intrathecal chemotherapy in collaboration with interventional radiology. Provide after-hours call coverage as part of a rotating schedule to include phone triage, clinical decision-making, and urgent inpatient/outpatient concerns in conjunction with on-call oncologist.
