Provide quality nutritional care to patients such as the development, implementation and monitoring of Medical Nutrition Therapies.
work collaboratively with the medical staff, patients, families and food service department to ensure the patients' nutrition care plans are implemented and departmental standards are maintained.
Perform nutritional assessments and re-assessments, identifying goals, recommending nutrition interventions, monitoring and evaluating patient progress.
Develop and monitor enteral and/or parenteral nutrition support regimens.
Work closely with the Speech Therapy Department to assist with the progression of textured modified diets to ensure patient safety and optimize oral intake.
Clarify admission orders to ensure nutrition support, diet and oral supplement orders are consistent with the facility's established nutrition support formulary and diet manual.
Consider religious, cultural and ethnic factors in development of nutrition care plans.
Review yearly updates and approves therapeutic diet manual annually. Educates clinical and food service staff on location of the Nutrition Care Manual. Maintains hard copy of simplified diet manual for downtime.
Keeps current with nutritional practices and theories. Maintains current professional registration requirements (i.e., Professional Portfolio, self-development and continuing education). Demonstrates interest in continuing education, academics or specialized certification to further enhance professional skills.
Evaluate potential food/drug interactions, and/or herb/supplement interactions.
Attend interdisciplinary team meetings and communicating progress and special needs.
Participate in developing, planning and standardizing menus.
Perform meal rounds to ensure patients' meal trays are consistent with the prescribed diet and meet standards as to quality, quantity, temperature and appearance.

**Hospital: Select Specialty Hospital - Cleveland Fairhill** Clinical Liaison (RN, RT, PT, OT, or SLP)** **Salary: $70,000 - $90,000 per year, based on experience plus uncapped monthly bonus plan** **Schedule: Full-Time, M-F 8am - 5pm**
**This position will transition patients to both Acute Inpatient Rehabilitation Hospitals and Critical Illness Recovery Hospitals.**
**Position Summary**
Clinical Liaisons perform clinical assessments of referred patients, then counsel with patient families to assist them in making the best care decisions for their loved-one. Liaisons partner with hospital case managers to create smooth patient transfer processes and to help remove payer-related and operational barriers to getting appropriate patients into the best next level of care. This is a salaried position with monthly bonuses. The hours are Monday- Friday with rotating weekend/holiday on-call responsibility.
**Responsibilities**
**Specific Duties**
+ Work with hospital case managers and physicians to identify and transition patients into the next, most appropriate level of care: Critical Illness Recovery or Acute Rehab hospital
+ Educate referral sources on services provided by our hospitals.
+ Demonstrate excellent customer service skills with focus on referrer satisfaction and account retention.
+ Educate referred patients and families on acute rehabilitation and/or critical illness recovery programs.
+ Deliver staff educational programs, hospital tours, meetings, etc. in assigned hospitals.
+ Develop strategies to grow market share in core business lines in assigned hospitals. Creates quarterly goals and objectives and submits to supervisor.
+ Works with VP/Director Business Development to identify opportunities for expansion of strategic business initiatives.
**Qualifications**
**Minimum Education & Experience (Including Licenses)**
+ Current state licensure as a Registered Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech Language Pathologist required.
+ Demonstrated familiarity with hospital admissions procedures.
+ Proven track record in sales and marketing with a least one year experience in a related position, preferred.

Responsibilities
Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. At Enhabit, an SLP may often work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson's disease and dementia.
Qualifications
Must possess a valid state driver's license.
